should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 2/22/2022 and all changes made during
the meeting have been incorporated in the attached document. The County

FIFTH AMENDMENT TO
AGREEMENT BETWEEN COLLIER COUNTY, FLORIDA
AND
PRESTIGE HOME CENTERS, INC.
This REINSTATEMENT AND FIFTH AMENDMENT TO AGREEMENT is made and
entered into as of this „la notday of Fe br&a r , 2022, by and between Collier County,
a political subdivision of the State of Florida (the "COUNTY" or "Grantee") and Prestige Home
Centers, Inc., existing under the laws of the State of Florida("SPONSOR").
WITNESSETH
WHEREAS, on May 8, 2018, Agenda Item 16.D.8, the County entered into an Agreement
with Prestige Home Centers, Inc. to administer the State Housing Initiatives Partnership (SHIP)
Demolition and Replacement Manufactured of Home Program;
WHEREAS, on August 14, 2018, Agenda Item 16.D.7, the County approved the First
Agreement Amendment;
WHEREAS, on October 23, 2018, Agenda Item 16.D.5, the County approved the Second
Agreement Amendment;
WHEREAS, on April 9, 2019, Agenda Item 16.D.8, the County approved the Third
Agreement Amendment;
WHEREAS, on July 13, 2021, Agenda Item 16.D.13, the County approved the Fourth
Agreement Amendment;
WHEREAS, the Parties desire to further amend the Agreement to retroactively reinstate,

extend the term of the agreement, revise agreement language and make the agreement consistent
with the 2019-2022 Local Housing Assistance Plan to allow a maximum household award up to
$150,000 for both the housing unit and building site improvements.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained
herein, the parties hereto agree to amend the Agreement as set forth below.

IV. PERIOD OF PERFORMANCE
This Agreement shall be in effect from May 8, 2018 through November 30, 31, 2021 June
30, 2023 for FYs 2016/2017,2017/2018,2018/2019 and 2020/2021 funds and all services
required hereunder shall be completed in accordance with the expenditure deadline set
forth in Exhibit C (Budget Narrative). Funds may be expended throughout the entire
period and are not limited to the fiscal year appropriation.
V. AGREEMENT AMOUNT
It is expressly agreed and understood that the total amount to be disbursed by the COUNTY
for the use by the SPONSOR during the term of the Agreement shall not exceed One
Million Twenty Thousand Six Hundred and Seventy dollars ($1,020,670.00) pursuant to
the quoted price offered by the Sponsor in response to the SHIP application dated
December 04, 2017, and the price methodology as defined below.
