COUNTY, FLORIDA. (FPN 441846-1-58-01)
WHEREAS, on May 10, 2022,Agenda Item No. 16.A.4, Collier County approved a Local
Agency Program Agreement (the "Agreement") with the State of Florida Department of
Transportation (FDOT), entered into an executed June 15, 2022, wherein FDOT will reimburse
Collier County up to the sum of$467,424 to construct five-foot bike lanes and paved shoulders
along 111th Ave No from Bluebill Avenue Bridge to 7th Street North (FPN 441846-1-58-01); and
WHEREAS, FDOT is willing to supplement the Agreement to increase the reimbursement
to $592,424 and make additional clarifications regarding project scope(Contract No. G2A77); and
WHEREAS, the Collier County Board of Commissioners finds that the supplemental
Agreement is in the public interest and benefits the citizens of Collier County.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that:
1. The Board of County Commissioners approves and authorizes its Chairman to sign the
Supplemental Agreement pertaining to FPN 441846-1-58-01 (Contract No. G2A77).
2. The Collier County Clerk is directed to forward a certified copy of this Resolution along
with the Supplemental Agreement for execution by FDOT.
3. This Resolution shall take effect immediately upon adoption.

PROJECT DESCRIPTION
Name 111th Avenue North from Bluebill Avenue Bridge to 7th Street Length .877 miles
Termini 5' bike lanes and paved shoulders along 111th Avenue North from east of Bluebill Avenue Bridge to 7th Street
North. Full widening at turn locations and keyhole bike lanes included.
Description of Work:
The scope of the project is the construction of 5' bike lanes and paved shoulders along 111th Avenue North from
Bluebill Avenue Bridge to 7th Street North. Elements of the project may also include the relocation of traffic signals and
roadway lighting.
Reason for Supplement and supporting engineering and/or cost analysis:
The purpose of this supplemental agreement#1 is to add additional funding in the amount of$125,000.00 (One Hundred
Twenty-Five Thousand Dollars) in Construction funds in fiscal year 2023 as shown in the attached adjusted Exhibit"B"
Schedule of Financial Assistance.
The Local Agency Program Agreement is amended as follows:
a. Exhibit"B" SCHEDULE OF FINANCIAL ASSISTANCE is replaced in its entirety by the "ADJUSTED EXHIBIT B
SCHEDULE OF FINANCIAL ASSISTANCE" and is incorporated by reference.
b. Section 4 a under project cost on page 1 of the June 15, 2022, Local Agency Program Agreement is replaced in its
entirety by:
The total cost of the project is $592,424.00 (Five Hundred Ninety-Two Thousand Four Hundred Twenty-Four
Dollars). The amount is based upon the schedule of funding in Exhibit"B", Schedule of Financial Assistance
attached to and incorporated in the Agreement. The Agency agrees to bear all expenses in excess of the total
cost of the Project and any deficits involved. The schedule of funding may be modified by mutual agreement as
provided in paragraph 5.1.
