EXECUTIVE SUMMARY
Recommendation to approve an Amendment and an Attestation Statement with Area Agency on
Aging for Southwest Florida,Inc. d/b/a Senior Choices of Southwest Florida for the Nutritional
Services Incentive Program (NSIP)to ensure grant compliance.
OBJECTIVE: To provide uninterrupted support services through the Senior Nutrition Program to
provide meals to elderly clients.
CONSIDERATIONS: In January 2008, the Housing Human and Veterans Services Department
Services for Seniors program received approval from the Board to begin administering the Senior
Nutrition Program. The program is partially funded through the Federal Nutritional Services Incentive
Program (NSIP) and currently consists of congregate meal sites in East Naples, Golden Gate, Goodlette
Arms senior apartments, and the Roberts Center in Immokalee. Home-delivered meals are also provided
to frail,homebound elderly clients through this grant program.
On October 22, 2013, the Board approved the current grant contract with Senior Choices (Agenda item
16D14). The contract term is October 1, 2013 through September 30,2014.
Senior Choices has changed the Service Cost Report requirement from semi-annual to annual. This report
provides information to Senior Choices for planning and negotiating unit reimbursement rates for
services. Upon execution of this amendment,the Service Cost Report will be due annually on August 15.
This change does not affect the overall program awards.
FISCAL IMPACT: There is no new fiscal impact associated with this action. Funding resides in Human
Services Grant Fund 707,NSIP project 33302.
GROWTH MANAGEMENT: There is no growth management impact associated with this action.
LEGAL CONSIDERATIONS: This is a standard form amendment provided by the Area Agency on
Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida. This item has been
approved for form and legality and requires a majority vote for Board approval.—JAB
RECOMMENDATIONS: That the Board of County Commissioners approves and authorizes the
Chairman to sign the Amendment and an Attestation Statement with Area Agency on Aging for
Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Nutritional Services Incentive
Program to ensure grant compliance.

NUTRITION SERVICES INCENTIVE PROGRAM
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices
of Southwest Florida("Agency") and Collier County Board of County Commissioners, ("Recipient"), amends agreement
NSIP 203.14.
The purpose of this amendment is to amend Paragraph 2.4.3., of the ATTACHMENT I.
Line denotes completion of above summary
ATTACHMENT I:
Paragraph 2.4.3 of the Attachment I, is hereby amended to read:
2.4.3. Service Costs Reports
The Recipient shall require subcontractors to submit to the Recipient annually service cost reports, which will
be due August 15 of each year and reflect actual costs of providing each service by program. This report
provides information for planning and negotiating unit rates.
This amendment shall be effective on the last day that this agreement is signed by both parties. All provisions in the
agreement and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with
this amendment
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.

This amendment and all of its attachments are hereby made a part of this agreement.
