EXECUTIVE SUMMARY
Recommendation to approve Amendment #0002 to the Criminal Justice, Mental Health and
Substance Abuse Reinvestment Grant Agreement with the Department of Children and Families
and the associated Subrecipient Agreement with NAMI of Collier County, Inc. to reflect a staffing
change to support a .75 Peer Counselor position provided by NAMI of Collier County and add
State and Federal Laws, Rules and Regulations.
OBJECTIVE: To maintain compliance with Department of Children and Families grant standards.
CONSIDERATIONS: The Criminal Justice Mental Health Substance Abuse (CJMHSA) program was
established by the State to provide funding to counties. These funds can be used to plan, implement, or
expand initiatives that increase public safety, avert increased spending on criminal justice, and improve
the accessibility and effectiveness of treatment services for adults and juveniles who have a mental
illness, substance abuse disorder, or co- occurring mental health and substance abuse disorders and who
are in, or at risk of entering, the criminal or juvenile justice systems.
On June 10, 2014, the Board approved an Agreement with the Florida Department of Children and
Families (DCF) to implement a three -year CJMHSA Reinvestment Grant Program and approved the
associated Subrecipient Agreement with NAMI of Collier County, Inc. (Agenda Item 16D9). The
agreements became effective July 1, 2014 and expire May 31, 2017. On September 23, 2014 Amendment
No. 1, incorporating grantor required language changes was approved by the Board (Agenda Item
16D11).
Under the Subrecipient Agreement, Community and Human Services is funding a Peer Counselor
employed by NAMI using the CJMHSA grant. The peer counselor provides services such as resource
coordination, supported employment, motivational interviewing, supported housing, group counseling,
and assistance with the attainment of disability benefits. After a review of the definition of an FTE (full -
time equivalent) with the grantor agency, it was detennined that the nature of the Peer Counselor position
allows for .75 to be considered full -time. The revision will allow the subrecipient to invoice for all peer
counselor services provided from July I forward. However, the reduction is considered a substantive
change and requires an amendment to the master Agreement with the Department of Children and
Families and a subsequent amendment to the Subrecipient Agreement with NAMI. In addition to the
position change, DCF has added State and Federal Laws, Rules and Regulations. Amending the
agreements to reflect a .75 Peer Counselor position as well as the addition of State and Federal Laws,
Rules and Regulations will enable the county and Subrecipient to maintain compliance with the grantor
requirements and provide adequate staff to allow for uninterrupted support and services to the clients of
Collier County.
FISCAL IMPACT: The proposed action has no new fiscal impact. The funding source for the grants is
DCF and ad valorem funds. Funds are budgeted within Human Services Grant (707), Project 33332, and
Collier County Seniors (123), Project 32001.
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated with this
Executive Summary.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a majority vote

THIS AMENDMENT, entered into between the State of Florida, Department of. Children and
Families, hereinafter referred to as the "Department," and Collier County Board of County
Commissioners, hereinafter referred to as the "Provider," amends Grant Agreement # LHZ46.
This amendment revises the peer counselor staffing FTE. and adds an Exhibit A2 to capture all
applicable state and federal laws, rules and regulations.
1. Page 11, CF Integrated Contract 2014, the sentence immediately above the signature
block, Is hereby amended to read:

All provisions not in conflict with this amendment are still in effect and are to be performed at the
level specified in the contract.
This amendment and all its attachments are hereby made a part of the contract.

SECOND AMENDMENT TO AGREEMENT CJMHSA -003 BETWEEN COLLIER COUNTY
AND
NAMI OF COLLIER COUNTY, INC.
This Amendment, is entered into this day of 12014,
by and between NAMI of Collier County, Inc. a private not- for - profit corporation existing under
the laws of the State of Florida, herein after referred to as Subrecipient and Collier County,
Florida, herein after to be referred to as "COUNTY," collectively stated as the "Parties."
WHEREAS, on June 10, 2014, the County and Subrecipient entered into an agreement
for Subrecipient to provide "FIRST grant services" to Collier County residents (hereinafter
referred to as the "Agreement "); and
WHEREAS, the Parties desire to modify the Agreement to reflect a staffing change to
support a .75 Peer Counselor position by NAMI of Collier County and add State and Federal
Laws, Rules and Regulations.
NOW, THEREFORE, in consideration of foregoing Recitals, and other good and
valuable consideration, the receipt and sufficiency of which is hereby mutually acknowledged,
the Parties agree to modify the Agreement as follows;
