as representative for Respondent and enters into this Stipulation and
Agreement with Collier County as to the resolution of the Citation in reference, Citation No. 4529
dated the 17th day of June 2009.
In consideration of the disposition and resolution of the matters outlined in said Citation for which a hearing is
currently scheduled for August 7, 2009; to promote efficiency in the administration of the code enforcement
process; and to obtain a quick and expeditious resolution of the matters outlined therein, the parties hereto
agrees as follows:
1) The violations noted in the referenced Citation are accurate and I stipulate to their existence.
2) The violations are that of Ordinance I Section(s) 2005-54 19 E 2 and are described as failure to
enter into a contract with approved contractor for the collection of solid waste.
THEREFORE, it is agreed between the parties that the Respondent shall;
1) Pay operational costs in the amount of$ °— incurred in the prosecution of this case.

COMES NOW, the undersigned, ANGEL MADERA SR, on behalf of himself or MYRA E ROL as representative
for Respondent and enters into this Stipulation and Agreement with Collier County as to the resolution of
Notices of Violation in reference (case) number CEV20090003488 dated the 30TH day of MARCH, 2009_.
In consideration of the disposition and resolution of the matters outlined in said Notice(s) of Violation for which
a hearing is currently scheduled for 8-7-09; to promote efficiency in the administration of the code enforcement
process; and to obtain a quick and expeditious resolution of the matters outlined therein the parties hereto
agree as follows:
1) The violations noted in the referenced Notice of Violation are accurate and I stipulate to their existence.
Commercial vehicle parked in the driveway of residence and vehicles parked on the grass without valid
license plates
THEREFORE, it is agreed between the parties that the Respondent shall;
1) Pay operational costs in the amount of$117.70 incurred in the prosecution of this case within 30
days of this hearing.
2) Abate all violations by:
3) Respondent is to move any and all vehicles from the front/rear yard of 5431 28th Ave SW and park them
on a stabilized pervious or imperviously treated surface within three days of this order or pay a fine of
$50.00 a day that this violation remains unabated.
4) Respondent must also obtain a current registration for the vehicle, store in an enclosed structure, or
remove it from his property within three days of this order or pay a fine of$50.00 a day for each day this
violation remains unabated
5) Respondent is also to remove, store in the backyard (concealed from view), or garage (in an enclosed
structure) any and all commercial vehicles at the property within three days of this order or pay a fine in
the amount of$100.00 per day that this violation remains unabated
3) Respondent must notify Code Enforcement within 24 hours of abatement of the violation and request
the Investigator perform a site inspection to confirm compliance.
(24 hours notice shall be by phone or fax and made during the workweek. If the violation is abated 24 hours prior to a Saturday,Sunday or legal holiday,then the
notification must be made on the next day that is not a Saturday,Sunday or legal holiday.)
4) That if the Respondent fails to abate the violation the County may abate the violation and may use the
assistance of the Collier County Sheriff's Office to enforce the provisions of this agreement and all
costs of abatement shall be assessed to the property owner.

STIPULATION/AGREEMENT
COMES NOW, the undersigned, Gabino Medina Jr., on behalf of himself enters into this Stipulation and Agreement with
Collier County as to the resolution of Notices of Violation in reference (case) number CEPM20080016415 dated the 5th
day of January, 2009.
In consideration of the disposition and resolution of the matters outlined in said Notice(s) of Violation for which a hearing is
currently scheduled for August 7th, 2009; to promote efficiency in the administration of the code enforcement process; and
to obtain a quick and expeditious resolution of the matters outlined therein the parties hereto agree as follows:
1) The violations noted in the referenced Notice of Violation are accurate and I stipulate to their existence.
Violation of Collier County Code of Laws and Ordinances, Chapter 22, Article VI, Section 22-231, Subsections 9,
12i, 12k, 121, 12p, 13, and 19 and described as a rental property being maintained in sub-standard condition.
THEREFORE, it is agreed between the parties that the Respondent shall;
1) Pay operational costs in the amount of$118.31 incurred in the prosecution of this case within 30 days of
this hearing.
2) Abate all violations by:
Hiring a general contractor licensed in Collier County to obtain all applicable permits; inspections; and certificates
of completion to repair all minimum housing violations as outlined in the Property Maintenance Inspection Report
dated 12-15-08 by November 7th, 2009, or a fine of$250 a day until all violations are abated.
Alternately, hiring a general contractor licensed in Collier County to obtain permit; inspections; and certificate of
completion for the demolition of the structure by November 7th, 2009, or a fine of$250 a day until all violations are
abated.
3) Respondent must notify Code Enforcement within 24 hours of abatement of the violation and request the
Investigator perform a site inspection to confirm compliance.

Subject: Declaration of Dangerous Building located at 3520 8th Ave. NE
The property located at the above noted address was inspected in response to a request from the Collier County Code
Enforcement Department for the purposes of determining if the subject building meets the definition of "dangerous
building".
In accordance with the 2004 Florida Building Code any building or structure or any individual member with any of the
structural conditions or defects described below shall be deemed dangerous. A check mark next to any paragraph below
is an indication one or more of the dangerous structural conditions have been found.
❑ 1. The stress in a member or portion thereof due to all factored dead and live loads is more than one and
one third the nominal strength allowed in the Florida Building Code, Building for new buildings of similar
structure, purpose,or location.

® 3. Any portion of a building, or any member, appurtenance, or ornamentation on the exterior thereof is not
of sufficient strength or stability, or is not anchored, attached, or fastened in place so as to be capable of
resisting a wind pressure of two thirds of that specified in the Florida Building Code, Building for new buildings of
similar structure, purpose, or location without exceeding the nominal strength permitted in the Florida Building
Code, Building for such buildings.
® 4. The building, or any portion thereof, is likely to collapse partially or completely because of dilapidation,
deterioration or decay; construction in violation of the Florida Building Code, Building; the removal, movement or
instability of any portion of the ground necessary for the purpose of supporting such building; the deterioration,
decay or inadequacy of its foundation;damage due to fire,wind or flood;or any other similar cause.
❑ 5. The exterior walls or other vertical structural members list, lean, or buckle to such an extent that a plumb
line passing through the center of gravity does not fall inside the middle one third of the base.
The field inspection revealed the building satisfied one or more of the conditions noted above. Based on the
inspection findings the building was found to be dangerous and unsuitable for the safe occupancy of its intended purpose.
Immediate remediation of the conditions up to and including demolition of the structure are required.
COMMENTS: A 1 story structure severely damages by fire. The structure shall be evaluated by a
registered design professional, and the evaluation findings shall be submitted to the code official. The
evaluation shall establish whether the damaged structure, if repaired to its predamaged state, would
comply with the provisions of the codes that were in effect when the building was permitted. Any
owner or authorized agent who intends to repair or demolish shall first make application to the
building official and obtain the required permit.
