EXECUTIVE SUMMARY
Recommendation to approve and authorize the chairman to sign eleven (11) mortgage satisfactions for the
State Housing Initiatives Partnership loan program in the amount of $237,565.90 and approve the associated
Budget Amendment to appropriate repayment amount totaling $237,565.90 (SHIP Grant Fund 1053).
OBJECTIVE: To support the affordability of housing in Collier County through State Housing Initiatives
Partnership (SHIP) down payment and emergency repair and/or rehabilitation assistance programs.
CONSIDERATIONS: SHIP, a state affordable housing program, offers assistance to first-time homebuyers for
use toward a portion of the required down payment and emergency repairs to the newly acquired home, and
rehabilitation assistance to homeowners for rehabilitation to their homesteaded property. As a condition of award,
the homeowner must repay the assistance provided upon sale, refinance, or loss of homestead exemption.
The following table provides details regarding the associated mortgages that have been repaid in full. As such,
satisfactions of mortgage are required.

*The total amount stated on the mortgage modification is $76,110.90. However, the client refused to sign a second
mortgage modification for the additional $45,750 funded awarded after the manufactured home project was
completed. The client requested a payoff letter from staff because she was selling her home. A payoff letter was
provided for the total outstanding balance of $121,860.90 and was paid in full by the client.
**The total amount of $5,000 stated on the mortgage was not dispersed to the clients. They were approved for $2,500
for Down Payment Assistance and $2,500 for SHIP Emergency Rehabilitation. The client only used $2,500 for Down
Payment Assistance to purchase the home.
No payment is required until the property is sold, transferred, refinanced, no longer Homesteaded, or is no longer the
primary residence of the homeowner. If any of the stated conditions occur the homeowner is in default of the
mortgage terms.

If the homeowner occupies the primary residence during the full term of the mortgage loan the assistance provided
is forgiven.
Approval of these items will authorize the Chairman to sign the aforementioned mortgage satisfactions and the
executed documents shall be recorded in the Public Records of Collier County, Florida.
FISCAL IMPACT: The repaid amount of $237,565.90 satisfies the payoff amount. This repayment is
considered program income and has been deposited in SHIP Grant Fund (1053), Project 33807. Dep osited funds
will be used for SHIP -eligible activities. The Budget Amendment recogniz es $237,565.90 in program income, of
which five percent (5%) will be used as administrative funds. A recording fee of $10 has been paid by
homeowners who are in default or sold their homes.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a majority vote for
Board approval. -DDP
GROWTH MANAGEMENT IMPACT: There is no Growth Management impact.
RECOMMENDATION: To approve and authorize the chairman to sign eleven (11) mortgage satisfactions for the

MORTGAGE MODIFICATION AGREEMENT
THIS IS a Mortgage Modification Agreement, made effective as of the ______day
of____________, 2023, by and between Deana Putnam, a single person, hereinafter called
“Borrower” and the Collier County, hereinafter called “Lender.”
WITNESSETH:
In consideration of the mutual covenants contained herein, the Borrower and the Lender
agree that:
1. Promissory Note and Mortgage. Borrower has given to Lender, a promissory
note and mortgage dated August 21, 2018, recorded on September 5, 2018, in OR Book 5549
Page 2395 and re-recorded on April 4, 2019, in OR Book 5615 Page 257 and Mortgage
Modification Agreement recorded on May 7, 2019, in OR Book 5627 Page 835 in the Public
Records of Collier County, Florida. Borrower warrants that it is still the owner of the property.
Lender warrants that it is still owner and holder of said note and mortgage. Both parties warrant
they have good right and authority to enter into the Mortgage Modification Agreement.
2. Status of Payments. There are no payments required and the mortgage and
promissory note are not in default.
3. Modification. In consideration of the mutual covenants herein and other good
and valuable considerations, the parties agree that said promissory note and mortgage are
modified as follows:
(a) The principal balance of the promissory note is now $121,860.90, which shall
increase at the rate of zero percent per annum.
(b) Payment in full is due upon sale, transfer, refinance, or loss of homestead of
property within fifteen years. A refinance is allowable if loan conforms to the County’s approved
Subordination policy guidelines. One-third of the loan will be forgiven in five-year increments so
that at the end of the fifteenth (15th) year the loan is forgiven in full.
4. Other Terms. Notwithstanding anything to the contrary herein or in the Renewal
Note or Mortgage, the lien and operation of the Mortgage shall continue in full force and effect
except as modified by this agreement. All other terms of said note and mortgage remain
unchanged.

5. Compliance. The Borrower covenants and agrees to perform, comply with, and
abide by each and every one of the provisions of the Renewal Note and Mortgage, as modified
hereby.
6. Agreement Binding. This agreement shall be binding upon and inure to the
benefit of the heirs, legal representatives, and assigns of the Borrower and to the successors and
assigns of the Lender.
