1. This change order will:
a. Add $35,386.00 to the existing Stormwater Drainage Improvement Task which was added through Amendment #1.
Additional funding will pay for preparation of the Technical Specification, Construction Bid Documents, Measurement of
Payment,and Engineer's Opinion of Probable Cost(EOPC).
b. Authorize the use of Additional Service funds in the amount of$12,075,00 to pay for Surficial Soil Sampling and Analysis
along the utility easement where the new force and water mains will be installed within Golden Gate City Golf Course.
This change is due to recent findings from the Golf Course's design team regarding potential soil contaminants within the
Golf Course property which triggered concerns regarding the installation of the new County's utility infrastructure within
the easement located in the Golf Course. In anticipation to the installation of the new utility infrastructure, staff needs to
sample and analyze the soil within the utility easement corridor and provide the result analysis to local and state permitting
agencies.
2. The change being requested was not included in the original contract because it was not anticipated to:
a, Phase the stormwater portion of the project and to sequence the construction within 14 subbasins in Golden Gate City.
b, Encounter potential issues with soils contaminants within the utility easement corridor.
3. If this change order is not processed, staff will not:
a. Be provided with adequate bid documents for the stormwater portion of the project.
b. Be able to provide soil analysis to the permitting agencies when requested,which would cause a delay with construction
commencing.

Request for Authorization to Use Task 4.0 Additional Services Funds
Dear Ben;
Please accept this letter as our request to provide additional professional engineering services using
Task 4,0 Additional Services to provide surficial soil sampling, soil analysis and a soil management plan (if
needed), at the request of Collier County. We propose to provide the following services through our
subconsultant, Universal Engineering Services:
TASK 5.5 Surficial Soil Sampling & Analysis
• Obtain six (6) surficial soil samples from six (6) location along the proposed utility line as shown
on the attached site plan. Soil samples from each location will be collected from approximately 6-
in. below the surface, just below the root zone, in general accordance with Collier County Land
Development Code (LDC), Section 3.08.00A.4.d. Soil samples will be submitted to a NELAP
approved laboratory for the following chemical analysis via United States Environmental
Protection Agency (USEPA) Methods: RCRA 8 Metals 6010; organochlorine 8081,
organophosphorous 8141, carbamates 8318, and triazine 8270 pesticides; and chlorinated
herbicides 8151.
• Deliver Surficial Soil Sampling & Analysis Report to client approximately 3 to 4 weeks following
written project authorization. Report will include summary of field activities, evaluation of data,
findings, and conclusions.
• Soil Management Plan (SMP), if needed, will be completed in general accordance with the FDEP
May 2022 Guidance for Preparation of Soil Management Plans.

The $12,075,00 fees will provide for the above additional services fees to be performed by our
subconsultant, Universal Engineering Services, under Collier Contract #19-7637. Do not hesitate to

Definitions:
Time and Materials (T&M): For the actual hours expended by the Sub-Consultant
professional and technical personnel, multiplied by the applicable hourly rates for each
classification or position on the Sub-Consultant's standard billing rate schedule in effect at the
time the services are rendered. For the actual costs expended by Sub-Consultants engaged by
Consultant and for the expended reimbursable expenses pertaining to the services in Exhibit A.

Dear Mr. Studney:
Collier County desires to have stormwater management improvements constructed within 14
subbasins in Golden Gate City. The locations of the subbasins are shown on the attached Sheet
03 and the construction plans are being provided separately. This proposal provides professional
engineering services during the construction bidding and award phase of the project. The project
will be divided into four distinct and independent construction contracts, with priority as follows
(first being highest): Alignment L-North, Alignment L-South, Alignment B, and all 12 remaining
alignments grouped together. Johnson Engineering, Inc. ("Consultant"),will perform the following
services for Collier County ("County"):
SCOPE OF SERVICES
PHASE 5.6
TECHNICAL SPECIFICATIONS & SERVICES DURING BIDDING
Consultant shall prepare one set of final technical specifications for all alignments which, together
with the four sets of final drawings, provide the general scope, extent, and character of the work
to be furnished and performed by the Contractor. Consultant will prepare one set of
specifications for review and approval by County. An engineer's opinion of probable construction
cost for each of the four alignment groupings will accompany the submittal. Technical
specifications will be based on the I 6-Division format of the Construction Specifications Institute
(CSI) in MS Word format (the Measurement and Payment section is included in Division I and
may be unique to each alignment set). In addition to technical specifications, Consultant will
coordinate with County staff to develop necessary General Conditions, Supplemental Conditions,
and Special Provisions Specifications specifically for the project being constructed. Standard
Collier County Specifications may be used for reference, but the Engineer shall not rely on them
as a replacement for sound engineering judgement and responsibility. Current County Right-of-
Way construction specifications shall be utilized. The first deliverable is anticipated within 30
days of County's acceptance of the 100% construction plans.

Except for the following, bid/quote services will be performed by the Collier County
Procurement Division. For each of the four alignment groupings. Consultant will assist County
with the following:
a. Prepare Minimum Qualifications list for Bidders and provide to the County to include in
the Bidding Documents (to be provided together with the submittals for Taslc I).
b. Assist in preparing addenda, as required, to interpret, clarify, or expand the Bidding
Documents.
c. Consult and advise County as to the acceptability of the Contractor and subcontractors,
suppliers, and other persons and organizations proposed by the Prime Contractor for
those portions of the work as to which such acceptability is required by the Bidding
Documents.
d. Consult with County concerning and determining the acceptability of substitute materials
and equipment proposed by Contractor when substitution prior to the award of
contracts is allowed by the Bidding Documents.
e. Attend a pre-bid/quote conference and the bid opening.
f. Review bids and qualifications of Bidders.
g. Provide a recommendation letter of award along with a "Reference Log" showing the
date, time, and comments of all contacted references provided by the prospective
