Executive Summary
Recommendation to approve and authorize the Chairman to sign the First Amendment to the Subrecipient Agreement
between Collier County and Baker Senior Center Inc. to amend Community Development Block Grant Agreement
#PS24-04. (No Fiscal Impact)
OBJECTIVE: To support and further the County’s strategic plan by supporting access to health, wellness, and human
services for low- to moderate-income citizens of Collier County
CONSIDERATIONS: The U.S. Department of Housing and Urban Development (HUD) Community Development
Block Grant (CDBG) program funds locally defined requests for program specific needs in the community and
infrastructure improvements. Every five years the County is required to submit a Consolidated Plan to HUD outlining
the goals and activities to be undertaken with HUD entitlement funds over the five-year period as well as Annual Action
Plans.
On June 22, 2021, the Board of County Commissioners (Board) approved the County’s Five-Year Consolidated Plan for
the use of entitlement funds for Program Years (PY’s) 2021-2025 (Agenda Item #16.D.7). As part of the County’s 5-
year Consolidated Plan, the Community and Human Services (CHS) Division is required to adopt and submit an Annual
Action Plan each year. This is the fourth (4th) year of the Five-Year Consolidated Plan. The Annual Action Plan was
developed in accordance with the County’s Citizen Participation Plan (CPP). The Board of County Commissioners
(Board) approved the Action Plan for Federal Fiscal Year 2024-2025 on July 23, 2024 (Agenda Item #16.D.4)
On September 10, 2024 (Agenda Item #16.D.3), the Board of County Commissioners (“Board”) approved the CDBG
Subrecipient Agreement with Baker Senior Center Inc. The agreement established $55,000.00 in funding for the salary
of one (1) geriatric case manager to support low- to moderate-income adults.
Baker Senior Center Inc. staff experienced hiring challenges which has necessitated the need to add a second project
component to ensure CDBG funds are expended in a timely manner. This action is an attempt to facilitate spending to
ensure that the County meets its federal timeliness requirement. Failure to expend project funds by Baker Senior Center
Inc. could result in adverse findings by HUD.
The proposed First Amendment will revise the agreement by:
a) Adding a Project Component Two: Supplies to include but not limited to laptop, software/member fees, cell phone
and service to support the geriatric case manager.
b) Shift $8,305.00 from Project Component 1 to Project Component 2.
c) Revise the Performance Deliverables to add continued use and inventory reporting.
d) Remove LGBTQ policy.
e) Add the required continued use period
f) Revise the scope of work to include equipment.
g) Add Exhibit F- Equipment Tracking Form.
This item is consistent with the Collier County strategic plan objective by supporting access to health, wellness, and
human services
FISCAL IMPACT: There is no new fiscal impact for this Executive Summary. The funding for this award is budgeted
in Housing Grant Fund (1835), Project 33915.
GROWTH MANAGEMENT IMPACT: This item has no impact on the Housing Element of the Growth Management
Plan of Collier County.
