EXECUTIVE SUMMARY
Recommendation to authorize the Chief of Emergency Medical Services to sign a Lee Memorial
Health System Computer Access Request form(s) and Physician / Office Staff Security
Agreement(s) to obtain medical records from Lee Memorial Hospital System.
OBJECTIVE: To obtain limited computer access to Lee Memorial patient records.
CONSIDERATIONS: As part of the Quality Assurance review of trauma alert patients
transported by Collier County Medflight to Lee Memorial, it is necessary for Emergency
Medical Services personnel to access patient records.
Lee Memorial Health System requires that a formal Computer Access Request form and a
Physician/Office Staff Security Agreement be completed and executed in order to gain access to
Lee Memorial patient records. Execution of these documents will allow Collier County EMS
personnel access to the Lee Memorial System to review Lee Memorial records of patients that
are transported by Collier County EMS.
FISCAL IMPACT: There is no Fiscal Impact.
LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney. I am
advised that Collier County EMS has had for many years computer access to our local hospital
systems, for the purpose of overseeing quality assurance and enhancement of patient care. This
agreement will extend this access to Lee Memorial Trauma Center. This item is approved as to
form and legality, and requires majority vote for approval. -JAK
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact resulting
from this action.
RECOMMENDATION: That the Board of County Commissioners authorizes the Chief of
EMS to sign the attached Lee Memorial Health System Computer Access Request form and
Physician/Office Staff Security Agreement and future forms as required for staff
changes/additions to review Lee Memorial records of patients that are transported by Collier
County EMS.
