EXECUTIVE SUMMARY
Recommendation to approve an Amendment and an Attestation Statement with Area Agency on
Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Older
Americans Act Program Title III to ensure grant compliance. There is no new fiscal impact
associated with this action.
OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors elderly
clients.
CONSIDERATIONS: Collier County Services for Seniors has provided support services to Collier
County's qualified senior citizens for over thirty years through Older Americans Act (OAA) programs.
Funding is provided through the Florida Department of Elder Affairs, and administered by Senior Choices
of Southwest Florida. The contract period for OAA programs is January 1, 2014 through December 31,
2014. The original award and budget amendments were approved by the Board on February 11, 2014
(Agenda item 16D5).
The Amendment to the Older American Act contract will revise the Invoice Schedule. The Invoice
Schedule is a time scheduled method of advances and expenditure reports for services provided. This
change does not affect the overall program awards.
FISCAL IMPACT: There is no new fiscal impact associated with this action. Funding resides in Human
Services Grant Fund 707 and Human Services Match Fund 708, 3B project 33306, 3E project 33307, Cl
project 33308 and C2 project 33309.
GROWTH MANAGEMENT: There are no growth management impacts from this recommendation.
LEGAL CONSIDERATIONS: The standard form amendment provided by Senior Choices of
Southwest Florida was used for each agreement. This item is approved as to form and legality, and
requires majority vote for approval.-JAB
RECOMMENDATIONS: That the Board of County Commissioners approves and authorizes the
Chairman to sign the Amendment and an Attestation Statement with Area Agency on Aging for
Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Older Americans Act Program
Title III program to ensure grant compliance.

OLDER AMERICANS ACT PROGRAM TITLE III
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc., dba Senior
Choices of Southwest Florida(Agency) and Collier County Board of County Commissioners (Contractor) and
amends contract OAA 203.14.
The purpose of this amendment is to (1) amend language in Paragraph 3.2.2 of ATTACHMENT I, and (2) update
ATTACHMENT VIII.

ATTACHMENT I:
(1) Paragraph 3.2.2 of ATTACHMENT I is hereby amended to read:
3.2.2 All advance payments made to the Contractor shall be returned to the Agency as follows: one-
tenth of the advance payment received shall be reported as an advance recoupment on each request
for payment, starting with report number five,in accordance with the Invoice Schedule,
ATTACHMENT VIII to this contract.
ATTACHMENT VIII:
(2) ATTACHMENT VIII is hereby replaced with the revised ATTACHMENT VIII and attached hereto.
This amendment shall be effective on the last day that this agreement is signed by both parties. All provisions in the
agreement and any attachments thereto in conflict with this amendment shall be and are hereby changed to conform with this
amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.

Note# 1: Report#1 for Advance Basis Agreements cannot be submitted to the Agency prior to January 1 or until

each of the reports,repaying advances issued the first two months of the agreement. The adjustment shall
be recorded in Part C, 1 of the report(ATTACHMENT IX).
Note#3: Submission of expenditure reports may or may not generate a payment request. If final expenditure
report reflects funds due back to the Agency, payment is to accompany the report.
Note#4: ALL Expenditure Reports are due by 12:00 p.m. on the 9th of each month. IF the 9th falls on a Saturday,
then the report will be due by the 8th by 12:00 p.m. AND IF the 9th falls on a Sunday,the report will be
due by the 10`h by 12:00 p.m. Actual submission of the vouchers to Dept. of Elder Affairs is dependent
on the accuracy of the expenditure report which is verified and paid by CIRTS data only.
