Work Directive No.1 is for Installing atop logs in Aeration Basins 6 and 6 for the purpose of Isolating flows for maintenance work and cleaning operations. Work Directive No.2 resulted from uncovering of the existing ductbank system to property tie In the new electrical wiring for the project, the contractor identified additional unmarked circuits within eight of nine existing conduits associated with the protect electrical scope of work,Wiring In one conduit was found to be uncemovabie and contained several damaged wires to the existing motor operated valves(MOVa). The Contractor cited that tracing and labeling of additional incidental wiring is required for a proper compliant and reliable working system. installation of additional conduit in the Electrical Room to reroute one return mixed liquor(RML)pump feed is required to complete the project. Removal and replacement of existing damaged MDV wiring Is necessary to restore operability to the equipment. This additional work requires an additional 01 days to trace down and identify existing unidentified wires, label them accordingly,to install new RML power wiring,to roster() electrical operation to the MOVs,and to reroute ono set of power wires for one RML variaibte frequency drive(VFD)Pump. The total approved contract allowance was$93,000.00.Work Directive No. 1 utilized$73,080.00 from the allowance and Work Directive No.2 utilizes the remaining$19,920.00.The remaining additional required funds in the amount of 3107,633.37 are being sought for epprovai ass pail of this change order. 2. The basins are almost always full of sewage,so once the basins were drained down for construction,it was discovered that stop logs were needed to perform this work and future maintenance and cleaning. Also the condition of the existing electrical components could not have been anticipated and was only discovered during construction once they were uncovered and identified. 3. Without installing the stop logs, isolation of the basins could not he performed and iiirakthe electrical system to the new RML pumps and the now VFDs would not operate or function property. This would hinder sewage treatment capabilities and could potentially put the wastewater operation of the plant In non-compliance with permit requirements. * Project is currently on a Stop Work Notice with 71 days remaining. s c h a I t ►►^^yy Digitally signed by schait_p Prepared by: —r' Date:2022.04.1907:56:22-04'00' Date: Peter Schell, PMP, Senior Project Manager-EPMD Acceptance of this Change Order shall constitute a modification to contract/work order identified above and will be subject to all the same terms and conditions as contained in the contract/work order indicated above,as fully as If the same were stated in this acceptance. The adjustment,If any, to the Contract shall constitute a full and final settlement of any and all claims of the Contractor/Vendor/Consultant/ Design Professional arising out of or related to the change set forth herein, including claims for impact and delay costs. ENGINEER: Hole Montes, Inc. You are directed to proceed promptly with the following change(s): Description: The Contractor shall install County procured stop logs between MLE Aeration Basin 5D and 6A at the common influent channel and common effluent channel. Work proposed by the Contractor for installation of stop logs includes, but it not limited to, chip out concrete (lip) to provide required width for stop logs installation at common influent channel and common effluent channel; install two County supplied stop logs per manufacturer's details and instructions; repair damaged membrane liner and epoxy coatings in influent and effluent channels caused by stop log installation; install grating on top deck to match existing. Contractor shall field verify the dimensions of the influent channel and effluent channel at the locations where the stop logs will be installed. Contractor shall include all work in record drawings. Purpose of Work Directive Change: The installation of stop logs in the common influent channel and common effluent channel will allow the County to isolate flow between Aeration Basin 5 and 6 for maintenance and/or future work. ENGINEER: Hole Monies,Inc. You are directed to proceed promptly with the following change(s): Description: During inspection of the existing ductbank system, the contractor identified additional unmarked circuit within the eight(8) of nine(9)conduits associated with the project scope of work. Wiring in one(1)conduit was found to be unremovable and contained several damaged wires to existing MOVs. Contractor cited that tracing and labeling of additional incidental wiring is required for a working and reliable system. Additional conduit in Electrical Room to reroute one(1)RML pump feed is required. Removal and replacement of existing damage MOV wiring is necessary to restore operability to the devices. Purpose of Work Directive Change: To trace down and identify existing unidentified wires, label accordingly and facilitate installation of new RML power wiring. To restore operation of MOVs. To reroute one(1)set of power wires for one(1)RML,VFD/Purnp. Reason/Justification: Identification of the incidental unmarked wiring will facilitate the removal and installation of the RML VFD/Pump wiring as well as assist in coordination of necessary shutdowns of equipment which maintains adequate facility operations. It will restore operation of the MOVs. It will allow the reuse of existing conduits to refeed one (1) RML VFD/Pump, in lieu of constructing a new ductbank. Quality Enterprises USA, Inc. DATE 01/12/2022 3494 Shearwater Street 3494 Shearwater Street Naples, FL 34114 Naples, FL 34114 ACTIVITY s RA i L AMOUNT January 2022 Services 1 21,474.68 21.474.68 This line item is for the removal of MOV wiring and replacement of MOV wiring which is damaged in conduit, At this moment only 2 of the 4 MOV's are operational because of damaged wiring in existing conduit. We will re-pull new wiring through other pipes in the existing duct bank's. This price does not include time or materials if we are to find another damaged conduit. This price is for the pulling of new MOV wiring as we are pulling our new pump wiring. All terminations will be done at the MCC's and at each of the 4 MOV's. We are replacing existing THHN with new XHHW-2. This line item will extend our RML project a potential of 2 weeks. This Line item is for all new wiring from MCC's to the MOV devices. Labor and wire are included in this price. Services 1 24,533.29 24,533.29 This line item is for the addition of one schedule 80 PVC from MCC which will house pump 1 basin 5's wiring. This new conduit is to replace the broken conduit at the man hole outside of the electrical room (as per on site meeting). All conduit, strut, straps and mounting hardware is included in this price. Also we have determined that there will be addition 65' added to the new pump feeders which is also included in this price. This line item will add a potential of 2 weeks onto the RML project. Services 1 69,400.00 69,400.00 This line item is for the tracing of all wiring in conduits which we are to be pulling our new pump wiring through. There 7 conduits out of the 9 conduits which have other wiring in them. These "other" wires will be traced out to the MCC's they are landed in and labelled with blue painters tape and a conduit number plus a pump number so that we can show the county what they are. At that time we will be able to form a plan with the county as to what will be shut off the day we pull in new wiring. All wiring that is in the pipes we will be pulling through will be removed from the MCC labeled then pulled out to the first man hole, then re-pulled back in with our new pump wiring. All existing wiring and our new pump wiring will then be re-landed in the same day. At the minimum all existing wiring will be re-landed in the same business day 16C6 ACT VIT`>' * , RATE AMOUNT pending there are no problems with the existing conduits as we have found prior to the on site meeting about this line item and tracing wiring for removal the day we pull our pump wiring. This line item will extend the RML project 1.5 months potentially. Please note, tracing wiring will be done manually (by hand) not with a wire toner which cannot be used while wires are energized. We will have to take multiple MCC blank covers off to trace out wiring, MCC covers will have to be replaced daily for safety of Staff and other contractors working on this Plant. Tie wraps will be removed from bundles of wires in MCC's to trace wiring, all tie wraps that will be removed will be replaced. There many wires that we have found previously that loop through other MCC's. Once all wires are traced and labeled on MCC buckets with what pump and basin number they coincide with we will be able to tell the Collier County exactly what devices or MCC's will have to be shut down on the day we pull each of the 9 new pumps circuits. This will give Collier County the opportunity to know if they will have to power some equipment with a portable generator or if these shut downs or some of the shut downs will have to be done at night. After this tracing and labeling is done Swansons Electric will provide 18"x18"vinyl danger stickers at every location there is live energized circuits in a MCC that is feed by another MCC. Again we have found this in one MCC already. Stickers will be approved by engineers and it has already been said by the engineers that they have a sample sticker for this very purpose. Lastly there is a 8th conduit that we will be tracing in this line item, there is a 3" coming out of the both of the MCC closet to the east entry door that houses 2 multi conductor control cables. We believe at least one of these cables goes to one of our existing pump starters. We plan on using this 3"conduit to house Basin 5 pump 1 replacement wiring and the new MOV wiring after we remove control cables if they both belong to the old starters on the old MCC buckets power the existing RML pumps. If we can oly delete one of the existing control cables we cannot pull new pumps or MOV's through this conduit and will have to separate wiring into the other 2 - 3"conduit that are right next to the one with control cables in them.
