EXECUTIVE SUMMARY
Recommendation to approve the First Amendment to an agreement with the Agency for Health
Care Administration and an agreement with Collier Health Services, Inc., d/b/a Healthcare
Network of Southwest Florida and authorize the Chairperson to sign the attached amendment.
OBJECTIVE: To amend the existing agreement with the Agency for Health Care Administration and
Collier Health Services, Inc. d/b/a Healthcare Network of Southwest Florida and with Collier County to
clarify payment distribution for medical services.
CONSIDERATIONS: The Agency for Health Care Administration (AHCA), is a State entity that
provides Medicaid services in Florida and operates the Medicaid Low Income Pool (LIP) Program.
Collier Health Services, Inc. (CHSI) is currently administering the LIP Program with AHCA using
County ad -valorem tax dollars.
The LIP Program leverages local funds to obtain federal matching dollars. CHSI will utilize the total
funds under the LIP Program to provide additional healthcare services for low-income individuals in
Collier County.
The Board of County Commissioners (Board) approved the Agreement on September 28, 2021 (Agenda
Item #I 6D6). The proposed Amendment seeks to provide clarification requested by CHSI to authorize a
payment of $250,000 for eligible services.
FISCAL IMPACT: There is no new Fiscal impact associated with this item. Funds are available
within Community & Human Services FY22 Community Mental Health & LIP Support Cost Center
(001-156010).
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote
for approval. -RTT
GROWTH MANAGEMENT IMPACT: No Growth Management impacts are associated with this item.
RECOMMENDATION: To approve the First Amendment to an agreement with Collier Health
Services Inc., d/b/a Healthcare Network of Southwest Florida and for clarification of payments in an
amount not to exceed $250,000 and authorize the Chairperson to sign the attached amendment.

FIRST AMENDMENT TO AGREEMENT
THIS AMENDMENT, made and entered into on this (date), by and
between Collier County, Florida, a political subdivision of the State of Florida, hereinafter
referred to as "the County" and Collier Health Services, Inc., d/b/a Healthcare Network of
Southwest Florida, a Florida not for profit incorporated under the laws of the State of Florida,
and a Federal Health Qualified Center hereinafter referred to as "Center".
WHEREAS, on September 28, 2021, the County entered into an Agreement with the
Center to become a community health partner to assist in providing payments for health
prevention programs, and medical health services to residents of the County.
WHEREAS, the parties desire to amend the Agreement to provide for payment of
eligible medical services authorized by County staff with proper documentation in an amount not
to exceed $250,000.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein
contained, it is agreed by the parties as follows:
Effective upon execution of this First Amendment, the agreement shall be amended to reflect as
follows:
1. Center will make payments in an amount not to exceed $250,000 for eligible medical
services authorized by County staff. Back up documentation will be provided for
services.
2. Upon execution of this First Amendment, the parties recognize such payments and will
be reviewed retroactively from the beginning of the agreement date of September 28,
2021.
3. Except as modified through this Amendment, all other terms and conditions shall remain
the same.
