7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on June,27 2023 N/A is not

1.The proposed change includes the following:
Monetary increase to be applied toward Task 3—30%Construction Documents in the amount of$126,500.00-Lump Sum Not to
Exceed-and a time extension of 258 days associated to this Task.The Rezoning process pertinent the Collier County Mental Health
Facility project has experienced unforeseen delays due to an unexpected and exceptionally long and challenging Planning Commission
review.These delays required for the Design Team to go beyond the anticipated effort as described under Task 3,and for the
anticipated project schedule to be extended.
2.These changes were not included in the original contract because the additional effort required to addressed the Rezoning process,
specifically,the addressing of all Planning Commission comments,were unforeseen and not anticipated.
3.The impact on this project if this Change Order if not processed is that RG Architects will not be able to Complete Task 3,the Collier
County Mental Health Center design will not be finalized,and the project will be delayed.
Acceptance of this Change Order shall constitute a modification to contract/work order identified above and will be subject to all the same terms
and conditions as contained in the contract I work order indicated above,as fully as if the same were stated in this acceptance. The adjustment,if
any,to the Contract shall constitute a full and final settlement of any and all claims of the Contractor/Vendor/Consultant/Design Professional
arising out of or related to the change set forth herein,including claims for impact and delay costs. CAO
