EXECUTIVE SUMMARY
Recommendation to approve Change Order No. 1, under Purchase Order No. 4500215169 with U.S. Water
Services Corporation for the Pump Station 121.02 Repair and Renovations project, to add stormwater
improvements to the scope of work included in Work Directive No. 1 in the amount of $115,336.42, to extend
the time of the project for an additional 120 days and authorize the Chairman to sign the attached change
order. (PUD Project Number 70240)
OBJECTIVE: To approve Change Order No. 1, under Purchase Order No. 4500215169 with U.S. Water Services
Corporation for the Pump Station 121.02 Repair and Renovations project, to add stormwater improvements to the
scope of work included in Work Directive No. 1 in the amount of $115,336.42 and extending the project an
additional 120 days.
CONSIDERATIONS: Pump Station (PS) 121.02 is located within the Collier County right-of-way at 57 West
Avenue, approximately 160 feet south of Bonita Beach Road. It receives wastewater flows via a gravity sewer and
repumps wastewater flows from eleven other wastewater pump stations. PS 121.02 is a critical component of the
wastewater infrastructure. It is approximately 46 years old and has reached the end of its useful life.
On October 26, 2021 (Agenda Item 16.C.4), the Board approved the issuance of a Purchase Order to U.S. Water
Services Corporation (“U.S. Water”) to repair and renovate wastewater pump station 121.02 in the amount of
$561,270.71 (the “Project”). The Project material submittals were approved and ordered for fabrication. Based on
the material delivery date confirmed by the manufacturer for the pumps and control panels, it was expected that
construction would occur sometime after February 2023.
During the November 11, 2021, wastewater PS 121.02 pre-construction meeting, staff noticed settlement of West
Avenue adjacent to PS 121.02. Supplementary inspections conducted in early 2022 noted additional settlement. A
service request was processed, and West Avenue repair work was completed in the March/April 20 22 timeframe.
During a second pre-construction meeting on November 30, site investigations revealed additional settlement of
West Avenue in the same area. The Collier County Road Maintenance Division staff investigated and discovered
that an existing 18” stormwater culvert crossing beneath West Avenue directly adjacent to the Pump Station 121.02
site had failed and caused West Avenue roadway to settle.
This scope of work for the Project, which includes the replacement of an old manhole adjacent to the roadway
settlement, cannot be completed without replacing the portion of the stormwater system under West Avenue. To
address this condition, a design for stormwater improvements was completed, and permits for construction
modifications were approved. These changes are necessary to restore the area, ensure proper stormwater drainage,
and facilitate planned improvements while also restoring West Avenue to its original condition.
This Change Order seeks approval to authorize a change in the scope of work for t he U.S. Water purchase order, to
add stormwater improvements to the scope of work. The additional work to be completed includes demolishing,
removing, and disposing of existing stormwater system components in the vicinity of the Pump Station 121.02 site,
furnishing and installing new stormwater system components per the plans, grading the site to allow proper
drainage, and restoring impacted areas. The purpose of staff’s work directive is to authorize a change in the contract
price and timeline to accommodate the stormwater system improvements added in the vicinity of the Pump Station
121.02 site.
The County received a quote for the additional construction work from U.S. Water in the amount of $115,336.42,
which the Engineer of Record (Johnson Engineering, Inc.) and County staff reviewed and found reasonable.
Subject to approval of this Change Order, the new Purchase Order amount will be $676,607.13 (see below chart),
which remains under the $750,000.00 threshold amount provided for in Agreement No. 19-7622 with U.S. Water.

FISCAL IMPACT: The cost of the additional work will be paid by the Collier County Stormwater Maintenance
Division. Funding is available within Stormwater Utility Fund (1005), within the Storm water Maintenance cost
center 172929. A new lump sum line item will be added to PO 4500215169 to cover the stormwater improvements
of $115,336.42.
GROWTH MANAGEMENT IMPACT: This project meets current Growth Management Plan standards to
ensure the adequacy and availability of viable public facilities and to remain in compliance with all regulatory
programs.
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote for

1. Additional Mobilization and Demobilization: Payment for the additional mobilization and
demobilization will be made at the Contract lump sum price. Mobilization shall include all costs
to move the Contractor’s equipment to the site, establish temporary facilities necessary to
perform the Work, bonds, and insurance associated with the stormwater improvements. The cost
for mobilization/demobilization shall not exceed ten percent (10%) of the total additional
stormwater construction cost. Demobilization shall include all costs for disposal of materials,
clean up, removing equipment form the site, restoration, and other miscellaneous items for
completing installation and removing the Contractor’s equipment and temporary facilities from
the site. All disturbed areas shall be sodded unless otherwise noted.
2. Additional Survey and As-Built Drawings: The cost for preparation of additional construction
layout and survey as-builts shall be made at the Contract lump sum price. This item includes
material, labor, and certification to prepare the “As-Builts” for the stormwater improvements,
field verification of existing underground facilities, construction stakeout of the proposed
stormwater improvements, and to survey the new stormwater facilities after they have been
installed. Prior to acceptance of the project by County, the Contractor shall submit two prints,
and one set of computer disk copies of AutoCAD formatted drawings marked as “Survey As-
Builts”. The As-Builts shall include vertical and horizontal alignment of all mains, valves, tees,
bends, reducers, fittings, air release valves, electrical conduits, electrical panels, wiring
diagrams, thrust blocks, concrete collars, concrete pads, driveways, and other pertinent
structures. As-Builts shall be certified by a Professional Land surveyor licensed in the State of
Florida. Horizontal data shall be in Feet and projected on the Florida State Plane Coordinate
System, East Zone, NAD83 (2011). Vertical data shall be in Feet referenced to the North
American Vertical Datum of 1988 (NAVD88).
3. Additional Maintenance of Pedestrian and Vehicular Traffic: Payment to provide additional
maintenance of pedestrian and vehicular traffic associated with the stormwater improvements in
accordance with the County Maintenance of Traffic Policy and the Collier County Growth
Management ROW standards will be made at the Contract unit lump sum price. The Contractor
shall submit an MOT plan to the County ROW department and gain approval prior to
construction.
4. Demolish, Remove, and Dispose of Catch Basin: Payment to demolish, remove, and dispose of
catch basins will be made at the appropriate unit price for each catch basin removed. This item
includes all labor and equipment necessary for abandonment and removal of the existing
structure, protection of existing facilities, excavation, dewatering, surface restoration, cleanup,
and disposal of the materials offsite.
5. Demolish, Remove, and Dispose of 18” RCP: Payment to demolish, remove, and dispose of 18”
RCP will be made at the appropriate unit price per linear foot of pipe removed. This item
includes all labor and equipment necessary for abandonment and removal of the existing pipe,
protection of existing facilities, excavation, dewatering, surface restoration, cleanup, and
disposal of the materials offsite.
6. Demolish, Remove, and Dispose of 12”x18” ERCP: Payment to demolish, remove, and dispose
of 12”x18” ERCP will be made at the appropriate unit price per linear foot of pipe removed.

Removal of the damaged existing stormwater infrastructure, installation of new stormwater system components,
grading of the site to allow proper stormwater drainage, and restoration to impacted areas. These improvements
will prevent further settling of the roadway & washout underneath the adjacent sidewalk.
This project proposes improvements and repairs to Pump Station 121.02 meet the current Collier County Utility
Standards including, but not limited to, replacement of the existing electrical equipment and panels, new
submersible wastewater pumps, provisions for a portable emergency generator, installation of above ground HDPE
discharge piping and valves, relining the interior surface of the wet well and master manhole, and the installation of
a driveway apron to allow County Operations staff a safe parking area. This project also proposes the installation of
a new 8” plug valve on the existing force main located approximately 900 feet south of the pump station near the
intersection of West Avenue and 3rd Street West, within the County ROW. All proposed improvements will be
owned and maintained by Collier County Public Utilities.

Chapter 556, F.S., sets a tolerance zone that extends 24 inches from the outer edge of each side of an
underground facility. Locate marks show the approximate location of an underground facility. To be sure
where that facility is located, you must expose it using a method below:
• Hand digging at an angle toward the facility
• Pot holing
• Soft digging
• Vacuum excavation methods
• Other similar procedures
Exposing the facility lets you see the facility size and the clearance you need to maintain during
excavation. Digging within the tolerance zone requires special precaution and using mechanized
equipment requires a spotter. Within existing pavement, soft digging will be required after removal of
pavement associated with a permitted open-cut.
Condition: CO Hold - As Built Drawings are required prior to final inspection for all Commercial ROW
permits. The as-built drawings may be submitted through the CityView Portal Conditions Tab - click
"Browse" to upload the documents. You must be signed into the CityView Portal as a registered user to
upload the document.
