EXECUTIVE SUMMARY Recommendation to approve Amendments and Attestation Statements with Area Agency on Aging for Southwest Florida, Inc. d /b /a Senior Choices of Southwest Florida for the Home Care for the Elderly Program to ensure continuous funding for FY 2013/2014 and to approve a budget amendment recognizing an additional $424 in grant revenue. OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors frail, elderly clients. CONSIDERATIONS: The Housing, Human, and Veteran Services' Services for Seniors program has provided support to Collier County's frail and elderly citizens for over thirty-two years through the Home Care for the Elderly (HCE) grant program. This grant is funded by the Florida Department of Elder Affairs (DOEA) through the Area Agency on Aging of Southwest Florida, Inc. d /b /a Senior Choices of Southwest Florida (Senior Choices). The grant funds services that enable seniors to remain in their homes and Iive with independence and dignity. On July 24, 2011 the Board approved the current grant contracts with Senior Choices (Agenda item 16D15). Numerous amendments have been approved by the Board. The contracts have a two -year term, effective July 1, 2012 through June 30, 2014 with 2 two -year renewals. An amendment to the Home Care for the Elderly (HCE) agreement was received from Senior Choices increasing Special Subsidy funding by $1,692 through transfers of $1,268 from Case Management funding and $424 from Basic Subsidy funding, with an effective date of February 13, 2014. The Basic Subsidy budget is retained and directly administered by Senior Choices for the benefit of qualified Collier County clients; therefore the budget administered directly by Collier County will realize a net increase of $424. The shift reflects fluctuations in the needs of the clients, and in the case workers' client load which routinely results from the loss or addition of clients. FISCAL IMPACT: The actions proposed in this executive summary have a net fiscal impact of $424. The funding source for the grant awards is the Florida Department of Elder Affairs. The award amount for the HCE program is $35.875, of which $19.657 is managed by the County. A budget amendment is required to recognize increased grant revenue of $424 for the 2013 -14 HCE Program within Human Services Grant Fund (707), Project 33272.
