signed by the Chairman,with the exception of most letters,must be reviewed and signed
by the Office of the County Attorney.
4. All handwritten strike-through and revisions have been initialed by the County Attorney's esm
Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the esm
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's esm
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip esm
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 9/23/14 and all changes made during esm
the meeting have been incorporated in the attached document. The County
Attorney's Office has reviewed the changes,if applicable.

SUBORDINATION OF AFFORDABLE HOUSING DENSITY BONUS LIEN
KNOW BY ALL MEN THESE PRESENT: In consideration of $10.00 and other good and
valuable considerations, Collier County, Florida, hereby agrees that it shall postpone
and make junior and subordinate in all respects that certain lien and/or assessments
created, by and between Collier County, a political subdivision of the State of Florida,
through its Board of County Commissioners, and Thomas E. Moonan, an unmarried
man, recorded on January 23, 2008, in OR Book 4323, Page 2089, Public Records of
Collier County, Florida. Said agreement pertains to the real property described in:
Legal Description: Lot 189, Ave Maria Unit 13, Middlebrooke Townhomes, a
subdivision according to the plat thereof recorded at Plat Book 48, Pages 48
through 53, in the Public Records of Collier County, Florida.
Commonly known as: 5182 Beckton Rd, Ave Maria, Florida 34142.
Said agreement will be subordinate only to the Affordable Housing Density Bonus
Lien executed by, Thomas E. Moonan, an unmarried man, to Suncoast Credit
Union as first lien holder on the 7 1ok-\ day of Se ��,�►.b�,r , 2014.
This agreement shall continue in full force and effect so long as Thomas E. Moonan,
an unmarried man, shall be indebted to the above financial institutions and
government agencies, and/or their assigns.
