5. The Chairman's signature line date has been entered as the date of BCC approval of the MD
document or the final negotiated contract date whichever is applicable.

7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 2-27-18,and all changes made during MD WA is not

LEASE AMENDMENT TO LEASE NO. 57-4209-15-003

THIS AGREEMENT, made and entered into this date by and between Collier County Government,whose address is 3335 E
TAMIAMI TRAIL,SUITE 101, NAPLES FL 34102 hereinafter called the Lessor, and the UNITED STATES OF AMERICA,
hereinafter called the Government:
WHEREAS, the parties hereto desire to amend the above Lease:
Now therefore,these parties for the considerations hereinafter mentioned covenant and agree that the said Lease is amended,
effective upon execution by the Government as follows:
The lease expiration date is terminated on December 1 2017.
The Collier County leadership has deemed this building untenable due to the severe storm damage that occurred during
Hurricane Irma,on September 11,2017.After much research it is determined the repairs could take in excess of 12 months and
possibly not at all.
The Government is seeking relocation of the files, furniture and all Government owned property and will retain access until this is
complete not to exceed January 15, 2018 No additional funds will be allocated toward this lease. Both parties have agreed to
the above.
All other terms and conditions of the Lease remain in force and effect and a current Sam.gov registration must be
maintained for the life of the lease.
This Lease Amendment contains 1 page.
All other terms and con.'.ions of the lease shall remain in force and effect.
