The purpose of this letter is to regretfully state that I must resign from membership on the
Emergency Medical Services Advisory Council (EMSAC). This 3-year appointment expires
August 31, 1999, thereby leaving a twenty month vacancy for the Commissioners to fill.
In that regard, I would like to respectfully request that the Commission consider Ms. Gail Dolan
as my replacement on the Council, effective January, 1998. Ms. Dolan is a long-tenured Vice
President with the NCH Healthcare System and posesses a keen grasp of the issues and challenges
facing the medical community, including those that pertain to the delivery of emergency medical
services. In her role as Vice President, Ms. Dolan is responsible for the operations of the
Emergency Departments at both North Collier Hospital and Naples Community Hospital, Women
& Children's Pavilion at North Collier Hospital and all the ambulatory centers for the System. As
such, I am confident that Gail can bring a wealth of valuable experience and energy to the Council
as an active member.
Please express my sincere appreciation to the Commission for having had the opportunity to work
with the Council this past year and a half. It has truly been an experience I will not forget. If
there is anything I can do to assist the Commissioners in their selection of my replacement, please

JUNE 20, 1997
The Collier County Board of Commissioners is seeking applications for positions on several advisory
committees.
The Black Affairs Advisory Board has several vacancies. This 9 member committee identifies and
evaluates problems unique to the Black Community, reviews and recommends ways to ensure open
communication between the minorities and Collier County Government and provides periodic reports
to the Board of County Commissioners.
The Board of Adjustments and Appeals has 2 terms expiring on August 9, 1997; 1 Architect and 1
Electrical Contractor. This board hears appeals on rejections by the building official as to the
materials or mode of construction used on a structure. Terms are 4 years and meetings are held
quarterly or on an as-needed basis. Members must file a Form 1 Statement of Financial Interests each
year with the Supervisor of Elections.
The Collier County Airport Authority has 4 terms expiring on August 10, 1997, 1 of the 4 shall
represent the Immokalee community and 1 shall represent the Marco Island community. This 7
member authority was created on June 22, 1993, by Ordinance No. 93-36. Members of the Authority
shall be residents of Collier County with business ability and experience in one or more fields which,
by way of example, may include, but are not limited to, general business, economic development,
aviation, public affairs, law, finance, accounting, engineering, natural resource conservation or other
related fields. No person holding elected office at the State, County, or Municipal level shall be
appointed to, or be a member of,the Authority. Members must file a Form 1 Statement of financial
Interests each year with the Supervisor of Elections.
The County Government Productivity Committee has 1 vacancy, expiring on 2/4/99. This 13
member committee reviews the efficiency of departments of County Government under the
jurisdiction of the Board of County Commissioners. Terms are 2 years.
The Emergency Medical Services Advisory Council has 3 terms expiring on August 31, 1997,
representing 2 At-Large members and 1 member representing Commission District 3. This 11
member council advises the Board of County Commissioners on all aspects of emergency medical
services, including the operational budget, level of service and equipment. Terms are 3 years.
The Hispanic Affairs Advisory Committee has 5 vacancies. This 9 member board identifies and
evaluates problems unique to the Hispanic Community, reviews and recommends ways to ensure
open communication between the minorities and Collier County Government and provides periodic
reports to the Board of County Commissioners. Terms are 4 years.
The Marco Island Beach Renourishment Advisory Committee has 3 terms expiring on August 26,
1997. This 9 member committee makes recommendations and aids and assists the Board of County
Commissioners in carrying out the purposes of Collier County Ordinance No. 86-91 relating to beach
renourishment, improved storm protection of beaches, and public access to beaches. The
representation on the committee consists of approximately one-half from co-ops or condominium
association and approximately one-half from the single-family residential areas of Marco Island.
Members are subject to filing a Form 1 Statement of Financial Interests each year with the Supervisor
of Elections. Terms are 4 years.
The Public Vehicle Advisory Committee has 2 terms expiring on September 2, 1997; 1 representing
the affiliated category and 1 representing the non-affiliated category. Three members must be
holders of Certificates to Operate a motor vehicle for hire company in Collier County and two
members who are in no way affiliated or associated with the vehicle for hire business. This 5
member committee reviews and approves; applications for a Certificate to Operate, taxicab color
schemes pursuant to ordinance, and reviews and make recommendations to the Public Vehicle Board
(BCC) regarding issues related to the motor vehicle for hire business and regarding alleged violations
of Ordinance 91-93. Terms are 4 years.
The Collier County Utility Authority has 1 vacancy due to a resignation. This 5 member council was
established on February 27, 1996,by Ordinance No. 96-6 to regulate utility rates and related matters
regarding non-exempt water and wastewater utilities in unincorporated Collier County. Members are
required to file a Form 1 Statement of Financial Interests each year with the Supervisor of Elections.
After initial staggered terms, the terms will be 4 years.
Members of the advisory committees must be residents and electors of Collier County. Residents
interested in applying for the advisory committee positions are requested to submit a letter and brief

The Emergency Medical Services Advisory Council met this week and

consideration. The EMS Advisory Council recommended he be re-appointed
as the representative for District#3.
The EMS Advisory Board also recommended that Mr. Robert Laird be re-
appointed to the Council. Mr. Robert Laird serves as the Chairman of the

The final recommendation by the Advisory Council for appointment by the
Board, was for Mr. George Leamon. Mr. Leamon expressed his interest in

Dear Diane:
Got a real shock when I went through the agenda for the BCC meeting Tuesday. Have I missed
something at an EMSAC meeting? I missed only one meeting, and I don't recall ever discussing
an increase in ambulance fees. Where and when did all this come about, and and why wasn't it
brought up before the advisory committee.
Second question regards why John Yonkosky introduced it, signed by Leo Ochs? You are the
Director and only you should be bringing up fees for ambulance services. Prior to bringing it
before the BCC it certainly should be thoroughly researched.
Third is the question of cost for outlying area that would pay entirely too much. Medicare pays
only $257, and we already did go to $302. Why go to $310 plus $5.00 a mile. Marco Islanders
would pay $310 plus $150 (30 miles away). What would Immokalee pay being about 40 miles
away? We are talking about $500. It does not make sense.
I talked briefly with Bob Fernandes about the item. He seemed surprised too. Something is not
right about this.Is there a hidden agenda? Did you know about it? Perhaps it is time to look at
the whole picture again, now that this has come up.
Don't know about you,but as an administrator, I did not like shocks like this coming from center
field.

COLLIER COUNTY AMBULANCE SERVICE USER FEES
OBJECTIVE: To have the Board of County Commissioners review empirical data
concerning ambulance fees and charges and, if appropriate, adopt a resolution
updating ambulance service user fees.
CONSIDERATIONS: On September 13, 1994 the Board of County
Commissioners approved an ordinance and resolution to establish a new
ambulance billing process. This process allowed staff to accept patient insurance
payments including Medicare/Medicaid assignments prior to direct billing the
patient for any allowable unpaid balance due. The initial rate established in 1994
was a single charge of$250 for a transport.
On June 25, 1996, the Board amended the 1994 ordinance and adopted a new
billing resolution. These changes were approved to optimize and maximize the
allowable revenues from Medicare and other sources. The rate approved by the
Board was $302 for an ambulance transport.
Medicare increases its allowable amount every January based on charges billed
for an ambulance transport for the twelve month period ending the previous June
30.
Attached as Exhibit 1 is the fee screen (allowable fees) Medicare provided to
Collier County for the twelve month period beginning January 1, 1997.
Attached as Exhibit 2 is a listing of the counties Medicare uses to develop Collier
County allowable rates.
Attached as Exhibit 3 is a schedule of definitions (charge categories) used by
Medicare to determine the allowable rates for Collier County.
Attached as Exhibit 4 is a Medicare provided worksheet used by Medicare to
determine the allowable rates for Collier County.
Attached as Exhibit 5 is a projection of revenues for Collier County based on four
different sets of assumptions. They are (assuming 14,500 billable runs):
• Continue the existing rate of$302 = $3,202,600
• $302 plus $5/mile (average 7 miles) = $3,549,700
• Existing rate increased by 2.8% to $310 = $3,284,700
• $310 plus $5/mile (average 7 miles) = $3,631,700

Medicare has pre-approved a mileage allowable rate for Collier County of $4.11
per mile. See Exhibits 1 and 4. At the present time, Collier County's rate
resolution does not provide a mileage charge. The attached rate resolution
includes a charge of $5.00 per mile for each ambulance transport.
Attached as Exhibit 6 (2 pages) is the results of a recent survey taken by
Escambia County. Every county respondent except Collier County has a mileage
charge and most have an oxygen charge.
The addition of a $5.00 per mile charge for each billable transport will generate a
net positive cash flow to the County of$337,000 over a twelve month period.
FISCAL IMPACT: Board approval of the attached resolution would generate a net
ambulance fee revenue of $3,631,700 based on 14,500 billable transports. At 15,200
transports, as reflected in the FY 98 budget, the ambulance fee revenue generated would be
$3,787,000. There is no additional operating cost to the Board to implement the new fees.

OBJECTIVE: The Board of County Commissioners approve an Ordinance
known as the "Billing and Collection procedure for Collier County Ambulance
Services Ordinance", for Collier County Ambulance services and a resolution to
approve user fees for said services.
CONSIDERATION: On 9/13/94 the Board of County Commissioners approved
Ordinance #94-44 which established a new billing procedure. This billing
procedure allowed Collier County to accept patient insurance payments including
Medicare/Medicaid assignment prior to direct billing the patient for any unpaid
balance due.
The new Ordinance sets standards for providing for findings and purposes;
providing for applicability; providing for rates, fees, and charges; providing for
billing and collection procedures; including removal of past due accounts from
active accounts receivable; providing for Medicaid billings; providing for Medicare
billings and other third party billings; providing for alternative methods of
payment; providing civil action procedures; repealing in part previously adopted
ordinances; providing for conflict and severability: providing for an effective date.
The new Resolution allows for a higher user fee which is more in line with
2 Medicare's allowable rate and more in line with the cost of operating a
ambulance service. The Resolution also sets standards for the billing and
collection procedure; adjustments of EMS user fees, waiver of EMS user fees
and establishes guidelines for hardship cases and payment plans.
Staff is recommending the approval of the Ordinance and Resolution to
streamline the billing and collection procedures and to obtain the maximum
reimbursement from Medicare and Medicaid and other third party insurances.
The Boards' Emergency Management Services Advisory Committee
unanimously approved the recommended rates at their May, 1996 meeting.
FISCAL IMPACT: Board approval of this Ordinance and user fee Resolution will
