Dear Anna Smith Bradley:
'Phis is to notify you that the site at the aforementioned project is ready for the installation of your underground electric distribution facilities:
1. The underground cable route has been cleared of trees stumps and all obstructions.
2. The cable route has been filled or cut to within 6"of final grade.
3 Grade stakes have been set along the cable route marked to indicate final grade.
4. Lot lines and corners have been staked as you requested for reference to locate the cable route.
5. Any grade or reference stakes found missing will he replaced by our surveyors at your request.
6. All flooded areas have been drained.
7. All underground facilities have been staked within 2 feet of their location along the cable route.Stakes are marked with depth.

8. The FPL transformer pads,splice boxes,and handholes have been installed at final grade,that they do not sit in drainage or swale
areas and are equal to or above road grades.
9. 2"conduit has been installed with 36"minimum cover,and 6"conduit at 42"minimum.Minimum clearances of 36"from gas
and 12"from all other utilities have been maintained. String has been pulled from end to end and labeled accordingly..
10. Customer conduit(secondary)has been installed in the secondary window of the transformer pads
I I. It is agreed that FPI.Company will not be held responsible for the removal from the job site of any excavated rock that cannot he
used for backfill during the trench restoration process. It is further agreed that suitable fill will be made available at no cost to
FPL Company at the job site for trench restoration should the amount of removed rock require it
12. Landscaping is(and will)adhere to the guidelines set by UN-21.
