Attach additional information from the Design Professional and/or Contractor if needed. This change order includes the addition of 153 days (0.5 day for Rain Delay, 1 day for Rock Delay, 1.5 days for WD #1, and 150 days for WD #7) which moves the substantial completion date to December 2, 2024 and the use of$880.00 of project allowance as detailed below: Time Extension Due to Rain 1, This change requests a contract time extension of half(0.5)day due to rain on April 12, 2023. 2, This change was not included in the original contract as it was unknown that heavy rain would impact the area on this specific day. 3. If the time extension is not processed, the Contractor will not be able to complete the project on schedule and would be liable for liquidated damages for no fault of their own. Time Extension Due to Rock 1. This change requests a contract time extension of one(1) day due to encountering cap rock during the installation of water main deflections. This additional one (1)day will cover the time it took to excavate and haul off the rock. 2. This change was not included in the original contract as it was unknown that cap rock was present in this area and would impact construction as the soil borings completed during the design of the project do not show any cap rock. 3. if the time extension is not processed, the Contractor will not be able to complete the project on schedule and would be liable for liquidated damages for no fault of their own. Revised 01/14/2021 (Divisions who may require additional signatures may include on separate sheet.) PROCUREMENT USE ONLY Admin BCC BCC ES -.A, 1 6C 2 Work Directive#1: Water Services Modification I. This change requests a time extension of 1.5 days for the modification of installing single water services instead of double water services in Area 1. 2. This change was not included in the original contract because during construction, property owners requested to leave the meters at their current location to avoid additional landscaping removal in private property. 3. If the time extension is not processed, the contractor will not be able to complete the project on schedule and would be liable for liquidated damages for no fault of their own. Work Directive #1 will not draw from the project Water Allowance and will increase the number of days in the contract by 1.5 days. Work Directive#5: Test Material of Unmarked Pipe 1. This change requestes that the Contractor test and confirm an unidentified and unmarked drainage pipe as asbestos pipe while potholing along Cypress Way East in Area 2. Testing determined that the unidentified and unmarked drainage pipe was asbestos cement pipe and therefore the Contractor would be required to remove the pipe. 2. The change was not included in the original contract as the asbestos pipe was not shown on any record drawings related to the project and unknown to the County. 3. If the change is not processed, the contractor will not be able to fullfil the County's Asbestos Removal Plan. Work Directive#5 will use $880 from the project Storm Allowance but DOES NOT change the contract time. Work Directive#7: Area 1 Gravity Sewer Redesign 1. The contractor encountered excessive ground water conditions at Pump Station 103.08. The owner and design professional investigated the conditions and agreed that it was necessary to redesign the wastewater system in Area I by raising the gravity sanitary system to immediately address this situation to avoid additional delays and minimize disruption to the neighborhood. This change requests that the Contractor proceed with construction based off of the updated design in Area 1. The contractor agreed to pay for additional costs that will be incurred due to the design changes, but the changes will result in additional time to complete the project including: 30 days for engineering redesign of the construction plans, 30 days for redesigning and retrofitting sanitary sewer manholes, 15 days for additional sanitary sewer bypassing, 15 days for grouting the existing sewer main and services, and 60 days for the installation of the additional pump station and force main. The redesign included some upsizing of the gravity main from 8" to 10" diameter. This will result in a section of 10" gravity main installed being paid from the 8"gravity main unit bid price. The daily inspection reports will indicate the materials physically constructed, but they will be measured for payment as included in the conformed construction plans. 2. The change was not included in the original contract as the excessive ground water were unknown and not anticipated. 3. If the changes are not processed, the contractor will not be able to complete the project on schedule and would be liable for liquidated dames for no fault of their own. Work Directive 117 will not draw from the project Wastewater Allowance, but DOES increase the number of days in the contract by 150 days. ENGINEER/DESIGN PROFESSIONAL: Q. Grady Minor& Associates, P.A. You are directed to proceed promptly with the following change(s): Description: Install single short water services to the existing water meter locations for the properties noted below instead of the double short service shown on the construction plans. 1. 563 and 569 Cypress Way E. 2. 436 and 448 Golfview Drive 3. 472 Golfview Drive and 606 Cypress Way E. Purpose of Work Directive Change: The plans currently depict double short water services to the three (3) locations noted above, three (3) double short water services in total. The proposed change will require two (2) single short water services instead of one (1)double short water service at the three(3) locations noted above, resulting in six(6)single short water services in total. Reason/J usti f ication: Water meters and backflows are typically relocated to the property line when the potable water system is replaced and the plans were designed in accordance with this standard. Due to existing constraints with existing power poles, above ground utility pedestals, and landscaping on private property, it is in the best interest of the Project to leave the meters in their existing locations. The six (6) short single water services will be billed at the contract unit price of$1,450 ($1,450 x 6 = $8,700) instead of the three (3) short double water services at the contract unit price of$1,930 ($1,930 x 3 = $5,790) for the locations noted above. There are currently sufficient quantities for single short water services in the contract, Attachments: N/A If a claim is made that the above change(s) have affected Contract/Allowance Price or Contract Times any claim for a Change Order based thereon will involve one or more of the following methods of determining the effect of the changes(s).
