5. The Chairman's signature line date has been entered as the date of BCC approval of the CB
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's CB
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip CB
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 10/10/23 and all changes made during
the meeting have been incorporated in the attached document. The County kI •
Attorney's Office has reviewed the changes,if applicable.

MODIFICATION #1 TO SUBGRANT AGREEMENT BETWEEN
THE DIVISION OF EMERGENCY MANAGEMENT AND
COLLIER COUNTY
This Modification is made and entered into by and between the State of Florida,
Division of Emergency Management ("the Division"), and Collier County ("Sub-
Recipient"), to modify Contract Number G0380. which began on October 1 . 2022
("the Agreement").
WHEREAS, the Division and the Recipient have entered into the Agreement,
pursuant to which the Division has provided a subgrant to Sub-Recipient under the
Emergency Management Performance Grant-American Rescue Plan Act-Competitive
Grant program of $109,275.00.
WHEREAS, the Agreement expired on September 30. 2023: and,
WHEREAS, the Division and the Sub-Recipient desires to reinstate and extend
the terms of the Agreement.
WHEREFORE, in consideration of the mutual promises of the parties contained
herein, the parties agree as follows:
1 . The Agreement is hereby reinstated and extended as though it had never
expired.
2. Paragraph 3 of the Agreement is hereby amended to read as follows:
(a) This Agreement shall begin October 1 , 2022 and shall end on March
31. 2024, unless terminated earlier in accordance with the provisions
of Paragraph (12) TERMINATION of this Agreement.
All provisions not in conflict with this Modification remain in full force and effect. and are
to be performed at the level specified in the Agreement.
IN WITNESS WHEREOF, the parties hereto have executed this Modification as of
dates set out below.
