Recommendation to approve Change Order No, 2 to Work Order#4500158367 for additional sixty(60) days with Hole*antes, Inc. for construction services for Kirkwood Alley Stormwater Improvement Project. EXECUTIVE SUMMARY Recommendation to approve Change Order No. 2 to Work Order#4500158367 for additional sixty (60) days with Hole Montes, Inc. for construction services for Kirkwood Alley Stormwater Improvement Project. OBJECTIVE: To receive Board of County Commissioners (Board) approval to add 60 days to Work Order #4500158367 to allow Hole Montes, Inc. to complete services during construction for Kirkwood Alley Stormwater Improvement Project. CONSIDERATIONS: On May 19, 2015, a Notice-To-Proceed was issued to Hole Montes, Inc. (consultant) to provide construction services for the Kirkwood Alley Stormwater Improvement Project located in the Gateway Triangle Area. These services include meetings, shop drawing reviews, on-site construction observation, review pay requests, and preparation of record drawings and certifications to regulatory agencies. Prior to construction, special materials needed to be ordered. Shop drawings for customized piping (Duras lot Surface Drains) needed to be reviewed and approved by the consultant. Approximately twenty-five(25) days were spent during this review process. Staff and consultant underestimated the time needed for reviews and approval of the shop drawings for these special items. Construction of the project commenced on July 6, 2015. During early stages of construction, the contractor found a major utility conflict involving a gas main line not shown in the plans. This gas main will need to be relocated by the gas company in order for the stortnwater infrastructure to be installed. County staff will need the assistance of the consultant during this time, which will be approximately twenty-one(21) days. Staff has issued a zero-dollar change letter to increase time (change order No.1), but now due to the unforeseen conflict with the gas line, more time will be needed for the consultant to provide construction services. Staff is requesting sixty(60) additional days be added to the existing Work Order to compensate for the time spent on pre-construction shop drawing reviews, and the time that will be spent on resolving the conflict with the gas main. Approval of the following documents by the County Manager is subject to formal ratification by the Board of County Commissioners. If the decision by the County Manager is not ratified by that Board, the document(s) shall be enforceable against Collier County only to the extent authorized by law in the absence of such ratification by that Board. FISCAL IMPACT: There is no fiscal impact associated with item since this is a request for additional time only. LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney, is approved as to form and legality,and requires majority vote for approval. -JAK GROWTH MANAGEMENT IMPACT: Approval of this item is in accordance with the goals and objectives of the Drainage Sub-element of the Growth Management Plan. Utility Conflicts Attachments: Kirkwood Alley Utility Conflicts Rev2.pdf Liz, We have the storm structure onsite now and have investigated more into the gas main's horizontal conflict with the storm box and have found that the existing gas main will be 6"into the junction box. I can think of a few options: 1. Have the gas main relocated 2. Have the gas main go through the box in a casing. 3. Re-lay the existing 36"RCP pipe that runs north-south shifting it away from the gas main and just east of the force main.Setting the new J.B.2'-3'west of its proposed location. 4. Use the FOOT index 280—'Concrete collar for joining mainline pipe and stub pipe' The project is currently on hold until we get direction from the County. I still do not have clear direction on the two existing cable conflicts. Attached is a sketch that shows some existing utilities all of which were not shown on the original plan. Utility Conflicts Jerry, We have found a couple issues with some existing utilities along Kirkwood Alley that conflict with the slot drain installation. I have attached a PDF sketch for clarification. 1. The gas main is going to be extremely close to the box we set.I'm not sure the separation required by TECO or Collier County but unless the gas main is moved I expect to have 0"-12" between the gas main and the new junction box we set.The good news is that it should be deep enough so there won't be a conflict with connecting to the new slot drain. 2. There is also a direct burry wire(Comcast or century link)that runs across the alley at elevation 2.05 which is a direct conflict with the slot drain. 3. There is a 1"or 1'A orange conduit that also crosses the alley.We found it to be at elevation 1.58 which is a direct conflict with the slot drain. 2 We plan to mill the asphalt Friday 7/10 and start to set then on Monday 7/13.By Tuesday hopefully we will start laying pipe. Unfortunately with the limited contract time we are going to need a resolution fairly quickly. Thank you.
