ITEM A-0102 120 A,TEMPORARY SIGNAL ADDITIONAL TIME COST.
1. This Change Order will utilize the Structures Allowance in the amount of$36,889.16 to pay for the temporary signals for
bridges 034834,034835 and 034836.
2. The change being requested was not included in the original contract because the construction plan estimated that each of
the bridge's construction phases would be accomplished within estimated durations.The actual construction phase durations
exceeded the plan estimate.
3. If this modification is not processed the equipment would not be available to safely direct traffic through the construction site
per FDOT Standards.
ITEM B-0455 34 3&0455 34 143 PRESTRESSED CONCRETE PILING, 18"SQ.
1. This Change Order will utilize the Structures Allowance in the amount of$39,501.87 to pay for prestressed concrete piles 18"
SQ preforming at each bridge as per FDOT Specification 455-5.10 and 455-11.12.
2. The change being requested was not included in the original contract because the elevation and depth of preforming was
unknown at the time of design and could only be discovered during the construction phase.The conditions under which
payment will be made for pre-formed pile holes is included under section 455-5.10.3 of the FOOT Standard Specifications for
Road and Bridge Construction.
3. If this modification is not processed the Engineer's requirement and FDOT Specification 455-5.10 for preformed pile holes
would not be met.
ITEM C—550 60 FENCE GATE TYPE B,SINGLE 0-6.0'OPENING(4'OPENING)(2 EACH), BRIDGE 034834.
1. This Change Order will utilize the Structure Allowance in the amount of$2,191.85 to pay for single fence gates with a 4-foot
opening to be installed at the north side of bridge 034834.
2. The gates will provide access for Road Maintenance and FDOT to perform annual inspections and routine maintenance on
the bridge. Due to fencing on private property around this bridge,this change will allow for better and easier access for
personnel and equipment. FDOT Standard Specifications for Road and Bridge Construction sections 550-5.1 and 550-6.3
include the method of compensation for the Type of gates installed.
3. If this modification is not processed it will be much more difficult for the County Road Maintenance Division and FDOT
designated inspection staff to get around the bridge for future inspections and maintenance, potentially requiring special
equipment. Furthermore,the gates are provided to ensure access for environmental personnel to maintain the waterway.

opening to be installed at the south side of bridge 034834.
