Executive Summary
Recommendation to approve a Third Amendment to Agreement No. 15-6474R, “Medical Director for Collier County
and Employment Physicals and Drug Testing,” with Advance Medical of Naples, LLC.
OBJECTIVE: To extend Agreement No. 15-6474R for one additional year with updated pricing to continue medical
oversight services for the County’s Occupational Health Program.
CONSIDERATIONS: On February 9, 2016 (Agenda Item 16.E.1), the County entered into Agreement No. 15-6474R
(the “Agreement”) with Advance Medical of Naples, LLC to provide medical services and an oversight physician for the
Occupational Health staff. On October 15, 2018, the County administratively approved the First Amendment to the
Agreement, which added two additional testing categories to Exhibit B-Price, Part B. On January 14, 2020, the County
administratively approved the Second Amendment, which extended the Agreement term for an additional five years,
starting on February 9, 2020, and ending on February 8, 2025, while also updating pricing.
The attached Third Amendment will extend the Agreement for one additional year, from February 9, 2025, to February
8, 2026, and update the pricing under Exhibit B-1. All other terms and conditions of the Agreement will remain
unchanged unless modified by this Amendment.
Advance Medical of Naples, LLC specializes in Occupational Medicine and offers services seven days a week with
extended office hours, diagnostic testing for Occupational Medicine, onsite drug screening, and an in-house Medical
Review Officer.
FISCAL IMPACT: Funds are budgeted annually in Fund 5018 (Workers’ Compensation Fund) and department
budgets for required physicals and drug tests. Total funds expended are dependent upon the need and types of services.
The estimated annual expenditure for FY 2025, based upon the proposed rates and estimated volume of service is
$150,000.
GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this item.
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote for Board
approval. —SRT
RECOMMENDATIONS: To approve a Third Amendment to Agreement No. 15-6474R, extending the Agreement for
one additional year, updating the pricing for services, and authorize the Chairman to sign the attached Amendment.

Good Morning,
In reference to the aforementioned agreement Risk Management would like to point out that
Section 12 E on the contract is a duplicate of Section 12 D; for Medical Professional Liability and
Medical Malpractice are the same coverage. During the BID process both items were checked by
mistake; but the only coverage that is required is Medical Professional Liability with limits of
$250,000 per occurrence and $750,000 aggregate. These limits are consistent with the limit carried
under this agreement in the past. Therefore, the Certificate of Insurance providing proof of
coverage for the Medical Professional Liability with a limit of $250,000 is in compliance.
