7. In most cases(some contracts are an exception),the original document and this routing slip JAK
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 6/24/14 and all changes made during the
meeting have been incorporated in the attached document. The County Attorney's
Office has reviewed the changes, if applicable.

RESOLUTION NO. 2014— 1 35
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, APPOINTING MICHAEL P.
BOHAYCHYK AND REAPPOINTING JOHN L. COWAN TO THE
AFFORDABLE HOUSING ADVISORY COMMITTEE.
WHEREAS, on July 31, 1991, the Board of County Commissioners adopted Ordinance
No. 91-65 establishing the Affordable Housing Advisory Committee; and
WHEREAS, Ordinance No. 91-65, as subsequently amended, on April 9, 2013 was
repealed and replaced by Ordinance No. 13-27, which recreated and reestablished the Affordable
Housing Advisory Committee in accordance with F.S. § 420.9076. This Advisory Committee
consists of eleven voting members appointed by the Board of County Commissioners and the
Naples City Council based on residency and professional experience related to affordable
housing; and
WHEREAS, the term of John L. Cowan will expire on September 2, 2014, and there are
also other vacancies on this Advisory Committee; and
WHEREAS, the Board of County Commissioners previously provided public notice
soliciting applications from interested parties; and
WHEREAS, the Advisory Committee has provided the Board of County Commissioners
with its recommendation for appointment.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that JOHN L. COWAN, representing
the City of Naples, is hereby reappointed to the Affordable Housing Advisory Committee for a
three-year term to expire on September 2, 2017, and that MICHAEL P. BOHAYCYK is hereby
appointed in the category of Employers within Jurisdiction to an initial three-year term to expire
on October 1, 2017.
THIS RESOLUTION ADOPTED upon majority vote on this the 24th day of June, 2014.
