This Work Order is for professional underground utility contractor services for work known as:
Project Name: Orange Tree WWTP—Screening System Replacement
Project No: 70173.8
The work is specified in the proposal dated March 20, 2024 which is attached hereto and made a part
of this Work Order. In accordance with Terms and Conditions of the Agreement referenced above,this
Work Order/Purchase Order is assigned to: Mitchell& Stark Construction Company, Inc.
Scope of Work: As detailed in the attached proposal and the following:
* Task I OTWWTP Screening System Replacement Construction
* Task II Allowance for Owner's Use as Directed
Schedule of Work: Complete work within 260 days from the date of the Notice to Proceed which is
accompanying this Work Order. The Consultant agrees that any Work Order that extends beyond the
expiration date of Agreement#20-7800 will survive and remain subject to the terms and conditions of
that Agreement until the completion or termination of this Work Order.
Compensation: In accordance with the Agreement referenced above, the County will compensate the

CHANGE 1:
The"Q/A Deadline"has been extended from March 1 @ 3:00pm to
March 6,2024 @ 3:00 pm
CHANGE 2:
On the 10"above ground flow meter assembly just prior to headworks,replace the 10"90 degree
bend upstream of the flow meter with a 10"Tee, 10"Plug Valve with gear actuator and
handwheel,and tapped blind flange. Valve to be oriented such that when in the open position,
the valve plug is rotated out the flow path and to the top of the valve body;valve seat to be
towards the flow meter assembly. Refer to attached figure for more details.
