1. Provide a detailed and specific explanation of the requested change(s)to the task(s) and/or the additional
days added: The ACOE and SFWMD in coordination with Florida Community Trust(FCT) has been delayed in issuing the
permits. The Conservation Easement to SFWMD was revised. Final SDP permit should be issued by Mid to
late July 2012.
2. Provide detailed rationale for the requested change: Consultant cannot resubmit for SDP until ACOE and SFWMD
permits are issued by mid June 2012.
3. Provide explanation why change was not anticipated in original scope of work: ACOE/SFWMD has taking
additional time to process the permits in conjunction with FCT and Conservation Easement to SFWMD. The unscertainty
of the fish pond design at the Zoo entrance will not be resolved until the June 26, 2012 BCC meeting..
4. Describe the impact if this change is not processed: Final work cannot be completed by consultant and finalize the
permits.

This change order is to have the Consultant,Kimley-Horn&Associates, inc.extend the time
to complete the project. The ACOE/SFWMD in coordination with(FCT)Florida Community
Trust required additional time from the agencies. SFWMD required clarification for the
Conservation Easement that will be supplied by Kimley-Horn.
