Attach additional information from the Design Professional and/or Contractor If needed 1 During Fre and Life Safety inspections,pre-existing.non-compliant conditions were found to be concealed,within the phase I construction space where new Probation offices are being built The openings at the bottom o14 shafts were not sealed,as code requires The shafts begin under the floor slab of the 2nd floor,i.e,the 31 foot high ceiling deck of the 1st floor,and carry HVAC ductwork from the 1st floor,up to the 2nd and 3rd floors The HVAC ducts contained within those shafts do.noi have dampen installed at the required locations where they enter into the shafts This Change Order(CO)is to correct those conditions and any i other unprotected penetrations that are addressed in the specified remedy Similar existing conditions are suspected to be in phase H,space currently occupied by Probation,end which will become a new courtroom.
