Agenda 04/09/2024 Item #16D 4 (Approve submission Request For Proposal to the Area Agency on Aging for SW FL. to obtain Community Care for the Elderly)04/09/2024 EXECUTIVE SUMMARY Recommendation to approve an after-the-fact submission Request For Proposal to the Area Agency on Aging for Southwest Florida to obtain the Community Care for the Elderly, Alzheimer’s Disease Initiative, Home Care for the Elderly Lead Agency Designation in order to provide case management services, home and community-based services and other required Lead Agency functions of the Community Care for the Elderly program for a total award amount of $2,285,487.61, including the match requirement of $124,556. OBJECTIVE: To assist functionally impaired elderly persons live dignified and reasonably independent lives in their own homes or the homes of relatives or caregivers through the provision of social services. CONSIDERATIONS: Collier County has been the recipient of the Community Care for the Elderly (CCE), Alzheimer’s Disease Initiative (ADI), and Home Care for the Elderly (HCE) award for over forty-five (45) years. This program provides a continuum of care through the development, expansion, and coordination of multiple community-based services to assist elders in residing in the least restrictive environment suitable to their needs. These grants are funded by the Florida Department of Elder Affairs (FDOEA), through the Area Agency on Agin g for Southwest Florida, Inc. (AAASWFL). Pursuant to § 430.203 and 430.204, F.S., a Lead Agency must be designated for each community care service system. The primary goal of the community care service system is to prevent unnecessary institutionalization of functionally impaired elderly persons through the provision of community-based services. The Lead Agency must be designated through competitive procurement, conducted through a Request for Proposal (RFP). The Lead Agency designation may be renewed or issued on a yearly basis and up to an additional five (5) years, contingent upon satisfactory performance and availability of funds. The Lead Agency is given the authority and responsibility to coordinate some or all services, whether directly or through subcontracts. The purpose of this RFP is to solicit applications from responsive, responsible service agencies and organizations interested in providing case management and coordination of core services and carrying out all functions required by law for the CCE program. As a result of the review of accepted proposals, the designated Lead Agencies will provide case management to all CCE, HCE, and ADI clients. The Lead Agency will be expected to provide case management and coordinate the delivery of approved core services on a countywide/community care service areawide basis. The announcement for the grant was received on January 17, 2024, with a deadline for submittal of March 25, 2024, prior to the next available Board of County Commissioners (Board) meeting on March 26, 2024. Collier County CMA #5330 authorizes the County Manager to approve the submittal of grant applications with subsequent Board action at the next available Board meeting to ratify the approval as “After-the-Fact.” The County Manager approved the submittal of the grant application on March 22, 2024. FISCAL IMPACT: Funding is provided by the Florida Department of Elder Affairs to the Area Agency on Aging for Southwest Florida. Collier County, if awarded, would enter into an annual contract with the AAASWFL for each grant program and the allocation is based on the number of clients served annually. The program includes two (2) methods of payment, cost reimbursement for supplies and unit cost for all direct client services. Annually the Division is required to prepare a unit cost methodology for submission to the grantor to determine the unit rate for the following year. Under this RFP Notice of Available Funding AAASWFL has projected the following award amounts: Program Anticipated Award Anticipated Match Total 16.D.4 Packet Pg. 1101 04/09/2024 Amount Requirement CCE $1,121,000 $124,556 $1,245,556 ADI $984,394 $0 $984,394 HCE $ 55,537.61 $0 $55,537.61 The CCE grant program requires a ten percent (10%) match that has historically been funded by a combination of Human Services Grant Fund (1837) carry forward (as available) supported by excess program revenue, client’s co - pays, required vendor match, and the General Fund (0001). A Budget Amendment will be brought to the Board at a future meeting should the County receive the award. *CCE ten percent (10%) Match is Award divided by .9, multiplied by .1, per grantor requirements for a total of $124,556. LEGAL CONSIDERATIONS: This item has been approved as to form and legality and requires a majority vote for Board approval. -DDP GROWTH MANAGEMENT IMPACT: There is no Growth Management impact associated with this item.
