Thank you Attachment 1603 FIRST AMENDMENT TO AGREEMENT FOR CHILD ADVOCACY SUPERVISED VISITATION This Amendment, is entered into this o2 to-On day of O�4"04NA.,, 2014, by and between the Collier County Child Advocacy Council, Inc. d/b/a Children's Advocacy Center of Collier County (hereinafter referred to as the "CCCAC") a not-for-profit Corporation, authorized to do business in the State of Florida and Collier County, Florida, herein after to be referred to as "COUNTY," collectively stated as the "Parties." WHEREAS, on December 11, 2012, the County entered into an agreement with CCCAC to provide supervised visitation and safe exchange of children by and between parents in situations involving domestic violence, child abuse, sexual assault, or stalking. (hereinafter referred to as the "Agreement"); and WHEREAS, the Parties desire to amend Paragraph 5, Compensation, and replace Attachment B, Budget Detail Worksheet and Narrative, with Revised Amendment B, Budget Detail Worksheet, and clarify Project Narrative, Goal 1 Objective 1. NOW, THEREFORE, in consideration of foregoing Recitals, and other good and valuable consideration, the receipt and sufficiency of which is hereby mutually acknowledged, the Parties agree to amend the Agreement as follows: 1. Personnel/Fringe The Program Director will oversee operations of the supervised visitation center, provide supervision to the Case Managers, and conduct case reviews. The Program Director will serve as Lead Staff member for the collaborative partnership, reporting the activity of the center. The Case Managers will accept intake referrals, conduct intake interviews, collect necessary reporting statistics, and prepare all reports to the court. The Program Director and Case Managers are employed by CCCAC, and the Victim Advocate is employed by The Shelter for Abused Women and Children. Case Managers are responsible for scheduling and supervising visits and exchanges. The Victim Advocate will meet directly with victims over the course of the grant period to assess safety, conduct safety planning, and receive feedback regarding visits and exchanges, and serve as a liaison between the visitation center staff and the shelter advocates, coordinating victim services. Partner agencies, Legal Aid Society, Collier County Sheriff's Office, David Lawrence Center, and The Shelter for Abused Women and Children, are expressly listed below under the `Consultants' sub-category. The Collier County Sheriff's Office statutorily cannot receive any compensation for these services, as this is within their regular scope of work. Health insurance increases occurred in 2014, and have been included over the applicable 16 month period.
