WHEREAS, the legislature of the State of Florida, by enacting section 336.05, Florida
Statutes, has conferred on all counties in Florida the power to name or rename streets and roads,
except for certain state roads; and
WHEREAS, the Collier County Board of County Commissioners ("Board") pursuant
thereto has adopted Collier County Ordinance No. 07-62, as codified in Chapter 22, Article X,
Sec. 22-357 of the Collier County Code of Laws and Ordinances, which establishes criteria and
procedures to rename streets or roads; and
WHEREAS, the Board has held a duly advertised public hearing after providing notice as
required by said ordinance and has considered the advisability of granting the request by Stephen
Geth of Tiger Construction, Inc., on behalf of property owners FOD Perry Naples, LLC and
Phelan Properties - Riverbend, LLC, to rename a street from Clear Bay Drive to Perry Dise
Lane; and
WHEREAS, Clear Bay Drive is located in in Section 21, Township 48 South, Range 25
East, Collier County, Florida, as shown as Clear Bay Drive, Tract 3, according to the plat of
Cocohatchee River Trust P.U.D., recorded in Plat Book 18, Page 98, of the Public Records of
Collier County, Florida, and as shown on Collier County Official Zoning Atlas Map Number
8521N; and
WHEREAS, the Board has determined that all criteria set forth in Ordinance No. 07-62
has been met and specifically finds that there is no other street in incorporated or unincorporated
Collier County with this name or any similar sounding name as is being requested; and
WHEREAS, the Board has determined that it is necessary for identification purposes to
confirm the name of this street.
NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA,that:
Effective this date, the name of the street described in the above recitals is hereby
changed from Clear Bay Drive to Perry Dise Lane and is confirmed as such.

BE IT FURTHER RESOLVED that this Resolution be recorded in the Public Records of
Collier County, Florida, and noted upon the maps of the street and zoning atlases of Collier

This Resolution adopted after motion, second, and super majority vote of at least four
