Agenda 11/12/2024 Item #16E 3 (Approve Agreement #24-040-NS w/Hinge Health, Ins., utlizing the Sourcewell Contract #102722-HNG to provide a digital solution for musculoskeletal health at an estimaned annual espenditure effective January 1, 2025)11/12/2024 Item # 16.E.3 ID# 2024-1879 Executive Summary Recommendation to approve Agreement No. 24-040-NS with Hinge Health, Inc., utilizing the Sourcewell Contract #102722-HNG, to provide a digital solution for musculoskeletal health at an estimated annual expenditure of $152,000, effective January 1, 2025. OBJECTIVE: To approve Agreement #24-040-NS with Hinge Health Inc. to provide a digital musculoskeletal health solution for employees and their dependents covered under the Collier County Group Health Plan, aimed at enhancing health, wellness, and productivity. CONSIDERATIONS: The Risk Management Division administers a partially self-funded Group Health Insurance Program for its employees, participating constitutional officer employees, and their eligible dependents. During the last three plan years, Allegiance Benefit Plan Management Inc., the County’s third-party claims processor, reports that the County’s expenditure for musculoskeletal claims was $2.8 million for 2021, $1.7 million for 2022, and $2.2 million for 2023. To improve employee outcomes through increased physical therapy adherence, avoid costly unnecessary surgeries, decrease presenteeism, and decrease musculoskeletal claims spend, staff is recommending a partnership with Hinge Health. The County’s brokerage and actuarial firm, Willis Towers Watson Inc., has recommended Hinge Health to many clients and rates them as one of the top service providers in the digital musculoskeletal (MSK) therapy field. Hinge Health, Inc. (“Hinge Health”) offers a comprehensive digital solution for MSK Health, focused on reducing pain and improving outcomes for individuals managing chronic back and joint pain. The Hinge Health platform integrates seamlessly with existing health and wellness programs to maximize effectiveness. Certified health coaches from Hinge Health provide thorough training and education to ensure members use the digital solution correctly and effectively. This includes instructional materials, workshops, and ongoing support, empowering employees to take control of their MSK health. The primary programs staff is seeking services for at this time are the acute and chronic programs. The scope of work outlines other programs that may be utilized in the future. Physical therapists establish clear metrics to monitor the effectiveness of Hinge Health’s digital program, tracking patient improvements in pain reduction, mobility, and healthcare utilization. Therapists regularly review these outcomes to identify areas for improvement and adjust strategies as needed. Open communication with employees is essential, allowing feedback to be gathered and any issues or concerns to be promptly addressed. The program provides a personalized experience, including a dedicated care team comprising a physical therapist and health coaches, along with virtual therapy sessions that members can complete for fifteen minutes a day, three times per week. Sessions are accessible anytime, anywhere through Hinge Health’s proprietary software and sensor technology. Sensors are placed on targeted areas of the body and, using a handheld device, members receive real-time feedback on proper sensor placement and exercise technique to ensure maximum results. If a member is not meeting their therapy goals, the health coach is notified and will contact the participant to determine whether the exercises are too challenging or if the participant is experiencing discomfort. The therapist then works directly with the member to resolve the issue and guide them back on their path to recovery. Partnering with Hinge Health presents a valuable opportunity to improve the musculoskeletal health and overall well- being of the County’s workforce while lowering healthcare costs and boosting productivity. Hinge Health’s digital solution offers targeted support for acute and chronic pain, with distinct pricing for each. Based on the County’s insured population, Hinge Health projects gross savings in the first year that could reach approximately $466,000, stemming from reductions in physical therapy office visits, surgery avoidance, and decreased opioid prescription costs. Hinge Health also guarantees a minimum 1.5 return on investment (ROI) for the first year, with a pro-rated refund of fees if the target ROI is not met. Staff requests approval of the use of the Sourcewell Contract #102722-HNG and Agreement #24-040-NS with Hinge Health Inc. for this purchase, pursuant to Section 11.4(a) of the Procurement Ordinance 2017-018, as amended, which allows for cooperative purchasing. Page 3275 of 5419 11/12/2024 Item # 16.E.3 ID# 2024-1879 The Scope of Work in the attached Agreement No. 24-040-NS contains redacted proprietary information regarding the pricing methodology, which is exempt from the Public Records Act. Finally, Hinge Health will file service claims on behalf of members with Allegiance, which will handle the processing and payment of these claims. This arrangement enables staff to track expenses and compare them to previous periods, ensuring the program delivers health improvements and cost savings. FISCAL IMPACT: The estimated annual cost to provide Hinge Health’s services is $152,000. Sufficient funds have been budgeted in Fund 5017, Group Health and Life for this purpose. GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated with this action. LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires a majority vote for Board approval. - CJS RECOMMENDATIONS: To approve the cooperative procurement and use of the Sourcewell Contract #102722-HNG, authorize Agreement 24-040-NS with Hinge Health, Inc. for an anticipated annual expenditure of $152,000, and authorize the Chairman to sign the Master Agreement and Scope of Work. Participating Entities reserve the right to inspect the Equipment and Products at a reasonable time after delivery where circumstances or conditions prevent effective inspection of the Equipment and Products at the time of delivery. In the event of the delivery of nonconforming Equipment and Products, the Participating Entity will notify the Supplier as soon as possible and the Supplier will replace nonconforming Equipment and Products with conforming Equipment and Products that are acceptable to the Participating Entity.
