EXECUTIVE SUMMARY
Recommendation to approve the submittal of a Volunteer Fire Assistance Grant Application to
purchase Class A Foam for the Isles of Capri Fire Rescue District requesting $1,260 in grant funds
and requiring a local match of$1,260.
OBJECTIVE: To receive approval from the Board of County Commissioners to submit a
Volunteer Fire Assistance Grant Application to fund the purchase of Class A Foam for the Isles
of Capri Fire Rescue District.
CONSIDERATIONS: Isles of Capri Fire Rescue District has received the Volunteer Fire
Assistance Grant from the Florida Forest Service in past years, including $4,900 awarded in
2012 and $4,680 awarded in 2011. In past years this grant has been used to purchase protective
clothing, firefighting foam, VHF radios, and a portable skid unit. This year grant funds are
proposed to be used for the purchase of 36 five-gallon pails of Class A Foam which will enable
the Isles of Capri Fire Rescue District to increase its fire suppression capabilities during the
upcoming brush fire season. During this period of budget constraints, obtaining Class A Foam
through grant resources economically increases the Isles of Capri Fire Rescue District's fire
protection power.
FISCAL IMPACT: The total cost of the Class A Foam is $2,520. A 50/50 match is required.
The Florida Forest Service will pay $1,260 leaving the Isles of Capri Fire Rescue District to
provide $1,260 from Reserves Fund 144 (Isles of Capri Fire Rescue). If awarded, a subsequent
item will be brought before the Board to accept the grant and authorize the necessary budget
amendments.
LEGAL CONSIDERATIONS: The Board will have the opportunity to accept or reject the funds
if the grant is approved. Accordingly, this Office has no issue with respect to the legal
sufficiency of this request, which is appropriate and requires a majority vote for Board action. -

GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact resulting from
this action.
RECOMMENDATION: To approve the submittal of the Volunteer Fire Assistance Grant
Application and authorize the Chairman to sign all documents comprising the application.

This certification is required by the regulations implementing Executive Order 12549,Debarment and Suspension,

INSTRUCTIONS FOR CERTIFICATION
By signing and submitting this form,the prospective primary participant is providing the certification set out
on the reverse side in accordance with these instructions.
2. The inability of a person to provide the certification required below will not necessarily result in denial of
participation in this covered transaction. The prospective participant shall submit an explanation of why it
cannot provide the certification set out on this form. The certification or explanation will be considered in
connection with the department or agency's determination whether to enter into this transaction. However,
failure of the prospective primary participant to furnish a certification or an explanation shall disqualify such
person from participation in this transaction.
3. The certification in this clause is a material representation of fact upon which reliance was placed when the
department or agency determined to enter into this transaction. If it is later determined that the prospective
primary participant knowingly rendered an erroneous certification,in addition to other remedies available to
the Federal Government,the department or agency may terminate this transaction for cause or default.
4. The prospective primary participant shall provide immediate written notice to the department or agency to
whom this proposal is submitted if at any time the prospective primary participant learns that its certification
was erroneous when submitted or has become erroneous by reason of changed circumstances.

6. The prospective primary participant agrees by submitting this form that,should the proposed covered
transaction be entered into, it shall not knowingly enter into any lower tier covered transaction with a person
who is debarred,suspended,declared ineligible,or voluntarily excluded from participation in this covered
transaction,unless authorized by the department or agency entering into this transaction.
7. The prospective primary participant further agrees by submitting this form that it will include this clause
titled"Certification Regarding Debarment,Suspension,Ineligibility and Voluntary Exclusion-Lower Tier
Covered Transactions,"provided by the department or agency entering into this covered transaction, without
