CHIEF COMPLAINT: Complex facial laceration from dog bite.
HISTORY AND PHYSICAL: I am asked to see this 2- year -old female by the
pediatrician attending Dr. Meyer in the emergency room for complex lacerations
involving her face after dog bite. Apparently, the child fell onto
the dog and was bitten several times. This was witnessed by her
parents, who state she did not lose consciousness. The dog is known
to the family, has had all shots up to date including rabies in
December. The patient has no allergies to medications and was given
IV Unasyn. When I was called I asked the physician assistant Ahley Brockman and Dr.
Meyer to evaluate her
facial movements to ascertain whether the eyebrow on the right was
able to be elevated. Upon arrival, the mother stated that she and
the PA, Ashley Brockman, visualized an elevation of the right brow.
PAST MEDICAL HISTORY: Unremarkable.
IMMUNIZATIONS: Up -to -date.
SURGERIES: No prior surgeries.
MEDICATIONS: No medications.

Final Report
SOCIAL HISTORY: Remarkable for the parents living abroad.
PHYSICAL EXAMINATION:
GENERAL: She is a well appearing, calm child, in no acute distress.
HEENT: Remarkable for a large, deep 3 cm stellate laceration of the
right frontoteraporal area. The frontalis muscle is visualized and is
torn. The course of the frontal branch of the facial nerve is
directly over this area and I was not able to get the child to
elevate her brow on the right side. She had another laceration more
lateral and superior in the cheek, which was superficial. There was
another laceration of the upper lip near the philtral column and
along the vermilion on the left side. There was a through and through
the membranous septum, but the columella was intact with no external
laceration. The extraocular motions of the eye are intact. There
a;e scratches just below in the lower lip skin. The globe is grossly
ihtact and the vision is intact. The rest of the HEENT was normal.
HEART: Regular rate and rhythm.
LONGS: Clear.
ABDOMEN: Benign.
EXTREMITIES: Within normal limits.
ASSESSMENT AND PLAN: I had a detailed discussion with the parents the
extent of the injury and the options. Because of the location of the
laceration, that the muscle is torn at about the level just medial to
Where the nerve inserts. There is a chance that she may not have
function of that right brow elevator permanently. There is no
benefit* to exploring the nerve since this has already inserted into
the muscle and with the muscles torn and avulsed in that area. The
best course of action is to put the muscle back together, but then I
discussed the potential for infection, their option including the
wound open but this would leave a horrible scar in this small pretty
girl and I feel like the chances of infection are less than the
chances of poor scarring if the wound was not closed primarily.
Subsequently, after explaining these options in detail in layman's
terms, the parents requested she be repaired.
The pediatric ER physician administered IV ketamine for conscious
sedation. After signed informed consent, the patient was prepped and
draped in the usual sterile fashion and then the wound was injected
With It lidocaine with epinephrine. The wounds around the lip was
injected as well as cheek wound. It was decided not to fix the nose
repair since closing it primarily could result in a closed space
abscess with nasal floor, and since the wounds were in the membranous
septum they would heal without any external scarring.

The wound was explored and the findings were consistent with avulsed
and torn frontalis muscle at the lateral aspect right around where the frontal branch
enters, with some frontalis intact cephalad and caudad to the avulsion. The wound
had an
undermining affect and it appeared the dog clamped down from
laterally below to medially above creating a large space beneath the
frontalis and into the cheek. This was explored and no nerve endings

were encountered. The wounds were irrigated further with dilute
Betadine in its deep space. It was decided to leave a wick of
Iodoform gauze to let this space drain and this was brought out
centrally to the stellate portion of laceration. The muscle was
brought together with 5 -0 Vicryl to reconstruct it so that the more
lateral segment was reconstructed through a more medial segment and
similarly above and below the sutures were placed to bring up in a
full continuity of the muscle. The skin was then brought together
with 5 -0 Vicryl deep dermal sutures followed by 6 -0 fast - absorbing
gut on the skin. The cheek wound was irrigated with dilute Betadine
and closed with 5 -0 gut as was the lip lacerations. The total repair
of the complex portion of the forehead incision was 4 cm, the cheek
wound was 1 cm, and the lip wounds were a total of 2 cm in a primary layered fashion.
Xeroform was applied and a lightly compressive dressing.

was to leave the dressing on until I see her in my office on Monday.
