Agenda 12/09/2025 Item #16E 1 (Fourth Amendment to Agreement No. 14-6293, “Collier County Onsite Medical Clinic,” with Millennium Physician Group, LLC)12/9/2025 Item # 16.E.1 ID# 2025-4703 Executive Summary Recommendation to approve the Fourth Amendment to Agreement No. 14-6293, “Collier County Onsite Medical Clinic,” with Millennium Physician Group, LLC, extending the agreement through June 30, 2026. OBJECTIVE: To continue to provide onsite medical and limited pharmacy services to employees and dependents covered under the Collier County Group Health Plan for the purpose of promoting health, wellness, and productivity. CONSIDERATIONS: The Human Resources Division administers a partially self-funded Group Health Insurance Program for its employees, participating constitutional officer employees and their eligible dependents. In 2006, the Board approved the creation of an onsite medical clinic to provide medical care to employees and dependents covered under the Group Health Insurance Program. Since that time, services provided through the clinic have expanded beyond acute care to include the management of chronic conditions such as pre-diabetes and diabetes under the Take Charge program; cardiac health under the Heart Smart program; and weight management under the Inside Out program. Services are provided at the Main Campus location as well as a clinic location in North Naples. On September 9, 2014 (Agenda Item 16.E.7), Millennium Physician Group, LLC (“Millennium”) was awarded an initial five-year term contract with a five-year renewal option. On October 10, 2017 (Agenda Item 16.E.2), the County approved a First Amendment to the Agreement providing for the addition of limited pharmacy services through The Millennium MedCenter. On October 8, 2019 (Agenda Item 16.E.3), the County approved a Second Amendment to the Agreement, which updated the Scope of Services to eliminate options that were no longer necessary and finalize the remaining five-year renewal, including an updated Fee Schedule. On June 25, 2024, the Board approved a Third Amendment (Agenda Item 16.E.1) to the Agreement to extend the terms through December 31, 2025. The Fourth Amendment to the Agreement provides a six-month extension to allow staff additional time to conclude the negotiation of Request for Proposal #25-8397, Comprehensive Clinic and Wellness Services, so a new agreement can be brought to the Board. Services provided by Millennium at on-site MedCenter locations have been more than satisfactory, and employee feedback continues to be positive. Further, proposed pricing for the extension will be at the current rate of $30.52 per employee per month. Staff recommends approval of the attached Fourth Amendment to the Agreement extending its terms at the existing pricing through and including June 30, 2026. This item is consistent with the Collier County strategic plan objective of responsible governance, promoting fiscal stewardship, and sound budget oversight. FISCAL IMPACT: The total estimated six-month cost to provide primary care services (current clinic model) is estimated at $432,000 based upon an estimated monthly enrollment of 2358 employees. Sufficient funds have been budgeted in Fund 5017, Group Health and Life, for this purpose. GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this item. LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote for Board approval. —SRT RECOMMENDATION(S): That the Board approves the Fourth Amendment to Agreement No. 14-6293, “Collier County Onsite Medical Clinic” with Millennium Physician Group, LLC through and including June 30, 2026, with a total anticipated expenditure of $432,000, and authorize the Chairman to sign the attached amendment.
