(Housing Grant Fund 705) OBJECTIVE: To provide accessible and equitable healthcare systems in the Immokalee farmworker communities to improve response to the COVID-19 pandemic. CONSIDERATIONS: On June 8, 2021 (Agenda Item #16.D.12), the Collier County Board of County Commissioners ("Board") approved the submittal of an application to the Centers for Disease Control and Prevention (CDC) for the Community Health Workers for COVID-19 Response for Resilient Communities (CCR) Grant. This three-year grant program supports the implementation of an outreach project to assist those most impacted and at risk of COVID-19. On September 28, 2021 (Agenda Item #16.D.9), the Board approved a Notice of Award from the CDC, in the amount of $421,744, for the Year 1 budget period of August 31, 2021-August 30, 2022. Year 2 and Year 3 budget periods are subject to future approval by CDC. On December 14, 2021 (Agenda Item #16.D.18), the Board executed a subrecipient agreement with Collier Health Services, Inc. (CHSI), to implement the CDC CCR grant program. Under this agreement, CHSI has hired and is training a team of six (6) Community Health Workers (CHWs) to deliver COVID- 19 related services and serve as resource navigators within medically underserved communities of Collier County. By the end of the three-year project period, individuals living in Collier County will have access to essential healthcare services and individuals living in and around the most vulnerable communities will be directly connected to Community Healthcare Workers. The continuation application for Year 2 award under the CCR program, will allow CHSI to continue delivering these necessary healthcare services within Collier County. County staff became aware of the funding opportunity on February 17, 2022, with a due date of April 21, 2022. Due to the complex nature of the grant application and collaboration with outside agencies, CMA#5330 authorizes the County Manager to approve the submittal of grant applications followed by "After -the -Fact" approval by the Board at the next available Board meeting. The County Manager approved the submittal of the grant application on April 18, 2022. There is no match required for this grant program. FISCAL IMPACT: There is no Fiscal impact at this time. If the Year 2 grant is awarded, funds will be recognized via a Budget Amendment in Housing Grant Fund (705) at a future Board meeting. There is no match required for this grant. Accountant 0.15 FTE: $ 9,931.00 The Accountant will oversee all fiscal aspects of the grant funds including grant payment requests, disbursements, contractual payments, and drawdowns from the grantor agency as applicable. The accountant will also ensure compliance with all other legal requirements of the grant and audit requirements. Operations Analyst 0.05 FTE: $ 3,448.00 The Operations Analyst will provide compliance review and monitoring of grant regulations, performs monitoring visits to sub -grantees and prepares closeout documentation. Fringe Benefit- $ 5,334.00 Collier County budget instruction manual identifies social security and Medicare (FICA) costs to be calculated at a rate of 7.65% (1.45% for Medicare and 6.20% for Social Security) of each employee's salary. Retirement is also available to full time employees and calculated at 11.17%. Benefit rates are standard for all employees and agree with acceptable IRS. Name of Contractor: Collier Health Services, Inc. d/b/a Healthcare Network — Non -Profit Organization 2. Method of Selection: Sole Source. Prior to application to the CDC, Healthcare Network and Partners in Health had already begun developing a Community Health Worker program in Immokalee, Florida, in the wake of the COVID-19 pandemic. These agencies have an established partnership with this program as well as experience in implementing this community -based program in some Extra Mile communities of Collier County, therefore, they were the only agencies capable of implementing this program. 3. Period of Performance: 8/31/2021 — 8/30/2024. Award amounts/budgets are authorized on an annual basis, pending CDC approval. 4. Scope of Work: Healthcare Network will serve as the primary implementing agency of program services. Healthcare Network is an established healthcare provider in several areas of Collier County, including Immokalee and Golden Gate, Florida, that will be beneficial for creating a community -based health response. Healthcare Network will staff the program with Community Health Workers that are recruited from the Extra Mile communities of Collier County and will provide the basic training and orientation to the role in order to deliver services to program participants. Healthcare Network will also be the primary entity responsible for data collection, conducted by the CHWs during client interventions. 5. Method of Accountability: Throughout program implementation, Healthcare Network participates in quarterly Partnership Meetings with Collier County to discuss program progress and corrective action, as necessary. Healthcare Network will also receive a minimum of annual monitoring evaluations from Collier County's Grants Compliance Unit. Day-to-day guidance is provided by the Collier County Grant Coordinator, who is primarily responsible for supervising the contract. Finally, Healthcare Network is contractually obligated to meet CDC requirements for data collection, performance measure reporting frequency, as well as meeting any identified performance targets. 6. Itemized Budget and Justification: Packet Pg. 724 16.D.5.a Personnel- $211,600.00: Six (LOE 80%) Community Health Workers will continue to support Extra Mile communities, at a cost of $3,500.00 per month per CHW ($201,600.00 in total). HCN's Human Resources Manager at 5% LOE ($270/month) for hiring and HR support and CHW Lead at 15% LOE ($562.50/month) for management of the CHWs and project implementation over 12 months, totaling $10,000.00 for Year 2. The Community Relations Director position is dedicated to leading the team of CHWs, specifically regarding data collection and analysis processes. Fringe Benefit- $52,900.00: HCN's fringes include FICA at 7.65%, Retirement at 8.01% and Health Insurance at 9.34% of salary costs and has been applied proportionately to each project role's level of effort. Travel- $18,312.00: Local travel- $18, 312.00: HCN CHW staff will use their own vehicles for travel related to project implementation and will be reimbursed for mileage at a cost of $0.56, the federal GSA rate. Mileage calculation assumes teams of 2 will travel together, and each of the three teams will travel 50 miles per day for 218 annual workdays for a total of 32,700 miles. Supplies- $7,256.00: Folding chairs for COVID-19 education, testing and vaccination events (20 @ $30.00), storage boxes to transport supplies (6 @ $50.00), handcarts (6 @ $100.00), office supplies, including paper, pens, water, snacks ($2,000.00), Testing Team Supplies/PPE (N-95 masks, face shields, etc. for 104 days @ $.O1/day), PPE to distribute during community outreach (218 days at $0.36/each for six CHWs), Hand sanitizers (44 @ $3.69). Other- $7,200.00: Monthly phone plans will be provided for CHW work phones and iPads to enable consistent communication with community stakeholders and clients, at a monthly cost of $50.00 per phone and iPad/tablet (6 phones, $3,600.00; 6 iPads/tablets, $3,600.00) Sub -Contractual- $90,801.00: A. $15,000.00: Local program evaluation services; 100% of contract costs ($15,000.00) are dedicated to support program evaluation. 1. Scope of Work: Dr. Harman and Dr. Goldfarb will: • Attend quarterly meetings for the Collier County CCR grant and provide updates about the status of the program evaluation and activities. • Conduct ongoing process and outcome metrics including tracking of activities, as well as instrument (surveys, focus groups, and interviews) development and data collection with target audiences, key stakeholders, and providers implementing CCR initiatives. These activities will occur virtually unless otherwise agreed upon by both parties. • Document evaluation results as needed and via a Quarterly Progress Report submitted to HcN. • Prepare a Quarterly Progress Report with all evaluation activities and findings to be submitted to HcN no later than July 31, 2023. This document will align with and address the questions as outlined in the HcN Evaluation and Performance Management plan. 5. Method of Accountability: Healthcare Network will be primarily responsible for monitoring their subcontractors, Dr. Jeffrey Harman and Dr. Samantha Goldfarb. 6. Itemized Budget and Justification: Total Payment: $15,000.00 Payment Schedule: $1,250.00/month for on -going project evaluation services (invoices submitted to HcN on the 301h of each month) B. $75,801.00: Healthcare Network will contract with Partners In Health to deliver CHW training, mentorship, program implementation guidance, and program evaluation services. 18% of contract costs ($13,644.18) are dedicated to support program evaluation. 1. Collier County Personnel: • Collier County budgeted $27,289.00, for salaries and benefits, to be expended by 8/30/2022. The revised projection for salaries and benefits is $15,302.00. This revision is due to the program starting later than anticipated. Estimated Collier Copp . unobligated funds in Year 1: $11,987.00 Healthcare Network Personnel: • CHWs budgeted for 9 months @ $251,250.00. Actual will be 5 months @ approximately $140,000.00 for salary and benefits. • HR manager budgeted for 12 months @ $10,500.00. Estimated Healthcare Network unobligated funds in Year 1: $101,977.00 Total Estimated Unobligated Funds in Year 1: $113,964.00 Packet Pg. 729 16.D.5.a Performance Narrative 8/31/2021— 8/30/2022 Successes to date: CBI: Hire & Train • Recruited and hired six (6) bilingual, local community health workers. Developed curriculum for training, including two-day promotors training, led by Partners in Health (PIH), which will be completed by April 8. Additional training is being scheduled at least once each week. • Pre and post -training evaluation forms are being used for CHW` training. These will be reviewed and updated for each CHW training session. • Continued existing relationships with organizations and leaders in priority population communities and developed new relationships with St. Vincent de Paul, Emmanuel Communities, Catholic Charities, Backpacks for the Homeless, Gargiulo Farms, Lipman Family Farms, Fu Time Children's Early Learning Academy, Help a Diabetic Child, Lion's Clubs, Core Health Services, Meals of Hope, Harry Chapin Food Bank, YMCA of SW Florida, St. Matthew's House, Golisano Children's Museum, Bethel Assembly of God, Blue Zones Project (food insecurity initiative) and AVOW (hospice/grief services). • HCN, PIH, CIW and Mision Peniel currently meet every other week to plan and update the workplan. Over time, these will be expanded to include other partners and collaborators. Looking at structures to bring in guest speakers from social support and other organizations to provide update on services available or needs identified in the community. CB3: Deploy - Partnerships • Established relationships with more than a dozen new organizations in or serving priority areas to help identify at -risk populations and provide access to social support services. • Continuing to refine and expand referrals to health services, including primary care at HCN, and social support services. Establishing two-way referrals with other organizations as we learn more about what each group can bring to the table to serve community needs. Packet Pg. 730 16.D.5.a • CHWs conducted successful home visit with client identified by HCN Chronic Care Management (CCM) team. Additional case assignments are being identified by HCN CCM and during COVID-19 testing and health fair events, and door-to-door canvassing and events in Extra Mile communities. HCN maintains a strong relations and good communications with DOH -Collier County. DOH is expanding into other health education at the next scheduled "Super" Spring Health Fair at RCMA on April 30. Success Story: In December 2021, HCN made a connection with St. Vincent de Paul, an organization providing monthly food distributions to priority populations, including migrant farmworker camps in North Naples, an area we had not previously served with our COVID-19 outreach efforts. The organization was looking for a partner to provide access to primary healthcare for their clients. Following participation in an initial Saturday food distribution event at the end of January, and with assistance from the manager of this and another farmworker camp in the immediate area, Healthcare Network's CHW team set up and promoted in advance a mini health fair for families on Saturday, Feb. 27. • 91 attendees (children and adults) • 31 vaccinated (2 children and 29 adults) — vaccines provided by Golisano Children's Hospital • 67 HCN goodie bags distributed (including rapid COVID-19 home test kits) • 51 blood pressure screenings • 46 glucose screenings • 37 bags of food, plus a quantity of food donations provided monthly by St. Vincent de Paul volunteers • 6 PCR COVID-19 tests Packet Pg. 731 16.D.5.a CB4: Deploy — Educating Partners/Messaging Setting up tracking systems to document messages delivered (dates, audiences, venues). • HCN Chief Medical Officer and Training Director are supportive of opportunities to shadow in practices. Establishing strong partnership with HCN Quality Director and Manager of the new HCN Chronic Care Management Team to begin planning protocols to care for priority populations. • Relationships/partnerships are expanded rapidly (see above). Success Story: HCN recently expanded its partnership with a Spanish-speaking radio station in Immokalee, hosted and supported by Bethany Assembly of God. HCN CHWs were invited to participate in interviews about COVID-19 and are now addressing other health issues. A regularly scheduled program has now features HCN CHWs who have talked about their CHW roles in the community and about specific chronic conditions, including diabetes and hypertension. CB5: Promote Engagement • Throughout the Omicron surge in fall and winter, HCN scheduled weekly testing events in Immokalee and Golden Gate, where COVID-19 education, including vaccine education, was provided. Several "combo' events for testing and vaccinations also drew children and adults. Testing events continue in Immokalee, and "pop-up" mini -health fairs with blood pressure and blood sugar screening, are also set up in priority population neighborhoods, including farmworker camps. • Referral systems and protocols are being fine-tuned (see CB4 above). C136: CHW Engagement • Tablets will be available and tracking software secured to begin capturing field data after CHWs have been trained. Packet Pg. 732 16.D.5.a Challenges Recruiting six (6) CHWs from Extra Mile communities: • The pandemic has caused significant challenges in the job market across the country and Southwest Florida is no exception. The unemployment rate is extremely low. • Bilingual individuals who want to serve their communities have been difficult to identify, particularly in the priority population areas. • As CHWs expand awareness of their roles and build trust in Extra Mile communities, they will naturally identify others who may have an interest in CHW roles. This grant program supports the implementation of an outreach project to assist those most impacted and at risk of COVID-19. During year 1, CHS executed a subrecipient agreement with Collier Health Services, Inc. (CHSI), to implement the CDC CCR grant program. Under this agreement, CHSI has hired and is training a team of six Community Health Workers (CHWs) to deliver COVID-19 related services and serve as resource navigators within medically underserved communities of Collier County. The continuation application for Year 2 award under the CCR program, will allow CHSI to continue delivering these necessary healthcare services within Collier County. The total project cost and grant application request is in the amount of $1,446,255 to be obtained over a three-year period. There is no match requirement for this grant. The Centers for Disease Control and Prevention issued a Notification of Funding Availability on February 17, 2022 with applications due no later than April 21, 2022.
