PERMITS EY AFFIDAVIT
The Florida Building Code permits the Building Official to accept an application for a building permit by a
sworn affidavit from a registered architect or engineer, Section 104.3.2 of the Florida Building Code.
The architect or engineer assumes fall responsibility that the plans and specifications conform to all the
technical codes. The architect or engineer who makes such affidavits shall agree to submit to the Building
Official copies of inspection reports as inspections are perform by them. Upon completion of the structure,
electrical, gas, mechanical, or plumbing systems, the architect or engineer must provide to the Building
Official certifications that these systems have been erected in accordance with the requirements of the
technical codes. Where the Building Official relies upon such affidavits, the architect or engineer shall
assume full responsibility for the compliance with all provisions of the technical codes and other pertinent
laws or ordinances. The Building Official shall ensure that any person conducting plans review is qualified
as a plans examiner under Part XII of Chapter 468, Florida State Statutes, and that any person conducting
inspections is qualified as a building inspector under Part XLt of Chapter 468, Florida State Statutes.

Agreement with Collier County as to the resolution of Notices of Violation in reference (case) number
2007080700 dated the 25th day of September, 2007.
In consideration of the disposition and resolution of the matters outlined in said Notice(s) of Violation for which
a hearing is currently scheduled for January 18th, 2008; to promote efficiency in the administration of the code
enforcement process; and to obtain a quick and expeditious resolution of the matters outlined therein the
parties hereto agree as follows:
1) The violations noted in the referenced Notice of Violation are accurate and I stipulate to their
existence.
The violations are that of section(s) 6 subsections 9,11,12b,12i,12p,19a,19c and 20 of Collier
County Ordinance 2004-58, and described as several minimum housing violations located within
three rental units on this property.
THEREFORE, it is agreed between the parties that the Respondent shall;
1) Pay operational costs in the amount of$227.15 incurred in the prosecution of this case.
2) Respondent must correct all minimum housing violations as outlined in the property
maintenance inspection report dated November 17th, 2007, within seven days of this hearing or
a fine of$250 a day until all violations are abated.
3) Respondent must notify Code Enforcement within 24 hours of abatement of the violation and
request the Investigator perform a site inspection to confirm compliance.
