Agenda 09/23/2025 Item #16D 1 (First Amendment to Subrecipient Agreement between Collier County and Collier Health Services, Inc)9/23/2025 Item # 16.D.1 ID# 2025-2847 Executive Summary Recommendation to approve and authorize the Chair to sign the First Amendment to Subrecipient Agreement between Collier County and Collier Health Services, Inc., d/b/a/ HealthCare Network, to amend the Community Development Block Grant Agreement #CD-CV21-09 to revise project details, payment deliverables, supporting documentation, and update the grant language and exhibits. (Housing Grants Fund 1835) OBJECTIVE: To provide a collaborative community healthcare plan in support of Collier County’s Strategic Plan to provide access to health, wellness, and human services. CONSIDERATIONS: The U.S. Department of Housing and Urban Development (“HUD”) provides Community Development Block Grant (“CDBG”), HOME Investment Partnerships Program (“HOME”), and Emergency Solutions Grant (“ESG”) entitlement funds to the County annually to address program-specific needs in the community. The Board of County Commissioners (“Board”) previously approved the 2019-2020 One-Year Action Plan on June 25, 2019 (Agenda Item #16. D.2). The Board approved the 5-Year Consolidated Plan and Citizen Participation Plan on June 28, 2016 (Agenda Item #11. E). On September 22, 2020, the County received notification of CDBG-CV funding in the amount of $1,561,633. On April 27, 2020, the County Manager signed the substantial amendment to Collier County’s U.S. Department of Housing and Urban Development Annual Action plan for FY2019-2020 to address additional funding. On May 12, 2020 (Agenda Item #16. F.1(f)), the Board ratified the County Manager’s action to amend the 2019-2020 One-Year Action Plan to recognize the first round of CDBG-CV funding. On February 5, 2021, the County received notification of the second CDBG-CV funding allocation in the amount of $2,671,095. On November 10, 2020 (Agenda item #16. D.8), the Board approved the second substantial amendment to the 2019- 2020 Action Plan, recognizing the second allocation of CDBG-CV funding, which brings the total award for Collier County to $4,232,728, respectively. On May 28, 2024 (Agenda item #16. D.4), Community and Human Services (“CHS”) amended the 2019-2020 Annual Action Plan for CDBG, CDBG-CV, ESG, and HOME funding to recognize the reallocation of funding within identified programs to be undertaken and respond to community needs. On April 8, 2025 (Agenda Item #16. D.8), the Board approved Subrecipient Agreement #CD-CV21-09 with Collier Health Services, Inc. d/b/a Healthcare Network (“Collier Health Services”), in the amount of $225,000.00, to support the development of a community health plan addressing healthcare access, preventative care and responsiveness to help mitigate the risk of future public health emergencies within the community. The plan includes partnerships with the health department and local medical providers/facilities. In July 2025, Collier Health Services requested a plan realignment requiring an amendment to the Subrecipient Agreement in order to reduce duplication of efforts with the Florida Department of Health in Collier County. As a result of this realignment, Collier Health Services has proposed a strategic shift toward developing and executing an implementation plan to address the needs of highly vulnerable populations within the community. The proposed First Amendment to the Subrecipient Agreement will amend the following: a) Part 1 Scope of Work- Update the project components to support the development and execution of an implementation plan. Additionally, increase staffing from one (1) to two (2) full-time equivalent (FTE) positions to add a data analyst to support the project. b) Section 1.1 Grant Special Conditions a. C. Performance Deliverables: To include Collier County Inventory Tracking Form, Conflict of Interest Form, Whistleblower Protections Certification, and Affidavit Regarding Labor and Services (Trafficking). H. Payment Deliverable: Update the project components to support the development and execution of an implementation plan. Additionally, increase staffing from one (1) to two (2) full-time equivalent (FTE) positions to enhance capacity and ensure effective project delivery. c) Section 1.3 Agreement Amount revise language from monthly to quarterly submission schedule. d) Part II Grant Control Requirements a. FISCAL IMPACT: There is no new fiscal impact. Funding for this agreement is available in Housing Grant Fund (1835), Project 33674 (CBDG-CV) in the amount of $225,000. GROWTH MANAGEMENT IMPACT: This item has no impact on the Housing Element of the Growth Management Plan of Collier County.
