EXECUTIVE SUMMARY
Recommendation to approve an after-the-fact Amendment to the Master Contract and Attestation
Statement with Area Agency on Aging of Southwest Florida, Inc. for Federal- and State-funded
Services for Seniors programs.
OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors' elderly
clients.
CONSIDERATIONS: The Community and Human and Services Department's Services for Seniors
program has provided support to Collier County's frail and elderly citizens for over thirty-two years
through Federal and State of Florida grants. These grants include the Federal Older Americans Act
(OAA) Title III-B, III-E, III-C1, and III-C2 programs, and the Florida Community Care for the Elderly
(CCE), Alzheimer's Disease Initiative (ADI), and Home Care for the Elderly (HCE) programs. These
programs are funded by the Florida Department of Elder Affairs (DOEA) through the Area Agency on
Aging of Southwest Florida, Inc. ("Agency"), and provide for services that enable clients to remain in
their homes and live with independence and dignity.
Historically, the Agency provided separate standard contracts for the Federal and the State grants. The
Agency has now provided a Master Contract which establishes standard terms and conditions for services
for both the Federal and the State grant programs. This is a three-year contract effective July 1, 2014
through June 30,2017.
On July 8, 2014, the Board approved the Master Contract for Federal and State Programs (Agenda item
16D15). The Amendment to the Master Contract proposes to change the due date of the Surplus/Deficit
Report in Paragraph 54.6 to the 18th of each month. This amendment shall be effective September 17,
2014.
This item is being presented after-the-fact because Collier County received the grant agreement on
October 27, 2014 from the grantor agency and is required to return a signed agreement within 30 days.
Pursuant to CMA 5330 and Resolution No. 2010-122, the County Manager authorized Stephen Y.
Carnell, Public Services Administrator, to sign the amendment. Collier County, as the Lead Agency, is
responsible to respond to seniors' needs and to manage the spending authority for the Federal and State
funded Services for Seniors programs.
FISCAL IMPACT: There is no fiscal impact associated with this action. Funding from the
Department of Elder Affairs via the Area Agency on Aging under the 2014 grant award OAA
203.14, CCE 203.14, ADI 203.14, HCE 203.14 and NSIP 203.15 is available in Human Services
Grant Fund 707 and Human Services Match Fund 708, Projects 33306 (3B), 33307 (3E), 33308
(C1), and 33309 (C2), 33336(CCE), 33337 (ADI), 33338 (HCE, and 33382 (NSIP).
LEGAL CONSIDERATIONS: The amendment to the Master Contract is standard form provided by the
Agency. This item is approved for form and legally and requires a majority vote for Board action-JAB
GROWTH MANAGEMENT: There is no growth management impact associated with this action.
RECOMMENDATIONS: That the Board of County Commissioners approves the after-the-fact
Amendment to the Master Contract and Attestation Statement with Area Agency on Aging for Southwest
Florida,Inc. for the Federal and State revenue funded grants.

MASTER CONTRACT FOR
FEDERAL AND STATE PROGRAMS
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest 'lorida, Inc. ("Agency") and
Collier County Board of County Commissioners, ("Contractor"), amends agreement HM203.14.
The purpose of this amendment is to change the effective due date of the Surplus/Deficit Report,the"Lead Agency
Spending Report."

Paragraph 54.6 of the Master Contract is hereby amended to read:
54.6 Surplus/Deficit Report:
The Contractor will submit a consolidated surplus/deficit report in a format provided by, the Agency to the Agency's
Contractor Manager by the 18th of each month, "Lead Agency Spending Report." This report is for all contracts or
agreements incorporating this Master contract by reference between the Contractor and;he Agency.
This amendment shall be effective September 17, 2014. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
