SECTION I. Section 1 of Ordinance 81-75 shall be amended tO
read as followss
This ordinance is adopted pursuant to Chapter~ 125, and 403
Florida Statutes an~-Ucc~-~2~-~e~i~e-6~e~ee= The
purpose of this ordinance is to provide better protection forj~the
health, safety and welfare of the residents of Collier ~ounty~in~.
medical transportation matters, by establishing unif~rm
county-wide standards for .certification of ambulance~ er ~vance~
life support~ and non-emergency medical transportation services
e~-~~-s~-~r-~~s by promulgating
rules and regulations for operation of all ambulances? e~ rescue
companies~ e~ and Don-emer~enc~ medical transportation services
in Collier County.
SECTION II. Section 2 of Ordinance 81-75 is amended by
adding the following definition of non-emergency medical
transportation services.
K. ~on-emergency medical transportation services shall include
the provision of transport, a%ion ~erviqes bY publicly or priv~t91y
owned land~ air or water vehicle that is desi~ned~
reco,structed~ maintained~ equipDqd~ o.r .operated;q~
for or intended to be .used f.~.~lr.~ land~ ~r water transportation

SECTZON III. Section 5 of Ordinance 81-75 ia amended as
~ollows~
An applicant for a certificate to.operate an ambulance
.ser¥ice{ advanced life support servicer .or non-em~r~.ency medical
tra~s~ortation service shall obtain forms from the department to
be completed and returned to the Division Administrator. Each
application shall contain=
A. The name, age, and address of the owner of the ambulance~
ALS provider or provider of non-emergency medical
transportation.service, or if the owner is a corporation,
then of the directors of the corporation and of all the
stockholders holding more than 25% of the outstanding shares.
For governmental units, this information shall be supplied
for members of the governing body.
The boundaries of the territory desired to be served.
The number and brief description of the ambulances or o~ther
vehicles the applicant will have available.
The address of the intended headquarters and any substations. ·
The training and experience of the applicant.
The names and addresses of three (3) Collier County residents
who will act as references for the applicant.
A schedule of rates which the service intends to charge.
Such other pertinent information as t~e administrator may
require.
An application or renewal fee of two hundred fifty dollars
{$250.00)
Financial data including assets and l~abilitios of the
operator. A schedule of all debts encumbering any equipment
shall bo included.
SECTION IV. Conflict and 9overability.
In tho event this Ordinance conflicts with other applicable
law, the moro restrictive shall apply.
If any section, subsection, sentence, clause, phrase, o~
portion of this Ordinance is, for any reason, held invalid or
unconstitutional by any court of competent Jurisdiction, such
portion shall be deemed a separate, distinct and independent
provision of such holding and shall not affect'the validity of
the remaining portion thereof.
SECTION V. Effective Date.
A copy of this Ordinance, as enacted, shall be filed by the
Clerk of the Board with the Department of State of the State of
Florida, as soon after enactment as is practicable by depositing
the same with the postal authorities of the government of the
United States by mail, postage prepaid, to the Florida Department
of State.
This Ordinance shall become effective October 1, 1982.
