EXECUTIVE SUMMARY Recommendation to approve an Amendment and an Attestation Statement with Area Agency on Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Alzheimer's Disease Initiative program to ensure continuous funding for FY 2013/2014 OBJECTIVE: To provide uninterrupted support services to Collier County Services for Senior's frail,elderly clients. CONSIDERATIONS: The Housing, Human, and Veteran Services "Services for Seniors" program'has been providing support to Collier County's frail and elderly citizens for over thirty- two years through the Alzheimer's Disease Initiative (ADI) grant program. This grant is funded by the Florida Department of Elder Affairs (DOEA) through the Area Agency on Aging of Southwest Florida, Inc., d/b/a Senior Choices of Southwest Florida(Senior Choices). The grant funds services that enable clients to remain in their homes and live with independence and dignity. On July 24, 2012, the Board approved the current grant contract with Senior Choices (Agenda item 16D15). The contract has a two-year term, effective July 1, 2012 through June 30, 2014, with 2 two-year renewal options. An Amendment and an Attestation Statement are needed to shift funds between allocation categories within this grant. Within the ADI program, $1,380 is being moved from direct services to case management.This move reflects fluctuations in the case workers' client load that routinely result from the loss or addition of clients. Reallocation of funds will allow Services for Seniors to provide the case management and services based on current and projected program needs. This shift in funding does not affect the overall program award. The effective date of these amendments is February 13, 2014. This item was approved by the Board on March 25, 2014 but the Agreement and Attestation was inadvertently not included in the agenda materials made available to the public. FISCAL IMPACT: The proposed actions described in this executive summary have no new fiscal impact. The funding source for the grant awards is the Florida Department of Elder Affairs. The ADI program award of $136,587 resides in Human Services Grant Fund (707), Project No. 33273. LEGAL CONSIDERATIONS:, This is a standard form amendment provided by the Area Agency on Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida. This item has been approved for form and legality and requires a majority vote for Board approval.—JAB GROWTH MANAGEMENT:There is no growth management impact associated with this action.
