EXECUTIVE SUMMARY
Recommendation to approve an amendment between Collier County and the Area Agency on
Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida to add Facility Respite
Service to the standard contract. (Fiscal Impact$0)
OBJECTIVE: To continue provision of support services to qualified Collier County Seniors.
CONSIDERATIONS: Collier County Services for Seniors has provided support services to
Collier County's qualified senior citizens for over 30 years through Older Americans Act
programs (OAA). Funding is provided through the Florida Department of Elder Affairs, and
administered by Senior Choices of Southwest Florida. The contract period for OAA programs is
January 1, 2013 through December 31, 2013. The original award and budget amendments were
approved by the BCC on January 22, 2013, Item 16.D.5.
The Amendment to the Older American Act will add "Facility Respite Services". Facility
Respite is the provision of short-term, temporary relief to those who are caring for family
members who might otherwise require permanent placement in a facility outside of the home.
This service will not result in any projected fiscal impact as it would be paid for within existing
funds. This additional service will offer seniors more alternatives to respite care.
GROWTH MANAGEMENT: There are no growth management impacts from this recommendation.
FISCAL IMPACT: There is no fiscal impact resulting from this amendment.
LEGAL CONSIDERATIONS: The standard form amendment provided by Senior Choices of
Southwest Florida was used for each agreement. This item is approved as to form and legality, and
requires majority vote for approval.-JAB
RECOMMENDATIONS: That the Board of County Commissioners approve and authorize the
Chairwoman to sign the amendment and attestation between Senior Choices of Southwest Florida and
Housing Human and Veteran Services.

COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices
of Southwest Florida ("Agency") and Collier County Board of County Commissioners, ("Recipient"), amends agreement
OAA 203.13.
The purpose of this amendment is to add Respite Service to OA3E and revise ATTACHMENT VII Rate Summary.
This amendment shall be effective on July 22,2013. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
