continuous funding for FY 2015 (Fiscal Impact$7,000).

Recommendation to approve after-the-fact contract Amendment and Attestation
Statement between the Area Agency on Aging for Southwest Florida, Inc. and Collier
County for Services for Seniors to extend the grant period through February 29, 2016 for
the Older Americans Act Programs and allow for additional in-home reimbursement rates
(No Fiscal Impact).

AREA AGENCY ON AGING FOR SOUTHWEST FLORIDA,INC.
MASTER CONTRACT
FOR FEDERAL AND STATE PROGRAMS
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. ("Agency") and
Collier County Board of County Commissioners("Contractor"), amends Master Contract HM203.14.
The purpose of this amendment is to add language.

1. Section 5.7 is hereby added:

5.7 Cooperation with the Inspector General
Contractor agrees to comply with the inspector general in any investigation, audit, inspection, review, or
hearing performed pursuant to Section 20.055, Florida Statutes.
The Contractor further agrees that it shall include, in related subcontracts, a requirement that subcontractors
performing work or providing services pursuant to any State Contract that incorporates this Master Contract by
reference agree to cooperate with the inspector general in any investigation, audit, inspection, review, or
hearing performed pursuant to Section 20.055,Florida Statutes.
2. Section 23.3 is hereby replaced:
23. Subcontracts:
23.3 Service Cost Reports — The Contractor shall require Subcontractors to annually submit to the Contractor
service cost reports, which reflect actual costs of providing each service by program. Any multi-year contracts
entered into with service providers on or after the effective date of this contract shall contain a provision
requiring the contract's parties to re-evaluate the contract's reimbursement rates on an annual basis. The
Contractor may annually renegotiate rates based on factors including but not limited to a review of
sustainability, the respective consumer price index, or current market conditions. However, it is the intent of
the Department and/or Agency that the quality of services provided to current program recipients not be reduced.

This amendment shall be effective December 1, 2015. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
