EXECUTIVE SUMMARY
Recommendation to approve amendments between Collier County and the Area Agency on Aging
for Southwest Florida, Inc. d/b /a Senior Choices of Southwest Florida to amend the contracted
service years in the Community Care for the Elderly (CCE), Home Care for the Elderly (HCE) and
Alzheimer's Disease Initiative (ADI) programs for FY 2012/2013
OBJECTIVE: To continue to provide support services to Collier County Services for Seniors frail,
elderly clients.
CONSIDERATIONS: The Services for Seniors program managed within the Housing Human and
Veteran Services (HHVS) Department has been providing support services to Collier County's frail and
elderly citizens for over 32 years through the Community Care for the Elderly (CCE), Home Care for the
Elderly (HCE), and the Alzheimer's Disease Initiative (ADI) grant programs. All three grants are allotted
to the entity holding the Lead Agency title only. The grants are received from the Florida Department of
Elder Affairs (DOEA) and are managed by Senior Choices of Southwest Florida (SCSF). The Lead
Agency is then a grantee of SCSF. These grants enable the County's Services for Seniors Program to
provide in home services to seniors and their caregivers so that they can remain in their homes and live
with independence and dignity.
The Board approved the current grant contract awards to Collier County as the Lead Agency on July 24,
2012, agenda item 16.D. 15. The contracts have a two year term effective July 1, 2012 through June 30,
2014.
Collier County Services for Seniors received funding for year one of the two year continuation contact
term. The contract amount for the first year will expire on June 30, 2013. Pursuant to the amendment
that is being presented, Senior Choices has provided the estimated amount to be allocated to Collier
County Services for Seniors for year two (2013 -2014) of the contact. When the final contract
amendments are received in late June, 2013, staff will submit them and budget amendments for the
Board's approval. This effective date of the amendments is June 6, 2013.
FISCAL IMPACT: .There is no net fiscal impact resulting from this amendment. The budget
amendments for the CCE, HCE and ADI grant amounts were approved by the BCC on May 28, 2012,
Item 16.13.14.
LEGAL CONSIDERATIONS: These are standard form amendments provided by the Area Agency on
Aging for Southwest Florida, Inc. d/b /a Senior Choices of Southwest Florida. This item is approved as to
form and legality and requires a majority vote for approval. — JAB
GROWTH MANAGEMENT: There is no growth management impact from this recommendation.
RECOMMENDATIONS: To approve the CCE, HCE and ADI amendments and attestation statements
between Collier County and the Area Agency on Aging for Southwest Florida, Inc. d/b /a Senior Choices
of Southwest Florida.

This AMENDMENT, entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior
Choices of Southwest Florida, hereinafter referred to as the "Agency," and Collier County Board of County
Commissioners, hereinafter referred to as the "Contractor," amends contract CCE 203.12.
The purpose of this amendment is to add an additional paragraph to Item #4. Contract Amount to read as follows:
The Agency agrees to pay for contracted services according to the terms and conditions of this contract in an
amount not to exceed $661,357.00 for year two, 2013 - 2014, or the rate schedule, subject to the availability of
funds. Any costs or services paid for under any other contract or from any other source are not eligible for
payment under this contract.
This amendment shall be effective June 6, 2013.
All provisions in the agreement and any attachments thereto in conflict with this amendment shall be and are
^hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level
specified in the agreement.
This Amendment, one page, is hereby made a part of this agreement.

This AMENDMENT, entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior
Choices of Southwest Florida, hereinafter referred to as the "Agency," and Collier County Board of County
Commissioners, hereinafter referred to as the "Contractor," amends contract HCE 203.12.
The purpose of this amendment is to add an additional paragraph to Item #4. Contract Amount to read as follows:
