EXECUTIVE SUMMARY
Recommendation to approve a Substantial Amendment to Collier County's U.S Department of
Housing and Urban Development Annual Action plans for FY2011/2012, FY2012/2013,
FY2013/2014,and FY2014/2015 for the Youth Haven Safe and Secure Homes project in order to
shift funds allocated to repaving activities to the installation of fiber-optic cable and network
equipment.
OBJECTIVE: To enhance affordable housing opportunities in Collier County for low to moderate
income persons.
CONSIDERATIONS: The U.S. Department of Housing and Urban Development (HUD) pro v i des
Community Development Block Grant (CDBG), funds to address program-specific needs in the
community.
On November 18,2014,the Board approved a CDBG Subrecipient Agreement with Youth Haven,Inc.
(Agenda item 16D4). The agreement established funding for the rehabilitation and installation of safety
and security improvements to Youth Haven's campus, in Naples,for the benefit of homeless youth.
The proposed Substantial Amendment will shift funds allocated for repaving activities to the installation
of fiber-optic cable and network equipment, as a result of significantly higher bids compared to the
original estimated costs for the project. The amendment changes the scope of the project by removing the
repaving portion of the project, which is allowable under the grant, and will not increase or decrease the
total amount of grant funds budgeted for this project.
The Second Amendment to the associated Subrecipient Agreement with Youth Haven to reallocate the
repaving activity funds was approved by the Board on September 22,2015 Agenda item 16D6.
This requires a substantial amendment to the FY 2011-2012, FY 2012-2013, FY 2013-2014, and FY
2014-2015 Action Plans. HUD regulations and the Five-Year HUD Consolidated Plan require citizen
participation whenever a substantial plan amendment is proposed.There was one notice published for this
project. The notice was published in the Naples Daily News on February 6, 2016. The advertisement
identified a thirty-day public comment period from February 6, 2016 to March 7, 2016. There were no
comments regarding the substantial amendment.
FISCAL IMPACT: The proposed actions described in this executive summary have no new fiscal
impact. The funding source for the grant award is the Community Development Block Grant. The
project is budgeted in the Housing Grant Fund(705), for Project No. 33176, 33236, 33289 and 33364.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a majority vote
for Board approval. —JAB
GROWTH MANAGEMENT IMPACT: This project furthers the Goals, Objectives, and Policies of the
Housing Element of the Growth Management Plan.
RECOMMENDATION: The Board approve the Substantial Amendment to the various years Action
Plans for the scope changes to the Youth Haven Safe and Secure Home project.

This Amendment is entered into this.2.62. day of .c PTtA1 EK2015, by and between
Youth Haven, Inc., a private not-for-profit corporation existing under the laws of the State of
Florida, herein alter referred to as SUBRECIPIENT and Collier County, Florida, hereinafter to
be referred to as"COUNTY,"collectively stated as the"Parties."
RECITALS
WHEREAS, on November 18' 2014, the COUNTY entered into an Agreement for
awarding Community Development Block Grant Program funds to be used for Safe and Secure
Home for Collier Youth Phase II & III Project (hereinafter referred to as the "Agreement");
and
WHEREAS, the Parties desire to amend the Agreement to clarify milestones deadlines in
order to achieve a National Objective in accordance with 24 CFR 570.208.
NOW, THEREFORE, in consideration of foregoing Recitals, and other good and
valuable consideration, the receipt and sufficiency of which is hereby mutually acknowledged,
the Parties agree to amend the Agreement as follows:
