EXECUTIVE SUMMARY
Recommendation to approve three (3) "After -the -Fact" agreements and attestation statements between
Collier County and the Area Agency on Aging for Southwest Florida, Inc., for the Community Care for the
Elderly #HCO24 CCE 203.24, Alzheimer's Disease Initiative #HZ024 ADI 203.24, and Home Care for the
Elderly #HH024 HCE 203.24 grant programs in support of the Collier County Services for Seniors Program
and authorize the necessary Budget Amendments in the amount of $393,007.26 to ensure continuous funding
for FY2024/2025. (Human Services Grant Fund 1837)
OBJECTIVE: To ensure continuous support for elderly clients through the Collier County Services for Seniors
(CCSS) program and enhance the Quality of Place focus area by improving access to health, wellness, and human
services.
CONSIDERATIONS: The Community and Human Services (CHS) Division's CCSS program has been providing
support to Collier County's frail and elderly citizens for over thirty (30) years through the Community Care for the
Elderly (CCE), Alzheimer's Disease Initiative (ADI), and Home Care for the Elderly (HCE) grant programs. These
grants are funded by the Florida Department of Elder Affairs (DOEA) through the Area Agency on Aging for
Southwest Florida (AAASWFL). These grants enable clients to remain in their homes and live with independence
and dignity.
On May 28, 2024 (Agenda Item #16.D.2), the Board approved the estimated funding levels for FY2024-2025 for
CCE, ADI & HCE to prevent disruption in services and ensure compliance with the grant service agreement
requirements.
Through the proposed contracts, AAASWFL has provided an increase in funding in the amount of $393,007.26 for
CCE, ADI, and HCE grant awards as shown in the chart below for the FY2024/2025 contract year. AAASWFL
agrees to pay for contracted services up to the amount of $1,265,563.62 for the CCE program, up to $1,169,353.43
for the ADI program, and up to $84,022.21 for the HCE Program. The overall funding will allow Services for
Seniors to provide case management and related services based on current and projected program needs. The CCE
grant requires a corresponding local match increase of $16,062. This match increase will be met through estimated
vendor match contributions.

*Ten Percent (10%) match is the increased amount of the Award divided by .9, then multiplied by .1, per grantor requirements,
rounded to the nearest dollar. The Cash Match is reduced by the Co -payment budget and vendor match contribution.
CCSS is also responsible for the collection of client co -payments for the CCE and ADI Grants, with a specified
collection goal established by CHS. The CCE co -payments are used to offset the local matching requirements
should they not be met through the vendor match contribution.

AAASWFL routinely sends CCSS agreements that require a 30-day turnaround time. This timeframe makes it
impossible to process the documents for the Board's approval in the time requested by the AAASWFL. The
agreements for CCE, ADI, & HCE are being presented "After -the -Fact" because Collier County received the grant
agreements on July 30, 2024, with an effective contract date of the last day the contract had been signed by both
parties. Consequently, pursuant to CMA #5330 and Resolution No. 2018-202, the County Manager authorized the
Public Services Department Head to sign the contracts. These contracts are being presented for Board ratification.
Collier County, as the Lead Agency, is responsible for responding to seniors' needs and managing the spending
authority for the Federal and State Funded Services for Seniors programs.
FISCAL IMPACT: Budget Amendments are required to recognize additional grant revenue in the amount of
$393,007.26 for the FY24-25 DOEA Grant Programs. The Budget Amendments will increase the funding by
$144,563.62 in CCE Project (33905), by $184,959.43 in ADI Project (33904), and by $63,484.21 in HCE Project
(33906).
Funding currently resides in Human Services Grant Fund (1837) Project 33905 (CCE); 33904 (ADI); 33906 (HCE);
33627 (CCE Co -Pays); and 33625 (ADI Co -Pays).
The CCE grant has a Local match requirement of ten percent (10%) in the amount of $140,618 which is met
through estimated co -payments received in the amount of $11,219 and estimated vendor match contribution of
$114,399, requiring a local cash match of $15,000. Local cash match in the amount of $15,000 is available through
carry forward (Reserves) in Human Services Grant Fund (1837). Neither ADI nor HCE have matching
requirements. There is no impact to the General Fund (0001).
GROWTH MANAGEMENT: There is no Growth Management impact associated with this Item.
LEGAL CONSIDERATIONS: This Item is approved as to form and legality and requires a majority vote for

A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, REPEALING AND SUPERSEDING
RESOLUTION NO. 96-268 and 10-122 AND AUTHORIZING THE
COUNTY MANAGER OR HIS/HER DESIGNEE TO EXECUTE CERTAIN
STANDARD SOCIALISENIOR SERVICES GRANT APPLICATIONS
AND CONTRACT DOCUMENTS INCLUDING AMENDMENTS IN
COMPLIANCE WITH CMA #5330, GRANT COORDINATION.
WHEREAS, the Board of County Commissioners (Board) has the authority to apply for
and receive social/senior services grants including the Older American's Act Program (OAA),
Community Care for the Elderly Program (CCE), Alzheimer's Disease Initiative Program (ADI),
Medicaid Waiver Agreement, the Home Care for the Elderly program (HCE), and USDA
Nutritional Supplement Incentive Program, and Emergency Home Energy Assistance Program
(EHEAP); and
WHEREAS, in order for Collier County's social/senior services grant documents to be
submitted in a timely fashion, the Board adopted Resolution No. 96-268 and 10-122 authorizing
the County Manager or his/her designee to execute the required standard grant documents; and
WHEREAS, Collier County CMA 95330, Grant Coordination, provides authorization for
the County Manager or his/her designee to execute certain grant documents and return to the
Board of County Commissioners for after -the -fact approval; and
WHEREAS, the Board wishes to delegate signature authorization to the County Manager
or his/her designee for the above -referenced social/senior services grant programs and as it relates
to grant applications, standard contract documents, and major and minor amendments provided
that said submissions and contracts require no additional Collier County matching funds.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that the County Manager or his/her
designee is hereby authorized to execute grant applications, standard contract documents, and

amendments in compliance with CMA H5330, Grant Coordination, for the Older American's Act

BE IT FURTHER RESOLVED that this Resolution repeals and supersedes Resolution
No. 96-268 and No.10-122.

RE: County Manager Designation to Execute Grant Applications and Contract Documents
outlined in RESO 2018-202 for the Cluster of Aging Grants.
On November 13, 2018, Resolution 2018-202 superseding 96-268 and 10-122 was approved by
the Board to allow the County Manager or designee to execute grant program documents
including applications, contracts and amendments for the cluster of aging programs.
Federal and State funding is received on an annual basis through the Older Americans Act and
Department of Elder Affairs administered by the Area Agency on Aging for Southwest Florida
(AAA) This action is necessary to expedite execution so that funding shifted between grant
programs to meet client needs is available timely to order to maintain program standards of
service. All executed documents are presented to the Board after -the -fact and coordinated with
the OMB Grants Compliance Office as required pursuant the CMA 5330 Grants Coordination
Policy.
This letter serves to designate Tanya Williams. Public Services Department Head; as the
Authorizing Official to fulfill these responsibilities.
