EXECUTIVE SUMMARY
Recommendation to approve the selection committee's ranking and authorize staff to enter into
contract negotiations with Johnson Engineering, Inc., for Request for Professional Services
("RPS") No. 21-7904, "CEI Services for Hamilton Avenue and Danford Street," and to bring a
proposed agreement back for the Board's consideration at a future meeting.
OBJECTIVE: To approve the selection committee's ranking of RPS #21-7904, and to negotiate an
agreement to engage professional Construction Engineering and Inspection ("CEI") services for the
Hamilton Avenue and Danford Street improvement project.
CONSIDERATION: Roadway beautification and parking improvements to Hamilton Avenue and
Danford Street are a component of the Thomason Drive Beautification project. These improvements will
create a seamless pathway from the completed Thomasson Drive beautification project down Hamilton
Avenue, add Bayview Park overflow boat trailer parking as well as a multi -use path to Hamilton Avenue,
and connect to a new sidewalk down Danford Street into Bayview Park.
On June 7, 2021, the Procurement Services Division released notices of RPS #21-7904, "CEI Services for
Hamilton Avenue and Danford Street." The County received four proposals by the July 12, 2021,
deadline. The County found all four firms to be responsive and responsible.
The evaluation criteria included: (1) ability of professional personnel; (2) certified minority business
enterprise; (3) past performance/references; (4) project approach, willingness to meet time and budget
requirements; (5) location; and (6) recent, current, and projected workloads of the firm. Each firm
provided at least three references from clients involving projects of a similar size for which the firm
provided services similar in scope and complexity within the past five years.
A selection committee met on August 24, 2021, scored each of the proposals, and shortlisted all four
firms to move to step 2, presentations. On September 14, 2021, the selection committee met, received
presentations, and ranked the firms as follows:

Staff is requesting authorization to begin negotiations with the top ranked firm, Johnson Engineering Inc.
In the event an agreement cannot be reached with the top ranked firm, staff would continue negotiating
with the remaining firms in ranked order until a proposed agreement can be brought back for the Board's
consideration.
FISCAL IMPACT: Approval of this item does not result in a direct fiscal impact; however, the future
contract (subject to board approval at a subsequent meeting) will require a budget amendment allocating
funding from Parks and/or Bayshore Beautification capital improvement program budgets.
GROWTH MANAGEMENT IMPACT: There are no Growth Management Impacts associated with
this Executive Summary.

INTRODUCTION
As requested by the Facilities Management Division (hereinafter, the "Division"), the Collier County Board of County
Commissioners Procurement Services Division (hereinafter, "County") has issued this Request for Professional Services
(hereinafter, "RPS") with the intent of obtaining proposals from interested and qualified Consultants in accordance with the terms,
conditions and specifications stated or attached. The Consultant, at a minimum, must achieve the requirements of the
Specifications or Scope of Work stated.
BACKGROUND
The beautification project is along Hamilton Avenue and Danford Street. The scope of the project consists of widening existing
roadway, new bike lanes, new sidewalks, new landscaping, and parking spaces.
TERM OF CONTRACT
County reserves the right to modify this scope during negotiations for budgetary reasons.
The contract term, if an award is made, will commence on the date of the Notice to Proceed and end upon acceptance and approval
of the final payment.
Prices shall remain firm for the term of the awarded contract.
Surcharges will not be accepted in conjunction with this award, and such charges should be incorporated into the pricing structure.
DETAILED SCOPE OF WORK
1.0 PURPOSE
This scope of services describes and defines the Construction Engineering and Inspection (CEI) services which are required for
constructability review, value engineering, contract administration, inspection, and materials sampling and testing for the
construction project listed below.
2.0 SCOPE
The project for which the services are required are:

documents prepared by RWA Engineering, Inc. and other design subconsultants to RWA. Plans have been provided as
attachments to this solicitation.

Exercise independent professional judgment in performing obligations and responsibilities under this Agreement. Pursuant to
Section 4.1.4 of the Construction Project Administration Manual (CPAM), the authority of the Consultant's lead person, such as the
Senior Project Engineer, and the Consultant's Project Administrator shall be identical to the Collier Cou

Services provided by the Consultant shall comply with FDOT manuals and all applicable Collier County manuals, standards,
procedures, and memorandums in effect as of the date of execution of the agreement unless otherwise directed in writing by the

On a single Construction Contract, it is a conflict of interest for a professional firm to receive compensation from both the County
and the Contractor either directly or indirectly.
3.0 LENGTH OF SERVICE
CEI contract may begin 1-3 months prior to the construction contract NTP and will end two months after construction completion.
The services for each Construction Contract shall begin upon written notification to proceed by the County. The County
Construction Project Manager shall track the execution of the Construction Contract such that the Consultant is given timely
authorization to begin work. While no personnel shall be assigned until written notification by the County has been issued, the
Consultant shall be ready to assign personnel within two weeks of notification. For the duration of the project, coordinate closely
with the County and Contractor to minimize rescheduling of Consultant activities due to construction delays or changes in c
scheduling of Contractor activities. 0
4.0 DEFINITIONS N
A. Agreement: The Professional Services Agreement between the County and the Consultant setting forth the
obligations of the parties thereto, including but not limited to the performance of the work, furnishing of services,

6.0 ITEMS FURNISHED BY THE CONSULTANT
6.1 FDOT DOCUMENTS
All applicable FDOT documents referenced herein shall be a condition of this agreement. All FDOT documents, directives,
procedures, and standard forms are available through the FDOT's Internet website. Most items can be purchased through the
following address. All others can be acquired through the District Office or on-line at the Department's website.

6.2 OFFICE AUTOMATION
Provide all software and hardware necessary to carry out the responsibilities efficiently and effectively under this agreement.
Provide each inspection staff with a laptop computer running software needed for project scheduling, documentation, and control
(such as, Site Manager application through PROCORE, Citrix, Primavera/Sidetrack, Claim Digger, etc.) connection using a mobile
broadband connection at the jobsite. All computer coding shall be input by Consultant personnel using equipment furnished by
them.
Ownership and possession of computer equipment and related software, which is provided by the Consultant, shall remain at all
times with the Consultant. The Consultant shall retain responsibility for risk of loss or damage to said equipment during
performance of this agreement. Field office equipment should be maintained and operational at all times.

6.3 VEHICLES
Vehicles will be equipped with appropriate safety equipment and must be able to effectively carry out requirements of this
agreement. Vehicles shall have the name and phone number of the consulting firm visibly displayed on both sides of the vehicle.
6.4 FIELD EQUIPMENT
Supply survey, inspection, and testing equipment essential to perform services under this agreement; such equipment includes non-
consumable and non -expendable items. Hard hats shall have the name of the consulting firm visibly displayed. Equipment
described herein and expendable materials under this agreement will remain the property of the Consultant and shall be removed at
completion of the work. Handling of nuclear density gauges shall be in compliance with their license.
Retain responsibility for risk of loss or damage to said equipment during performance of this agreement. Field office equipment
shall be maintained and in operational condition at all times.
6.5 LICENSING FOR EQUIPMENT OPERATIONS
Obtain proper licenses for equipment and personnel operating equipment when licenses are required. The license and supporting
documents shall be available for verification by the County, upon request. Radioactive Materials License for use of Surface
Moisture Density Gauges shall be obtained through the State of Florida Department of Health.
7.0 LIAISON RESPONSIBILITY OF THE CONSULTANT
For the duration of the agreement, consultant must keep the County's Construction Project Manager informed of all significant
activities, decisions, correspondence, reports, and other communications related to its responsibilities under this agreement.
Facilitate communications between all parties (i.e. architectural, mechanical, materials, landscaping, local agencies, etc.) ensuring
responses and resolutions are provided in a timely manner. Maintain accurate records to document the communication process.
Submit all administrative items relating to Invoice Approval, Personnel Approval, User IDs, Time Extensions, change orders and
Supplemental Amendments to the Construction Project Manager for review and approval.

8.0 PERFORMANCE OF THE CONSULTANT
During the term of this agreement and all Supplemental Amendments thereof, the County will review various phases of Consultant
operations, such as construction inspection, materials sampling and testing, and administrative activities, to determine compliance
with this agreement. If deficiencies are indicated, remedial action shall be implemented immediately. County recommendations and
Consultant responses/actions are to be properly documented by the Consultant. No additional compensation shall be allowed for
remedial action taken by the Consultant to correct deficiencies. Remedial actions and required response times may include but are
not necessarily limited to the following:

A. Further subdivide assigned inspection responsibilities, reassign inspection personnel, or assign additional in

personnel, within one week of notification.
B. Immediately replace personnel whose performance has been determined by the Consultant and/or the County to be
inadequate.
C. Immediately increase the frequency of monitoring and inspection activities in phases of work that are the Consultant's
responsibility.
D. Increase the scope and frequency of training of the Consultant personnel.
9.0 REQUIREMENTS OF THE CONSULTANT
9.1 GENERAL
It shall be the responsibility of the Consultant to administer, monitor, and inspect the Construction Contract such that the project is
constructed in reasonable conformity with the plans, specifications, and special provisions for the Construction Contract. Observe
the Contractor's work to determine the progress and quality of work. Identify discrepancies, report significant discrepancies to the
County, and direct the Contractor to correct such observed discrepancies. Consult with the Construction Project Manager as
necessary and direct all issues, which exceed delegated authority to the Construction Project Manager for action or direction.
Inform the Construction Project Manager of any significant omissions, substitutions, defects, and deficiencies noted in the work of
the Contractor and the corrective action that has been directed to be performed by the Contractor.
9.2 SURVEY CONTROL
Check or establish the survey control baseline(s) along with sufficient baseline control points and bench marks at appropriate
intervals along the project in order to: (1) make and record measurements necessary to calculate and document quantities for pay
items, (2) make and record pre -construction and final cross section surveys of the project site in those areas where earthwork (i.e.,
embankment, excavation, subsoil excavation, etc.) is part of the construction project, and (3) perform incidental engineering
surveys.
9.3 ON -SITE INSPECTION
Monitor the Contractor's on -site construction activities and inspect materials entering into the work in accordance with the plans,
specifications, and special provisions for the Construction Contract to determine that the projects are constructed in reasonable
conformity with such documents. Maintain detailed accurate records of the Contractor's daily operations and of significant events
that affect the work. The Consultant will monitor off -site activities and fabrication unless otherwise stipulated by this agreement.
Monitor and inspect Contractor's Work Zone Traffic Control Plan and review modifications to the Work Zone Traffic Control Plan,
including Alternate Work Zone Traffic Control Plan, in accordance with the FDOT's procedures. Consultant employees performing
such services shall be qualified in accordance with the FDOT's guidelines of Traffic Training (Topic No. 625-010-010-a).
9.4 SAMPLING AND TESTING
Perform sampling and testing in conjunction with an independent, licensed laboratory, whose selection has been approved in c
writing by the Construction Project Manager, hired by the Consultant, sampling and testing of component materials and completed
work items so that the materials and workmanship incorporated in the project are in reasonable conformity with the plans, :9
specifications and contract provisions. The minimum sampling frequencies set out in the FDOT's Materials Sampling, Testing and 2
Reporting Guide shall be met. In complying with the aforementioned guide, provide daily surveillance of the Contractor's Quality i0
Control activities and perform the sampling and testing of materials and completed work items for verification and acceptance.
The Consultant will perform inspection and sampling of materials and components at locations remote from the project site and c
perform testing of materials normally done in a laboratory remote from the project site. Determine the acceptability of all materials CD
and completed work items on the basis of either test results or verification of a certification, certified mill analysis, DOT label, N
DOT stamp, etc. The County will monitor the effectiveness of the Consultant's testing procedures through observation and };
independent assurance testing.

Sampling, testing and laboratory methods shall be as required by the FDOT's Standard Specifications, Supplemental Specifications 0
or as modified by the Special Provisions of the Construction Contract. Q
Documentation reports on sampling and testing performed by the Consultant shall be submitted during the same week that the
construction work is done.
Transport laboratory samples to the appropriate laboratory. The Consultant shall provide daily surveillance of the Contractor's
Quality Control activities at the project site in regard to concrete and perform acceptance sampling at the specified frequency.
Supply CTQP (Construction Training Qualification Program) qualified technicians for concrete inspection. Supply CTQP qualified
Asphalt Paving and Plant Technicians for asphalt inspection. Supply CTQP qualified Earthwork Construction Inspection
Technicians for embankment, pipe backfill, subgrade, base and asphalt inspection.
9.5 CONSTRUCTION ENGINEERING AND INSPECTION (CEI)
The consultant shall provide onsite inspection services during construction as necessary to determine if the Contractor is in general

compliance with the construction documents and applicable specifications. Such visits and observations by the consultant 16.C.4.a
intended to be exhaustive or to extend to every aspect of the work in progress, or to involve detailed inspections of the work beyond
the responsibilities specifically assigned to the consultant in the resultant contract. The consultant shall keep the County informed
of the progress of such inspections and results.
The consultant shall not, during such visits or as a result of such observations of the contractor's work in progress, supervise, direct
or have control over the contractor's work nor shall the consultant have authority over or responsibility for the means, methods,
techniques, sequences or procedures of construction selected by the contractor, for safety precautions and programs incident to the
work of the contractor or for any failure of the contractor to comply with laws, rules, regulations, ordinances, codes or orders
applicable to the contractor's furnishing and performing the work. Accordingly, the consultant neither guarantees the performance
of any contractor nor assumes responsibility for any contractor's failure to furnish and perform its work in accordance with the
construction documents.
Services shall include maintaining the required level of surveillance of Contractor activities, interpreting plans, specifications, and
special provisions for the Construction Contract. Maintain complete, accurate records of all activities and events relating to the
project and properly document all project changes. The following services shall be performed:
(1) Attend a pre -service meeting for the agreement within ten (10) business days after the Notice -to- Proceed. At the pre -
service meeting the CONSULTANT shall submit action request packages for personnel approval for immediate staff
needs and a copy/ computer file of the final negotiated staffing. The CONSULTANT shall record a complete and
concise record of the proceedings of the meeting and distribute copies of these minutes to the participants and other
interested parties within seven (7) days of the meeting date.
(2) Prepare and submit to the Construction Project Manager for County approval, within thirty (30) days after the pre -
service meeting, a hard copy of the project specific CONSULTANT contract administration documents listed below
The CONSULTANT contract administration documents, which shall be routinely used by the CONSULTANT
throughout the project, shall be in the format and content provided by Collier County Construction Management
Department.
The CONSULTANT contract administration documents consist of but are not limited to the documents listed below:
• Action Request Form
• Contract Status Report, or other approved fund tracking report
• CEI CONSULTANT Firm Estimated Staffing (completed in accordance with the original CONSULTANT
Contract agreement)
• CEI CONSULTANT Fee Proposal
• CEI CONSULTANT Data
(3) Schedule and conduct a pre -construction conference in accordance with Article 8-3.5 of the FDOT Standard
Specification for Road and Bridge Construction. The purpose of these meetings is to discuss the required
documentation, including as-builts, necessary for permit(s) compliance. Record significant information revealed and
decisions made at this conference and distribute copies of these minutes to the appropriate parties. The meeting shall
also be electronically recorded, and the CONSULTANT shall maintain all tapes of the meeting for the duration of the
Contract.
(4) Consultant should organize and conduct monthly progress meetings, set the agenda, record and provide minutes. The
Consultant shall make use of the existing construction management software (Procore) and input updates to the same
for this project.
(5) Verify that the Contractor is conducting inspections, preparing reports and monitoring all storm water pollution
prevention measures associated with the project. For each project that requires the use of the NPDES General Permit,
provide at least one inspector who has successfully completed the "Florida Stormwater, Erosion, and Sedimentation
Control Training and Certification Program for Inspectors and Contractors". The Consultant's inspector will be
familiar with the requirements set forth in the FEDERAL REGISTER, Vol. 57, No. 187, Friday, September 5, 1992,
pages 4412 to 4435 "Final NPDES General Permits for Storm Water Discharges from Construction Sites" and the
Department's guidelines. Coordinate with the Contractors Public Information Officer to assure updated information
and MOT issues are communicated to the public.
(6) Analyze the Contractor's schedule(s) (i.e. baseline(s), revised baseline(s), updates, as -built, etc.) for compliance with
the contract documents. Elements including, but not limited to, completeness, logic, durations, activity, flow,
milestone dates, concurrency, resource allotment, and delays will be reviewed. Verify the schedule conforms with the
construction phasing and MOT sequences, including all contract modifications. Provide a written review of the
schedule identifying significant omissions, improbable or unreasonable activity durations, errors in logic, and any

Agency's Approved Materials List. Facilitate coordination and communication between Utility Agency's

in the resolution of utility issues including Department and Local Government owned facilities.
Identify, review, and track progress of Joint Project Agreements, and/or other Department and utility agreements.
Address work progress, track reimbursement activities, and address betterment and salvage determination. Prepare all
necessary documentation to support reimbursement activities and betterment and salvage determination.
(9) Produce reports, verify quantity calculations and field measure for payment purposes as needed to prevent delays in
Contractor operations and to facilitate prompt processing of such information in order for the Department to make
timely payment to the Contractor.
(10)Prepare and make presentations for meetings and hearings before the Dispute Review Boards in connection with the
project covered by this agreement.
(11)Monitor each Contractor and Subcontractor's compliance with specifications and special provisions of the
Construction Contract in regard to payment of predetermined wage rates in accordance with FDOT procedures.
(12)Prepare and submit to the Construction Project Manager monthly, a Construction Status Reporting System (CSRS)
report, in a format to be provided by the County.
(13)Video tape the pre -construction conditions throughout the project limits. Provide a digital photo log or video of
project activities, with heavy emphasis on potential claim items/issues and on areas of real/potential public contro-
versy.
(14)Provide a digital camera for photographic documentation of pre -construction state and of noteworthy incidents or
events during construction. Photos should be provided monthly to show progress. These photographs will be filed
and maintained on the Consultant's computer using a Digital Photo Management system.
Photographs shall be taken the day prior to the start of construction and continue as needed throughout the project. Photographs
shall be taken the days of conditional, partial and final acceptance.
10.0 GENERAL REQUIREMENTS
Provide qualified personnel necessary to efficiently and effectively carry out its responsibilities under this agreement.
10.1 PERSONNEL QUALIFICATIONS
Provide competent personnel qualified by experience and education. Submit in writing to the Construction Project Manager the
names of personnel proposed for assignment to the project, including a detailed resume for each containing at a minimum: salary,
education, and experience. The Consultant Action Request form for personnel approval shall be submitted to the Construction
Project Manager at least two weeks prior to the date an individual is to report to work.
Before the project begins, all project staff shall have a working knowledge of the current CPAM and must possess all the necessary
qualifications/certifications for fulfilling the duties of the position they hold. Cross training of the Consultant's project staff is
highly recommended to achieve a knowledgeable and versatile project inspection team but shall not be at any additional cost to the
County and should occur as workload permits. Visit the training page on the State Construction Office website for training dates.
Minimum qualifications for the Consultant personnel are set forth as follows. Exceptions to these minimum qualifications will be
considered on an individual basis. However, a Project Administrator working under the supervision and direction of a Senior
Project Engineer or an Inspector working under the supervision and direction of a Senior Inspector shall have six months from the
date of hire to obtain the necessary qualifications/certifications provided all other requirements for such positions are met and the
Consultant submits a training plan detailing when such qualifications/certifications and other training relative to the FDOT's
procedures, Specifications and Design Standards will be obtained. The County Construction Manager or designee will have the
final approval authority on such exceptions.
The Senior Project Engineer assigned to the project must have prior experience in roundabout construction and must have advanced
MOT certification (10.1 Certification with DOT).

10.2 STAFFING
Once authorized, the Consultant shall establish and maintain appropriate staffing throughout the duration of construction and
completion of the final estimate. Responsible personnel, thoroughly familiar with all aspects of construction and final
measurements of the various pay items, shall be available to resolve disputed final pay quantities until the County has received a
regular acceptance letter.

Construction engineering and inspection forces will be required of the Consultant while the Contractor is working. If Contractor
operations are substantially reduced or suspended, the Consultant will reduce its staff appropriately.
In the event that the suspension of Contractor operations requires the removal of Consultant forces from the project, the Consultant
will be allowed ten (10) days maximum to demobilize, relocate, or terminate such forces.
11.0 QUALITY ASSURANCE (QA) PROGRAM
11.1 QUALITY ASSURANCE PLAN
Within thirty (30) days after receiving award of an agreement, furnish a QA Plan to the Construction Project Manager. The QA
Plan shall detail the procedures, evaluation criteria, and instructions of the Consultant's organization for providing services
pursuant to this agreement. Unless specifically waived, no payment shall be made until the County approves the Consultant QA
Plan. Significant changes to the work requirements may require the Consultant to revise the QA Plan. It shall be the responsibility
of the Consultant to keep the plan current with the work requirements. The Plan shall include, but not be limited to, the following
areas:
A. Organization
A description is required of the Consultant QA Organization and its functional relationship to the part of the organization
performing the work under the agreement. The authority, responsibilities and autonomy of the QA organization shall be
detailed as well as the names and qualifications of personnel in the quality control organization.
B. Quality Assurance Reviews
Detail the methods used to monitor and achieve organization compliance with agreement requirements for services and
products.
C. Quality Assurance Records
Outline the types of records which will be generated and maintained during the execution of the QA program.
D. Control of Subconsultants and Vendors
Detail the methods used to control subconsultant and vendor quality.
E. Quality Assurance Certification
An officer of the Consultant firm shall certify that the inspection and documentation was done in accordance with FDOT
specifications, plans, standard indexes, and Department procedures.
11.2 QUALITY ASSURANCE REVIEWS
The CCEI shall perform an initial QA review within the first month of the start of construction.

Consultant personnel preparing the Certified Final Estimate Package shall be CTQP Final Estimates Level II. Duly authorized
representative of the Consultant firm will provide a notarized certification on a form pursuant to FDOT's procedures.
The County's Project Administrator/Manager shall be responsible for verifying and maintaining copies of all required licenses,
certifications, and qualifications requested from the Consultant throughout the term of the contract.
12.3 OFFER OF FINAL PAYMENT
Prepare the Offer of Final Payment package as outlined in Chapter 14 of the Review and Administration Manual. The package
shall accompany the Certified Final Estimates Package submitted to the Construction Project Manager for review. The Consultant
shall be responsible for forwarding the Offer of Final Payment Package to the Contractor.
13.0 AGREEMENT MANAGEMENT

13.1 GENERAL 16.C.4.a
With each monthly invoice submittal, the Consultant will provide a status report for the agreement. This report will provide an
accounting of the additional agreement calendar days allowed to date, an estimate of the additional calendar days anticipated to be
added to the original schedule time, an estimate of the agreement completion date, and an estimate of the Consultant funds
expiration date per the agreement schedule for the prime Consultant and for each subconsultant. The Consultant will provide a
printout from the Equal Opportunity Reporting System showing the previous month's payments made to subconsultants. Invoices
not including this required information may be rejected.
When the Consultant identifies a condition that will require an amendment to the agreement, the Consultant will communicate this
need to the Construction Project Manager for acceptance. Upon acceptance, prepare and submit an Amendment Request (AR), and
all accompanying documentation to the Construction Project Manager for approval and further processing. The AR is to be
submitted at such time to allow the County 12 weeks to process, approve, and execute the AR. The content and format of the AR
and accompanying documentation shall be in accordance with the instructions and format to be provided by the County.
The Consultant is responsible for performing follow-up activities to determine the status of each Amendment Request submitted to
the County.
14.0 OTHER SERVICES
Upon written authorization by the Construction Project Manager, the Consultant will perform additional services in connection with
the project not otherwise identified in this agreement. The following items may be required by the County to supplement the
Consultant services under this agreement.
A. Assist in the timely preparation of all claims and necessary rebuttals with the organization of all supporting
documentation as needed or required by Collier County pursuant to the Collier County Alternative Dispute
Resolution Procedures.
B. Provide qualified engineering witnesses and exhibits for any litigation or hearings in connection with the Contract.
C. Assist the County Attorney or his designee as directed in the preparation for Binding Arbitration and Pre -Suit
Mediation.
D. Provide on- and off -site inspection services in addition to those provided for in this agreement.
14.2 CONTRADICTIONS
In the event of a contradiction between the provisions of this Scope of Services and the Consultant's proposal as made a part of
their agreement, the provisions of the scope of services shall apply.
14.3 COUNTY AUTHORITY

The County shall be the final authority in considering modifications to the Construction Contract for time, money or any other

1.1 The selection process will involve a two-step proposal review to minimize Proposers' costs in preparing initial
qualification proposals and expediting the review process. The Proposers will initially submit a qualifications proposal
which will be scored based on the Scoring Criteria for Development of Shortlist, which will be the basis for short -listing
no less than three (3) firms.
The Proposers will need to meet the minimum requirements outlined herein in order for their proposal to be evaluated and
scored by the COUNTY. The COUNTY will then issue an invitation for oral presentations (Step 2) to at least the top
three short-listed firms and produce a final ranking for approval by the Board of County Commissioners. Upon approval,
the County will enter into negotiations with the top ranked firms to establish cost for the services needed. With successful
negotiations, a contract will be developed with the selected firm, based on the negotiated price and technical components.
The COUNTY plans to include an allowance in the contract for items such as changes in scope and permit acquisition.
1.2 The COUNTY will use a Selection Committee in the Request for Professional Services selection process.
1.3 The intent of the Step 1 scoring of the qualifications proposal is for consultants to indicate their interest, relevant
experience, financial capability, staffing and organizational structure.
1.4 Based upon a review of these qualification proposals, the short-listed firms will be requested to submit Step 2, to make
oral presentations.
1.5 The scores from the RPS - Step 1 used for short -listing for this Project will not be carried forward for the Step 2 of this
selection process.
1.6 The intent of the Step 2 oral presentations is to provide the firms with a venue where they can conduct discussions with the
Selection Committee to clarify questions and concerns before providing a final rank.
1.7 After review of the Step 2 oral presentations, the COUNTY will rank the Proposers based on the discussion and clarifying
questions on their approach and related criteria, and then negotiate in good faith an Agreement with the top ranked
Proposer.
1.8 If, in the sole judgment of the COUNTY, a contract cannot be successfully negotiated with the top -ranked firm,
negotiations with that firm will be formally terminated and negotiations shall begin with the firm ranked second. If a
contract cannot be successfully negotiated with the firm ranked second, negotiations with that firm will be formally
terminated and negotiations shall begin with the third ranked firm, and so on. The COUNTY reserves the right to
negotiate any element of the proposals in the best interest of the COUNTY.

Tie Breaker: In the event of a tie at final ranking, award shall be made to the proposer with the lower volume of work
previously awarded. Volume of work shall be calculated based upon total dollars paid to the proposer in the twenty-four

system of record. The tie breaking procedure is only applied in the final ranking step of the selection process and is
invoked by the Procurement Services Division Director or designee. In the event a tie still exists, selection will be
determined based on random selection by the Procurement Services Director before at least three (3) witnesses.

3.1 The purpose of the pre -proposal conference is to allow an open forum for discussion and questioning with County staff
regarding the RPS with all prospective Consultants having an equal opportunity to hear and participate. Oral questions will
receive oral responses, neither of which will be official, nor become part of the RPS. Only written responses to written
questions will be considered official, and will be included as part of the RPS as an addendum.
3.2 All prospective Consultants are strongly encouraged to attend, as, this will usually be the only pre -proposal conference for

4. COMPLIANCE WITH THE RPS

Proposals must be in strict compliance with this RPS. Failure to comply with all provisions of the RPS may result in

5. AMBIGUITY, CONFLICT, OR OTHER ERRORS IN THE RPS
It is the sole responsibility of the Consultant if the Consultant discovers any ambiguity, conflict, discrepancy, omission or other
error in the RPS, to immediately notify the Procurement Strategist, noted herein, of such error in writing and request
modification or clarification of the document prior to submitting the proposal. The Procurement Strategist will make
modifications by issuing a written revision and will give written notice to all parties who have received this RPS from the
Procurement Services Division.
6. ADDENDA
The County reserves the right to formally amend and/or clarify the requirements of the solicitation where it deems necessary.
Any such addendum/clarification shall be in writing and notifications shall be distributed electronically to all parties who
received the original RPS prior to the deadline for submission of proposals. All changes to this RPS will be conveyed
electronically through a notice of addendum or questions and answers to all Vendors registered under the applicable
commodity code(s) at the time when the original RPS was released, as well as those Vendors who downloaded the RPS

Rank Form is merely a tool to assist the selection committee member in their review of the proposals.
11.2.7 At the publicly noticed selection committee meeting, the members will present their independent findings /
conclusions / comments based on their reading and interpretation of the materials presented to each other, and may
ask questions of one another. Time will be allowed for public comment.
11.2.8 Collier County selection committee members may consider all the material submitted by the Proposer and other
information Collier County may obtain to determine whether the Proposer is capable of and has a history of
successfully completing projects of this type, including, without limitation, additional information Collier County
may request, clarification of proposer information, and/or additional credit information.
11.2.9 Once the individual scoring has been completed, the Procurement professional will read the results publicly.

14.2 The County and the Consultant waive against each other and the County's separate Consultants, Contractors, Design
Consultant, Subcontractors agents and employees of each and all of them, all damages covered by property insurance
provided herein, except such rights as they may have to the proceeds of such insurance. The Consultant and County shall,
where appropriate, require similar waivers of subrogation from the County's separate Consultants, Design Consultants and
Subcontractors and shall require each of them to include similar waivers in their contracts.
14.3 Collier County shall be responsible for purchasing and maintaining, its own liability insurance.
14.4 Certificates issued as a result of the award of this solicitation must identify "For any and all work performed on behalf of
Collier County."
14.5 The General Liability Policy provided by Consultant to meet the requirements of this solicitation shall name Collier
County, Florida, as an additional insured as to the operations of Consultant under this solicitation and shall contain a
severability of interests provisions.

14.7 The amounts and types of insurance coverage shall conform to the minimum requirements set forth in Insurance and
Bonding attachment, with the use of Insurance Services Office (ISO) forms and endorsements or their equivalents. If
Consultant has any self -insured retentions or deductibles under any of the below listed minimum required coverage,
Consultant must identify on the Certificate of Insurance the nature and amount of such self- insured retentions or
deductibles and provide satisfactory evidence of financial responsibility for such obligations. All self -insured retentions or
deductibles will be Consultant's sole responsibility.
14.8 Coverage(s) shall be maintained without interruption from the date of commencement of the Work until the date of
completion and acceptance of the scope of work by the County or as specified in this solicitation, whichever is longer.
14.9 The Consultant and/or its insurance carrier shall provide 30 days written notice to the County of policy cancellation or
non -renewal on the part of the insurance carrier or the Consultant. The Consultant shall also notify the County, in a like
manner, within twenty-four (24) hours after receipt, of any notices of expiration, cancellation, non -renewal or material
change in coverage or limits received by Consultant from its insurer and nothing contained herein shall relieve Consultant
of this requirement to provide notice. In the event of a reduction in the aggregate limit of any policy to be provided by
Consultant hereunder, Consultant shall immediately take steps to have the aggregate limit reinstated to the full extent
permitted under such policy.
14.10 Should at any time the Consultant not maintain the insurance coverage(s) required herein, the County may terminate the
Agreement or at its sole discretion shall be authorized to purchase such coverage(s) and charge the Consultant for such
coverage(s) purchased. If Consultant fails to reimburse the County for such costs within thirty (30) days after demand, the
County has the right to offset these costs from any amount due Consultant under this Agreement or any other agreement
between the County and Consultant. The County shall be under no obligation to purchase such insurance, nor shall it be
responsible for the coverage(s) purchased or the insurance company or companies used. The decision of the County to
purchase such insurance coverage(s) shall in no way be construed to be a waiver of any of its rights under the Contract
Documents.
14.11 If the initial or any subsequently issued Certificate of Insurance expires prior to the completion of the scope of work, the
Consultant shall furnish to the County renewal or replacement Certificate(s) of Insurance not later than ten (10) calendar 0
days after the expiration date on the certificate. Failure of the Consultant to provide the County with such renewal 00
certificate(s) shall be considered justification for the County to terminate any and all contracts. N

18.1 The selected Consultant shall be required to sign a standard Collier County contract or accept the County's Purchase
Order terms and conditions to serve as a formal contact.
18.2. The resultant contract(s) may include purchase or work orders issued by the County's Project Manager (a/k/a Contract
Administrative Agent).
18.3 The County reserves the right to include in any contract document such terms and conditions, as it deems necessary for
the proper protection of the rights of Collier County. A sample copy of this contract is available upon request. The County
will not be obligated to sign any contracts, maintenance and/or service agreements other documents or agree to any
exceptions to the County's terms and conditions provided by the Consultant.
18.4 The County's Project Manager shall coordinate with the Vendor / Contractor the return of any surplus assets, including

21.2 Payments will be made for articles and/or services furnished, delivered, and accepted, upon receipt and approval of
invoices submitted on the date of services or within six (6) months after completion of contract. Any untimely submission
of invoices beyond the specified deadline period is subject to non-payment under the legal doctrine of "laches" as
untimely submitted. Time shall be deemed of the essence with respect to the timely submission of invoices under this
agreement.
21.3 In instances where the successful contractor may owe debts (including, but not limited to taxes or other fees) to Collier
County and the contractor has not satisfied nor made arrangement to satisfy these debts, the County reserves the right to
off -set the amount owed to the County by applying the amount owed to the vendor or contractor for services performed of
for materials delivered in association with a contract.
21.4 Invoices shall not reflect sales tax. After review and approval, the invoice will be transmitted to the Finance Division for
payment. Payment will be made upon receipt of proper invoice and in compliance with Chapter 218 Florida Statutes,
otherwise known as the "Local Government Prompt Payment Act." Collier County reserves the right to withhold and/or
reduce an appropriate amount of any payment for work not performed or for unsatisfactory performance of Contractual
requirements.
22. ENVIRONMENTAL HEALTH AND SAFETY
22.1 All Consultants and Sub Consultants performing service for Collier County are required and shall comply with all
Occupational Safety and Health Administration (OSHA), State and County Safety and Occupational Health Standards and
any other applicable rules and regulations. Consultants and Sub Consultants shall be responsible for the safety of their
employees and any unsafe acts or conditions that may cause injury or damage to any persons or property within and
around the work site. All firewall penetrations must be protected in order to meet Fire Codes.
22.2 Collier County Government has authorized OSHA representatives to enter any Collier County facility, property and/or
right-of-way for the purpose of inspection of any Consultant's work operations. This provision is non-negotiable by any
department and/or Consultant.
22.3 All new electrical installations shall incorporate NFPA 70E Short Circuit Protective Device Coordination and Arc Flash
Studies where relevant as determined by the engineer.
22.4 All electrical installations shall be labeled with appropriate NFPA 70E arch flash boundary and PPE Protective labels.
23. POLLUTION PREVENTION
The vendor is required to implement industry relevant pollution prevention and best management practices. Should pollution
incidents occur, Collier County Pollution Control must be notified immediately.
24. LICENSES
24.1 The Consultant is required to possess the correct Business Tax Receipt, professional license, and any other authorizations o
necessary to carry out and perform the work required by the project pursuant to all applicable Federal, State and Local
Law, Statute, Ordinances, and rules and regulations of any kind. Additionally, copies of the required licenses must be
submitted with the proposal response indicating that the entity proposing, as well as the team assigned to the County o
account, is properly licensed to perform the activities or work included in the contract documents. Failure on the part of C0
any Consultant to submit the required documentation may be grounds to DEEM VENDOR NON- c
RESPONSIVE. A Consultant, with an office within Collier County is also required to have an occupational license. ti
24.2 All State Certified contractors who may need to pull Collier County permits or call in inspections must complete a Collier N
County Contractor License registration form and submit the required fee. After registering the license/registration will c
need to be renewed thereafter to remain "active" in Collier County. E

25. SURVIVABILITY
The Vendor agrees that any Purchase Order/Work Order/Solicitation Documents that extends beyond the expiration date of the
original Solicitation will survive and remain subject to the terms and conditions of that Agreement until the completion or
termination.
26. PRINCIPAL/COLLUSION
By submission of this Proposal the undersigned, as Consultant, does declare that the only person or persons interested in this
Proposal as principal or principals is/are named therein and that no person other than therein mentioned has any interest in this
Proposal or in the contract to be entered into; that this Proposal is made without connection with any person, company or
parties making a Proposal, and that it is in all respects fair and in good faith without collusion or fraud.

27. RELATION OF COUNTY

It is the intent of the parties hereto that the Consultant shall be legally considered an independent Consultant, and that neither
the Consultant nor their employees shall, under any circumstances, be considered employees or agents of the County, and that
the County shall be at no time legally responsible for any negligence on the part of said Consultant, their employees or agents,
resulting in either bodily or personal injury or property damage to any individual, firm, or corporation.
28. TERMINATION
Should the Consultant be found to have failed to perform services in a manner satisfactory to the County, the County may
terminate this Agreement immediately for cause; further the County may terminate this Agreement for convenience with a
thirty (30) day written notice. The County shall be sole judge of non-performance. In the event that the award of this
solicitation is made by the Procurement Services Director, the award and any resultant purchase orders may be terminated at
any time by the County upon thirty (30) days written notice to the awarded vendor(s) pursuant to the Board's Procurement
Ordinance.
29. LOBBYING
After the issuance of any solicitation, no current or prospective vendor or any person acting on their behalf, shall contact,
communicate with or discuss any matter relating to the solicitation with any Collier County employee or elected or appointed
official, other than the Procurement Services Director or his/her designees. This prohibition ends upon execution of the final
contract or upon cancellation of the solicitation. Any current or prospective vendor that lobbies any Collier County employee
or elected or appointed official while a solicitation is open or being recommended for award (i) may be deemed ineligible for
award of that solicitation by the Procurement Services Director, and (ii) will be subject to Suspension and Debarment outlined
in section Twenty -Eight of County Procurement Ordinance 2017-08, as amended.
30. CERTIFICATE OF AUTHORITY TO CONDUCT BUSINESS IN THE STATE OF FLORIDA (Fl Statute 607.1501)
In order to be considered for award, firms must be registered with the Florida Department of State Divisions of Corporations in
accordance with the requirements of Florida Statute 607.1501 and provide a certificate of authority
(www.sunbiz.org/search.httnl) prior to execution of a contract. A copy of the document should be submitted with the
solicitation response and the document number should be identified. Firms who do not provide the certificate of authority at the
time of response shall be required to provide same within five (5) days upon notification of selection for award. If the firm
cannot provide the document within the referenced timeframe, the County reserves the right to award to another firm.
31. SINGLE PROPOSAL
Each Consultant must submit, with their proposal, the required forms included in this RPS. Only one proposal from a legal
entity as a primary will be considered. A legal entity that submits a proposal as a primary or as part of a partnership or joint
venture submitting as primary may not then act as a sub -consultant to any other firm submitting under the same RPS. If a legal
entity is not submitting as a primary or as part of a partnership or joint venture as a primary, that legal entity may act as a sub -
consultant to any other firm or firms submitting under the same RPS. All submittals in violation of this requirement will be
deemed non -responsive and rejected from further consideration.
32. PROTEST PROCEDURES
32.1 Any Vendor who alleges to be aggrieved in connection with the solicitation or award of a contract, may protest to the
Procurement Services Director, who shall serve as the sole receipt of the any and all notices of intent to protest and all
formal protests.
32.2With respect to a protest of the terms, conditions and specifications contained in a solicitation, including any provisions
governing the methods for evaluation of bids, proposals or replies, awarding contracts, reserving rights for further
negotiation or modifying or amending any contract, the protesting party shall file a notice of intent to protest within three
(3) days, excluding weekends and County holidays, after the first publication, whether by posting or formal advertisement
of the solicitation. The formal written protest shall be filed within five (5) days of the date the notice of intent is filed.
Formal protests of the terms, conditions and specifications shall contain all of the information required for the
Procurement Services Director, to render a decision on the formal protest and determine whether postponement of the bid
opening or proposal/response closing time is appropriate. The Procurement Services Director's decision shall be
considered final and conclusive unless the protesting party files an appeal of the Procurement Services Director's decision.
32.3Any actual proposer or respondent to who desires to protest a recommended contract award shall submit a notice of intent
to protest to the Procurement Services Director within three (3) calendar days, excluding weekends and County holidays,
from the date of the initial posting of the recommended award.
32.4A11 formal protests with respect to a recommended contract award shall be submitted in writing to the Procurement
Services Director for a decision. Said protests shall be submitted within five (5) calendar days, excluding weekends and
County holidays, from the date that the notice of intent to protest is received by the Procurement Services Director, and

33. PUBLIC ENTITY CRIME

A person or affiliate who has been placed on the convicted Consultant list following a conviction for a public entity crime may
not submit a bid, proposal, or reply on a contract to provide any goods or services to a public entity; may not submit a bid,
proposal, or reply on a contract with a public entity for the construction or repair of a public building or public work; may not
submit bids, proposals, or replies on leases of real property to a public entity; may not be awarded or perform work as a
contractor, supplier, subcontractor, or consultant under a contract with any public entity; and may not transact business with
any public entity in excess of the threshold amount provided in s. 287.017 for CATEGORY TWO for a period of 36 months
following the date of being placed on the convicted Consultant list.
34. SECURITY AND BACKGROUND CHECKS
34.1 The Contractor is required to comply with County Ordinance 2004-52, as amended. Background checks are valid for five
(5) years and the Contractor shall be responsible for all associated costs. If required, Contractor shall be responsible for the
costs of providing background checks by the Collier County Facilities Management Division for all employees that shall
provide services to the County under this Agreement. This may include, but not be limited to, checking federal, state and
local law enforcement records, including a state and FBI fingerprint check, credit reports, education, residence and
employment verifications and other related records. Contractor shall be required to maintain records on each employee
and make them available to the County for at least four (4) years.
34.2 All of Contractor's employees and subcontractors must wear Collier County Government Identification badges at all
times while performing services on County facilities and properties. Contractor ID badges are valid for one (1) year from
the date of issuance and can be renewed each year at no cost to the Contractor during the time period in which their
background check is valid, as discussed below. All technicians shall have on their shirts the name of the contractor's
business.
34.3 The Contractor shall immediately notify the Collier County Facilities Management Division via e-mail (DL-
FMOPS@colliergov.net) whenever an employee assigned to Collier County separates from their employment. This
notification is critical to ensure the continued security of Collier County facilities and systems. Failure to notify within
four (4) hours of separation may result in a deduction of $500 per incident.
34.4 CCSO requires separate fingerprinting prior to work being performed in any of their locations. This will be coordinated
upon award of the contract. If there are additional fees for this process, the vendor is responsible for all costs.
35. CONFLICT OF INTEREST
Consultant shall complete the Conflict of Interest Affidavit. Disclosure of any potential or actual conflict of interest is subject
to County staff review and does not in and of itself disqualify a firm from consideration. These disclosures are intended to
identify and or preclude conflict of interest situations during contract selection and execution.
36. PROHIBITION OF GIFTS TO COUNTY EMPLOYEES
No organization or individual shall offer or give, either directly or indirectly, any favor, gift, loan, fee, service or other item of
value to any County employee, as set forth in Chapter 112, Part III, Florida Statutes, the current Collier County Ethics
Ordinance and County Administrative Procedure 5311. Violation of this provision may result in one or more of the following
consequences: a. Prohibition by the individual, firm, and/or any employee of the firm from contact with County staff for a
specified period of time; b. Prohibition by the individual and/or firm from doing business with the County for a specified
period of time, including but not limited to: submitting bids, RPS, and/or quotes; and, c. immediate termination of any contract
held by the individual and/or firm for cause.
37. IMMIGRATION LAW AFFIDAVIT CERTIFICATION
37.1 Statutes and executive orders require employers to abide by the immigration laws of the United States and to employ only
individuals who are eligible to work in the United States.
37.2 The Employment Eligibility Verification System (E-Verify) operated by the Department of Homeland Security (DHS) in
partnership with the Social Security Administration (SSA), provides an Internet -based means of verifying employment
eligibility of workers in the United States; it is not a substitute for any other employment eligibility verification
requirements. The program will be used for Collier County formal Invitations to Bid (ITB) and Request for Proposals
(RFP) including Request for Professional Services (RPS) and construction services.

and regulations relating thereto, as either may be amended and with the provisions contained within this affidavit. Failure
by the awarded firm(s) to comply with the laws referenced herein or the provisions of this affidavit shall constitute a
breach of the award agreement and the County shall have the discretion to unilaterally terminate said agreement
immediately.

On June 7, 2021 the Procurement Services Division released notices for Request for Professional Services 21-
7904 CEI Services for Hamilton Avenue and Danford Street. The County notified 7,703 firms, 66 firms viewed
the information, and four (4) responsive and responsible proposals were received by the July 12, 2021
submission deadline.
Per CCNA, Fla. Stat. the evaluation criteria included ability of professional personnel; certified minority
business enterprise; past performance/references; project approach, willingness to meet time and budget
requirements; location; and recent, current, and projected workloads of the firm. A minimum of three
references were requested from clients for projects with a similar size for which the firm provided services
similar in scope and complexity.
The selection committee convened on August 24, 2021 and as described in step 1 of the solicitation
documents, scored each of the proposals and shortlisted all four firms to move to step 2 in the order listed
below:

Dear Selection Committee:
Johnson Engineering is pleased to submit our response to the Request for Professional Services (RPS) for the Construction
Engineering Inspection (CEI) services forthe improvements on both Hamilton Avenue and Danford Street.We have assembled an
inspection team dedicated to helping facilitate these multi -corridor roadway enhancement projects.
If requested, we have available the use of an aerial drone for construction site surveillance, via Capital Consulting Solutions, to
monitorthe project site with a bird's eye view as an optional service.This aerial photography/videography would not only beneftthe
County from a progress monitoring standpoint but could also potentially be utilized for public outreach to the Bayshore community
for the construction progress being made. We are committed to assisting Collier County answer the resident's questions and
providing the community with current information on the project status as it progresses. We understand these improvements will
likely be constructed as separate projects with the Hamilton Avenue improvements beginning first.To that end, our team is flexible
and has additional CEI staff capacity to serve overlapping projects, should it be requested by County staff. Additionally, our team
also includes experienced landscape architects and an arborist who are available if there is a need for additional landscaping, root
barrier protection, or irrigation conflict resolution or monitoring during construction.
Our team is well suited for this project for several reasons:
Local Representation - We are familiar with standard roadway and Collier County construction requirements, local
agency staff, contractors, project specific landscaping concerns, and area utilities. Additionally, our office is located
just minutes from the project site(s). If needed, our office may serve as a location available for storing project
information and conducting construction progress meetings.
Qualifications -Johnson Engineering inspectors are prequalified by Florida Department of transportation (FDOT) and
our inspectors have the necessary Construction Training Qualification Program (CTQP) certifications.
Expertise -We have considerable in-house expertise in a wide range of related services, if needed.
Commitment - Johnson Engineering has had an office in, and has remained committed to, Collier County for over 28
years.
Our team is well versed and very comfortable in providing the desired CEI and testing services. As a local firm just minutes away

Johnson Engineering has a full transportation department dedicated to the design and construction
of roadway projects such as these. Our team for this project was selected specifically because they
have performed similar projects in the past, i.e, roadway construction projects for local municipalities
in Southwest Florida.This is what we do. Each of these team members have worked together in the
past and know what is expected, not only from each other but from Collier County as well. This
overall project team has a tremendous amount of experience in providing construction assistance and
CEI services on successfully completed CEI projects in and adjacent to Collier County. We will apply
the ability and skill gained through years of experience within the general vicinity of the projects to
the direct benefit of the County. In addition to our design review and CEI experience, our team also
provides a wide range of ancillary and optional services that often prove useful on projects such as
these including landscaping, arborist, irrigation, other utility, material testing (via Ardaman & Associates),
and aerial drone photography (via Capital Consulting Solutions) support services during construction.
In working for Collier County, we make the County's goals, objectives, and interests our own. In
this case, the goal of the corridor improvements will be to deliver project CEI documentation in a
professional and timely manner. We strive to function as an extension of the County. In doing so, we
will facilitate the overall CEI process to make sure the conditions of the construction contract are met
and stay on schedule.
Upfront, ourteam's focus will be to meet with design Engineer and discuss the overall project timeline
and discuss any potential construction related issues. We will also begin immediately coordinating
with the utility companies within the right-of-way to help clear the path for construction. Once the
contractor has been selected, we will meet with County staff, the project engineer, and roadway
contractor to verify the schedule of construction activities and to ensure all shop drawing reviews
are properly addressed prior to the project start. When all pre -construction preparations have been
adequately addressed, our team will initially ramp up for administering the Hamilton Avenue project
as the County's representative. Our proposed team is familiar with Collier County procedures as well
as the FDOT Construction Project Administration Manual (CRAM), which details standard operating
procedures for CEI construction administration activities. Our actual field inspection services will
generally follow these guidelines. As the construction documents are based on County Specifications,
our adherence to industry standard inspection and documentation procedures will help ensure each
project is built per the contract documents.
Prior to release of either the Hamilton Avenue or Danford Street projects for construction, our team
will perform thorough pre -construction reviews of the plans.The purpose of these reviews will be to
help ensure that all items are correctly quantified and accounted for in the bid documents.We will also
be reviewing existing field conditions and existing utility locations to help mitigate any delays caused
by unforeseen conflicts.This additional up -front measure repeatedly saves both time and money once
the project goes to construction.

Ryan Bell, PE, Ill I Principal -in -Charge / Senior PE
Ryan serves as the director of the transportation group and has a degree in civil engineering from the
University of Florida. He was born and raised in southwest Florida giving him a keen understanding
and inherent appreciation for local transportation issues. Ryan has 22 years of experience in roadway
design and construction right here in Southwest Florida, providing him a comprehensive understanding
of the big picture as it pertains to transportation improvements in our community. With a wide range
of experience, he has a clear understanding of what it actually takes to get a project done, not just
what the books and manuals require. Ryan understands that as a consultant our job is to take care of
you, the client, and to help you meet your goals on your terms.
Josh Hildebrand, PE, PTOE I Project Manager
Josh currently serves as a design engineer and project manager in our transportation group. With his
primary focus on both corridor design construction administration, he understands what must be done
to successfully design and construct roadway projects to best serve both the client and the public's
needs. Under the direction of Ryan Bell, Josh has worked on various projects throughout southwest
Florida including several roadway projects in Collier, Lee, Hendry, and Charlotte Counties. Josh will
serve as the project manager on these projects. He was the project manager for the Tree Farm Road
Phase II Post Design Inspection and Verification Testing and Golden Gate Boulevard Phase II contracts
for Collier County. He is also currently serving as the CEI project manager for the replacement of all
existing sidewalks within Pelican Bay.

John Glenn, PE I Project Engineer/Administrator
John currently serves as a project engineer working in both our design and CEI departments. With his
primary focus being roadway construction projects for local municipalities, he understands that being
mindful of budget constraints and efficient solutions to unforeseen issues are crucial to the success of
these projects. Having served as design engineer, inspector, and project administrator on several Lee,
Collier, Hendry, Glades, DeSoto and Okeechobee Counties, John has a comprehensive understanding
of both roadway design and construction.
Kathy Hanagan I Contract Support
Kathy has 31 years of diversified experience in management and construction administration, including
contract support for CEI projects in Lee, Collier, and Hendry Counties. She specializes in construction
documentation management including coordinating submittals, RFI's, estimates, meetings, contract
changes, CPM schedule variance reviews, subcontractor documentation, letter composition, and EDMS
attributing.
Roy Godshall I Senior Field Inspector
Roy joined Johnson Engineering in 1987 working as rodmen in the surveying group. In 1988, he
was transferred to the firm's transportation group as a construction inspector. Roy has 33 years of
experience working on FDOT and County Transportation Projects located in District One. He has
worked on a wide variety of transportation and utility improvement projects with varying degrees
of complexity as project administrator, contract support specialist, and senior lead inspector. Roy
currently holds, and is continuing to seek and renew, numerous certifications in the Construction
Training Qualification Program (CTQP) qualifications as required by FDOT and FHWA. Roy currently
serves the firm as project administrator and is a multidiscipline person who can perform and fit in and
adapt in many work -related roles.
Ron Ward I Field Inspector
Ron has 22 years of experience working in the CEI industry on various FDOT and County construction
projects. He has considerable CTQP training and continues to seek qualifications. Ron has extensive
asphalt paving inspection experience. He is familiar with the FDOT lot package system and well -versed
on monitoring all aspects of a paving operation. Ron is also experienced in roadway construction, major
utility construction as well as FDOT materials testing requirements. His duties include performing
earthwork testing, field concrete testing, checking and verifying various elements of construction for
compliance with plans, specifications and performing turbidity monitoring tests and reports as required
by the various permitting agencies.
Brennan McGill, El I �spector
Brennan serves as an engineer intern and construction inspector with the transportation group at
Johnson Engineering. Brennan has gained experience performing a variety of design and CEI tasks in
LEE, Hendry, Collier and DeSoto Counties. He recently graduated with a degree in civil engineering
from Florida Gulf Coast University and is a Charlotte County native. Brennan will provide inspection
services for these projects.

Ryan joined Johnson Engineering in 1998 and is the firm's director of transportation. He has worked on

culvert construction. Ryan served as the SR PE.
Colonial Boulevard Widening, Lee County - CEI project engineer for three miles of four
to six -lane roadway widening including widening of the bridge over Six Mile Cypress Slough
Three Oaks Imperial Parkway, City of Bonita Springs - CEI project engineer for four miles
of new four -lane roadway and one mile of four to six -lane widening construction including
a 285-foot bridge, drainage and signalization.
Gasparilla Road (CR 771), Charlotte County - Design FOR and CEI Senior PE for this
CM at Risk project including 2.5 miles of 2 to 4 lane widening with associated utility
relocation, drainage, signalization, street lighting and a new pre-fab deck span bridge over
the Butterford Canal.
SR 739 (Old 41), Lee County - Senior project engineer for nearly two miles of two to four
lane widening. This FDOT construction project in North Fort Myers includes significant
drainage, utility relocation and maintenance of traffic coordination.
CR 832 (Keri Road), Hendry County - Senior project engineer for CEI on 13 miles of
milling, resurfacing, shoulder construction and isolated base reconstruction of this two-lane
rural roadway in central Hendry County. This was an ARRA stimulus project administered
by FDOT and recently completed.
Design Experience
Alico Road, Lee County - Ryan served as project manager for this two to four -lane
roadway reconstruction project from Ben Hill Griffin Parkway to Airport Haul Road in Lee
County. In addition to roadway improvements the project included associated survey,
utilities, landscaping, signalization and signing and marking and all related permitting.
Burnt Store Road Phase 2, Charlotte County - Ryan served as project manager on this 4-
mile, two to four -lane roadway reconstruction project in Charlotte County. The project
includes associated drainage, utilities, lighting, signing and marking and five box culverts.
Gasparilla Road (CR 771), Charlotte County - Ryan served as project manager for 2.7
miles of two to four -lane roadway reconstruction from Rotunda Boulevard to SR 776 in
Charlotte County. In addition to roadway improvements the project included associated
survey, utilities, landscaping, signalization, signing and marking, all related permitting and a
new bridge over the Butterford Canal.

six -lane roadway including a bridge over a railroad.

John currently serves as Project Design Engineer and CEI Project Administrator in the Transportation
Group at Johnson Engineering.. John's experience includes both design and CEI on various projects
throughout Lee, Hendry, Glades and Charlotte counties. Serving as design engineer and CEI project
administrator on many of these projects has given John an understanding of what it takes to successfully
complete both the design and construction phases of a project.
Relevant Design Experience
CR 862 (Vanderbilt Beach Rd), Collier County — Project Engineer for the design of nearly
one mile of paved shoulders / bike lanes on a two lane undivided rural roadway. This project
was Federally Funded through FDOT's LAP program. John was responsible roadway design
and coordination with FDOT through its LAP program.
Sidewalk, Bicycle Lane, and Pathway Feasibility / Constructability Studies —John served as a
team member for the preparation of constructability assessments for the implementation of
pedestrian facilities and bike lanes at a number of locations throughout Collier county.
County Road 771, Charlotte County - Team member for the design of approximately two
and a half miles of roadway widening from two to four lane rural roadway including all
associated drainage, permitting, signalization, and signing and pavement marking. This project is
currently under construction.
County Road 733, Glades County - Design team member for milling and resurfacing, drainage
improvements and signing and marking. This project has been completed.
State Road 739 Miscellaneous Improvements, Lee County - Design team member for
roadway improvements including the addition of drainage structures, sidewalk reconstruction,
driveway reconstruction and placement of fencing. This project has been completed.
Alico Road, Lee County - Team member for the design of approximately two miles of
roadway widening from two to four lane urban roadway including all associated drainage, and
permitting. This project is currently underway.
Gasparilla Road, Charlotte County - Team member for the design of approximately one half
mile of roadway widening, milling and resurfacing. This project has been completed.
Bass Road, Lee County - Design team member for roadway and signalization improvements
atthe intersection of Bass Road and Park Royal Drive forthe new Golisano Children's Hospital.

Relevant CEI Experience
Helms Roadway Extension, Hendry County - John is currently serving as the Senior Inspector
and Assistant PA for approximately three miles of new four -lane urban roadway
improvements. His roles include soil density testing and sampling, concrete testing, review of
pay requests, shop drawings and RFI's. This project includes the construction of four box
culverts and one box culvert extension. John also served as a design team member on this
project.
County Road 720, Glades County - John participated in both the design and construction
inspection for approximately two miles of roadway resurfacing and drainage improvements.
This project was completed on time and on budget.
County Road 78 Phase III, Hendry County - John served as both a design team member and
inspector for the construction of approximately three miles of roadway widening, associated
drainage and milling and resurfacing. This project has been completed.

Roy joined Johnson Engineering in 1987 working as rodmen in the surveying group. In 1988, he was
transferred to the frm's transportation group as a construction inspector. Roy has 32 years of
experience working on FDOT and County Transportation Projects located in District One. He has
wormed on a wide variety of transportation and utiltiy improvement projects with varying degrees of
complexity as project administrator, contract support specialist, and senior lead inspector. Roy
currently holds, and is continuing to seek and renew, numerous certifications in the Construction
Training Qualification Program (CTQP) qualifications as required by FDOT and FHWA. Roy currently
serves the firm as project administrator and is a multidiscipline person who can perform and fit in and
adapt in many work related roles.

Colonial Boulevard (CR 884) Six Laning from 1-75 to SR-82 Capital Improvement - 2.62
miles of roadway widening related construction activities, roadway, lighting, signing, and
one bridge 500 lineal feet over six -mile slough. Roy was the project administrator.
Six -lane improvements of Collier Boulevard from Golden Gate Boulevard to
Immokalee Road - 2.5 miles of widening two lanes to six lanes in an urban area. Adding
curbing, roadway lighting, additional storm drain and three signalized intersections. Roy
was the project administrator.
Immokalee Road widening from 43rd Ave., N.W. to Collier Blvd., Collier County - This
project included widening a two- to six -lane roadway approximately seven miles long with
storm drain, earthwork, sidewalks and street lighting, curb and gutter, paving and box
culvert construction. Roy served as senior inspector.
1-75 ramp improvements at Immokalee Road interchange for FDOT and Collier
County Transportation Services Division - This project widened the 1-75 on/off ramps
along with the widening of Immokalee Road from Oaks Boulevard to Strand Boulevard
(approximately one-half mile) through the interchange including storm drain, sidewalks,
curbing and lighting with four signalized intersections. Roy served as senior inspector.
Veterans Boulevard widening in Port Charlotte, for Charlotte County Public Works -
This project included 6.2 miles of roadway construction between Toledo Blade Boulevard
and Peachland Boulevard converting the two-lane roadway to four -lanes including
stormwater drainage, earthwork, lighting and sidewalks along with business coordination
and entrances for condominium developments. Roy provided construction management
services for this project.
Bonita Beach Road Phase II Widening - This project included two bridges and 2.4 miles
of new four -lane roadway construction. The project required coordination between two
counties and complete reconstruction of major utilities for two large utility companies, as
well as telephone, electric power and cable systems. Entire project completed in 18
months and was honored with the APWA National Project of the Year Award for 1998.
Florida Cities Water Company - Roy coordinated and performed quality assurance for
installation of 16" waterline main for which was designed to supply Health Park Florida.
Westinghouse Gateway Communities, Lee County - Roy was the lead field inspector on
a 16" waterline project for the Gateway Services District

Mark joined Johnson Engineering in 2017 as a Senior Landscape Designer. His experience in
technology, certification as an ISA Arborist, Florida Master Naturalist and former masonry
construction business owner, inform his designs and contribute to the success of challenging projects.
He brings with him a diverse set of skills that allow him to confidently take projects of a range of
scales, from concept to completion, while meeting functional constraints and incorporating creative

members during a design charrette that clearly illustrated planting design proposals, cross -

cost data created for bidding and installation for Collier County.
Dade City Florida, Bicycle Trailhead Park - Senior Landscape Designer on a team that
provided conceptual and diagrammatic solutions to revitalizing an underutilized City
property that emphasized existing neighborhood connections, preserving mature tree
canopy while promoting user access to a regional trail network for bicyclists and
pedestrians. Site improvement suggestions included restrooms, bicycle tune up stands,
picnicking areas, water fountains for people and their pet friends, trail signage, as well as on
and off-street parking.
Billy Bowlegs Park, Fort Myers, Lee County — As part of a team effort, the Johnson
Engineering team generated 3 completely unique concept plans for an existing
neighborhood park that envisioned approximately 180 paved parking spaces, drive turn-
around drop-off areas, covered basketball courts, practice and team play soccer fields with
home and away bleachers, audio sound systems, a circulation path through the site and the
provision for existing and proposed offices, restrooms and on -site food concession.
Hancock Bridge Parkway Median (1.87m) Landscape Design - Senior Landscape Designer
that facilitated the collection of base information and gathered site data with field visits to
create an informative existing conditions plan. Subsequent plans, based on information
acquired during a meeting with the City of Cape Coral leadership and community members
included recommendations for both tree removal and new alternative plantings that would
bring the medians into compliance with F.D.O.T. sight visibility requirements for turning
vehicles. Final constructions plans provided the city guidance for avoiding conflicts with
existing utilities, connecting proposed low volume irrigation to existing reclaimed water
service packs and a landscape that would evolve into a more diverse and user-friendly
version of what was currently available.
Charlotte County Government Campus Tree Inventory and Analysis - Senior Arborist
consultant to Charlotte County. JEI worked with the County Facilities Director to identify
exceptional existing trees that deserved extra consideration or increased planting area as
well as trees which would be better removed and replaced with new candidates that would
provide shade and other environmental/ cultural benefits in the future. Plans were
developed that provided guidance on root pruning techniques, root barrier options and a
tree removal/ protection plan with trees identified for removal, protection or replacement.
Lee Health Golisano Children's Hospital Parking Lot Existing Tree Canopy Evaluation -
Senior Arborist, JEI consulted to the Facilities Project Manager for Lee Health to identify
critically hazardous trees to be removed in the immediate future (prior to a re -pavement
program), recommended trees for removal based on limited growing area for a species that
requires large volume planting areas, and suggested future removals for trees that would
become a hazard in the near future. Following the report of the tree removal
recommendations Johnson also provided a plan for suggested replacement trees that would
be more likely to succeed in the constricted planting diamonds located throughout the
parking lot.

Mr. Kuehn is responsible for determining scope of services and manpower requirements for geotechnical and
groundwater projects. He is involved in siting, design, groundwater monitoring, in situ permeability testing,
infiltration capacity testing, geophysical surveys and aquifer pump testing associated with liquid and solid waste
disposal systems. He is also involved with physical soils testing, deep and shallow foundation design, settlement
analysis, slope stability analysis and excavation dewatering system design. He plans and supervises field and

Mr. Kuehn recently completed geotechnical investigations to evaluate the general stratification and

Mr. Kuehn provided geotechnical services for replacement of the existing structure on the Cocoplum Canal.
Activities included SPT borings, laboratory testing, and analysis for the proposed sheet pile barrier.
• Greater Port Charlotte Storm Structures, Charlotte County, Florida
Mr. Kuehn analyzed soil samples and laboratorytest results as part of the project team forthe replacement
of six water control structures over selected waterways in Charlotte County. Services included a
geotechnical investigation and foundation design recommendations.
• Coral Creek Weirs, Charlotte County, Florida
Mr. Kuehn was responsible for determining scope of services of geotechnical explorations fortwo proposed
weir (water control) structures in Coral Creek. This included directing field explorations and laboratory
testing; then performing foundation design analysis, settlement analysis and lateral stability analysis fortwo
different design alternatives, including a reinforced concrete cantilever wall and a sheet pile wall. Analyses
performed also included assessing the effectiveness of subsurface seepage cut-off walls. A geotechnical
engineering report was prepared to summarize the results and conclusions, which provided our client with
the information necessary to determine that the sheet pile wall would be the most cost-effective
alternative.

PROJECT EXPERIENCE Our experience gives us an advantage as we have a comprehensive understanding of the entire
transportation development process from planning to design, to permitting through construction. Our
proposed team has extensive experience in both roadway design and CEI. Key projects demonstrating

Golden Gate Sidewalk Improvements CEI
Collier County, Florida
Johnson Engineering provided full construction engineering and inspection (CEI) for this LAP
funded sidewalk project in Collier County. The sidewalk improvements were provided on both
sides of the roadway on 20th Court S.W. (from Sunshine Blvd. to 46th Terrace S.W), on the west
side of the roadway on 46th Terrace S.W. (from 23rd Avenue S. W. to Sunset Road) and on the
west side of the roadway on 4I 2th Street S.W. (from 23rd Avenue S.W. to 17th Avenue S.W.).
Improvements included associated drainage, safety upgrades, signing and pavement marking and
roadside restoration.
Immokalee Road Widening CEI
Collier County, Florida
Johnson Engineering provided full CEI and construction administration services for the
Immokalee Road widening project in Collier County The project consisted of 8.1 miles
of widening of a two-lane rural section to a six -lane urban section from CR 951 to
43rd Avenue NE. The project included roadway construction, stormwater drainage,
signalization, lighting, utilities, a small deck span bridge, several box culverts, and a con -
span structure.This was a highly visible project given the sheer magnitude and traffic
volumes.
rid SR 29 — Milling & Resurfacing (Phase 1)
Collier County, Florida
Johnson Engineering provided full CEI for eight miles of resurfacing,
shoulder construction, miscellaneous base repair, guardrail and sidewalk
improvements just south of Immokalee.This was a state funded project
administered by FDOT in Collier County.

SR 90Turn Lane & Pedestrian Crosswalk Safety Improvements
Miccosukee Tribe of Indians
Johnson Engineering was contracted by the Miccosukee Tribe of Indians to design and provide
CEI services for safety improvements to the two main entrances to the Miccosukee Tribes
entrances located on S.R 90. Improvements consisted of the addition of guardrail, turn lanes,
installation of a ADA compliant crosswalk with solar powered lighted signage, new sidewalk
improvements, paved shoulders and milling and resurfacing. This project was Federally Funded
and permitted through FDOT.
Santa Barbara Boulevard Landscape & Irrigation Design & Construction Oversight
Collier County, Florida
Collier County contracted with Johnson Engineering's
Landscape Architecture team in an initiative to design landscape
and irrigation from concept planning through construction
oversight on 1.7 miles of roadway within the medians of Santa<
Barbara Boulevard from the 1-75 bridge to Copperleaf Lane.
The design is based on the Collier County Best Management -
Practices for Landscape Design and Permitting within the Right
of Way, Collier County Best Management Presentation, and
Florida Friendly Principles.With keen attention to community
feedback, the landscape design echoes the existing landscape
of connecting roadways of existing neighborhoods and that
of Collier County to create a bold and sustainable landscape
befitting the character of this major Collier County corridor.
Collier Boulevard
Collier County, Florida
The Collier Boulevard widening project known as (Collier Boulevard six -lane from
Golden Gate Boulevard to Immokalee Road) is a 3.03-mile roadway that converted
an existing rural section to an urban section with curb and gutter. The original
design was to widen from four -lanes to six lanes for added capacity. Collier County
determined prior to construction that in the best interest of cost and Level of Service
Jill of the road it was beneficial to increase the widening to an ultimate six -lane design
prior to construction bidding.The project included access management, stormwater
collection, conveyance and treatment facilities, environmental permitting, mitigation,
signalization, street lighting, sub -consultant coordination (survey, geotechnical, noise
studies, and aerial photogrammetry), regulatory agency coordination (SFWMD,
ACOE, EPA, USFWS) and significant utility coordination.The original estimated price
was set at approximately $40 million; the final construction cost came in just under
$37 million for a cost savings of $3.8 million.

Estero Boulevard
Lee County, Florida
Johnson Engineering Inc. was selected to provide CEI services, phases 2 thru 6, for the Estero Blvd corridor improvements on Fort
Myers Beach.This is a multi phased project constructed via CM at Risk to completely reconstruct a very busy and narrow corridor.
Improvements include reconstruction of a flush shoulder
roadway to a curbed facility with closed drainage and
pedestrian/bicycle facilities on both sides of the road. For
much of the corridor roadway drainage is achieved thru an
underdrain system beneath pervious pavers in the center
turn Iane.The project had to be constructed while maintaining
traffic and continuouys bike, ped, business and residential
access. The project also required a tremendous amount of
=_ utility relocations, coordination amongst the Town of Fort
Myers Beach, Lee County DOT and Lee County Utilities.
Homestead Road
Lee County, Florida
Johnson Engineering provided CEI services for the Homestead Road 4-laning improvements
in Lehigh Acres.The project includes roadway, drainage and utility improvements in a very .r
tight and congested corridor.The alignment also includes a fully super -elevated curve. During
construction Johnson Engineering inspectors noticed staking discrepancy which caused the
curbs to be set at the wrong elevation.The contractor was made aware and able to remedy -,
the situation before the adjacent roadway base and asphalt construction. 'y
Helms Road
Hendry County, Florida
This project included design and CEI of nearly three miles of new four -lane
roadway in Hendry County.The design and plans were developed to allow phased
construction and better suit the available funding. As a new roadway through an
existing and active citrus grove the design incorporated considerable offsite drainage
improvements, including four large box culverts, to the existing grove so that it
may remain functional during and after construction. Johnson Engineering provided
all project management, design efforts and CEI for the roadway system including
environmental analysis and permitting, survey, ROW mapping, drainage, signing and
marking, signalization and permitting through FDOT.

Keeping a project on schedule and under budget requires a project team that can quickly resolve issues
as they arise. Johnson Engineering's staff is unique in that many of our engineers are crossed -trained in
both design and CEI. Our staff's design proficiency provides us with an understanding of the designers
intent. Simply put, understanding why you are building something provides significant benefit during
construction. Our experience allows us to quickly and confidently make decisions to keep things
moving forward.
Johnson Engineering's approach to CEI is what sets us apart. Not only will our staff monitor the
progress of construction and perform the required materials testing and documentation, we will also
be looking ahead, identifying and resolving conflicts to help prevent unnecessary delay and added
contract cost. When issues arise, our experience allows us the confidence to make decisions quickly
and confidently, regardless of whether they are design or construction in nature.The primary purpose
of the CEI team on these projects will be to provide inspection services, quality assurance engineering,
construction administration, and geotechnical testing in accordance with the requirements set forth
by Collier County. During construction, our staff will work closely with the contractor to promote
project progression and efficiency of ongoing work, improve safety, and foster a partnering attitude to
achieve the common goals beneficial to all parties.The success of the CEI consultant on these projects
will depend on their ability to facilitate the progression of work, perform and document all required
materials testing, maintain timely and accurate records, and control the cost during construction. We
are confident in our approach as our team has a history of successfully completing similar projects for
Collier County as well as neighboring Hendry County, Lee County, Charlotte County, DeSoto County,
and Glades County.
Material Sampling and Testing
We understand the plans reference FDOT specifications and as such the CEI staff will be required to
provide verification testing on this project. Our inspectors all hold CTQP qualifications in earthwork,
asphalt paving and concrete testing. We will equip our inspectors with nuclear density gauges during
critical earthwork operations so verification testing can be performed if irregularities or areas of
concern are identified. Additionally, our materials testing sub -consultant, Ardaman & Associates, has
extensive experience in asphalt plant inspection. It is our experience on these projects that plant
inspection is vital to the quality control of the paving operations.
We are very familiarwith the FDOT testing requirements and will use the FDOT developed Earthwork
Record System for plotting pipe backfill. Our asphalt paving and plant inspection will also generally
follow the requirements outlined by the FDOT. Lot packages for each project will be produced
including copies of all plant tests, field paving reports, and asphalt tickets.
-Mecordkeeping
Our personnel will maintain complete, timely, and detailed records relating
to each project. Dailywork reports will be completed electronically, detailing
contract time, weather, quantity increases, and equipment utilization. In
addition to reporting daily work, we also develop an ongoing picture library
cumenting each step of construction. Our inspectors are trained to
current controlling work items each day with pictures. All project files will
organized per our internal filing index and reviewed for completeness by

her to being recognized by FDOT for her outstanding work on state projects.

Maintaining existing drainage in the Phase I A scheme
of the maintenance of traffic (MOT) plan.
The proposed MOT plan specifies
constructing a temporary roadway on the west side of
Hamilton Avenue in the existing roadside swale.To maintain
existing drainage patterns under construction, temporary
drainage facilities will need to be constructed. Johnson
Engineering's CEI engineers are well versed in the design of
storm drain facilities and can assist in accommodating these
drainage facilities during construction.
Maintaining traffic throughout the roundabout construction.
Johnson Engineering has experience both designing and providing CEI
services for roundabout projects. Our CEI team is currently wrapping up construction on a
roundabout project in Lee County. We have firsthand experience on roundabout projects to
work with the contractor in establishing a phasing and MOT sequence for the construction of
the roundabout.
Proximity of the project to the Bayview Park boat ramp and consideration for truck /
trailer traffic through the work zone.
Being mindful of the truck and boat trailer traffic when laying out the MOT
for the project is a must. It will also be imperative to keep the project clean and MOT devices
correctly placed prior to leaving the project location on Friday afternoon in preparation for the
weekend traffic to the boat ramp.
Organic material in Stratum 4 with a soil classification ofA-8.
RESOLUTION: A-8 materials are designated as muck in the FDOT Embankment Utilization
guide and are not permitted under the roadway. Our staff has experience in delineating
areas that require de -mucking and can work with the FOR to determine the limits of subsoil
excavation to be performed to prevent future settlement of the roadway.

The proposed pavement section on the Hamilton Avenue project specifies
a 9"Type B 12.5 black base. If the contractor was able to remove the existing asphalt, scarify the
base and utilize a Iimerock base in lieu of black base a considerable cost savings is IikelyThese
savings could offset the cost of the subgrade under the areas of the proposed widening (i.e.,

MAINTAINING THE
SCHEDULE
By "maintaining" the schedule, we do not mean simply getting into the software program and updating
the document itself. This is important and needs to be completed, but "maintaining" the schedule in
the field typically proves to be the hard part. With so many factors influencing and impacting the
construction progression, getting it done and keeping up with your own intentions becomes a challenge.
✓e have no direct contractual relationship with the contractor, the contractors resources, means
methods are their own. We have found that taking a proactive approach, doing more than simply
imenting construction activities and performing the required testing, is the best way to facilitate
truction and keep the project on track and within budget. It has been our experience that quickly
Ling the contractor in conflict resolution and maintaining proper documentation are ways to "take
away the excuses" and keep the project on track.
One of our team's strengths on these projects is in the combined construction and project -
specific design experience. Knowing what you are building and why, helps you to build it better.
Over the years, we have designed, built, and permitted many similar facilities with similar roadway
and drainage aspects. When problems arise, and they will, we will snap the pictures and take the
notes like any other inspection team, but unlike many others, we can help you do something
about it. We have the knowledge to help solve the problem.
A key component of promoting project progression is resolving issues at the lowest level possible.
This can also have a positive benefit on the budget. It is our intent to apply an escalation matrix
for conflict resolution. We have found that this process maximizes the efficiency of the decision -
making process. We anticipate regularly scheduled progress meetings with the County, contractor, and
inspection staff. At these meetings potential issues, construction observations, RFI's, schedule, and any
testing issues will be discussed.
Controlling Cost During Construction
Our team has an excellent track record of completing roadway projects within the budgeted amount.
Our success is attributed to our experience in both design and construction and our proactive
approach to these projects. Discrepancies must be resolved before they are constructed to avoid
unnecessary expense. With a CEI team who understands the design intent, we are able to review
the plans, bid and specifications, to ensure all work is accounted for and check the accuracy of all
quantities. Should there be an oversight, or quantity "bust", it is imperative that it is discovered as
soon as possible. Early identification of plan oversights allow for them to be mitigated through value
engineering, mitigating any additional expense to the County.
On a recent SCOP funded project in which we provided CEI services, construction bids were
$60,000 in excess of the grant funding amount. With the roadway in need of immediate
repair, the local agency agreed to pay the additional $60,000 out of their very limited budget.
After review of existing conditions and the contract documents, our staff recommended
importing additional fill to establish the required clear zone in lieu of the planned guardrail
improvements. The result was a net savings of over $70,000.This change was coordinated
with the FOR and FDOT and did not result in any contract delay due to timely decision
making.
Efficient use of funding is not limited to cost reduction. We have found in some instances there
are opportunities to implement additional improvements. Our staff maintains their own monthly
estimate. In addition to using it to check the contractors pay application, we also use it to project the
final construction cost. As items are completed over and underruns are highlighted. On a recently
completed SCOP and SCRAP funded project in which Johnson Engineering provided CEI services, we
projected a total contract underrun of $65,000 at substantial completion.With many existing signs not
meeting MUTCD requirements and no provisions in the plans to replace them, our staff recommended
overrunning the sign quantity on the project to provide an additional 61 street signs throughout the
residential area.The result was $20,000 in signing improvements paid under the existing contract.

MANAGEMENT APPROACH
Having performed numerous similar projects over the years, we have developed an approach to
help us get these projects done in the most efficient manner possible. Though site conditions will
vary for every CEI project we do, the overall requirements and deliverables generally stay the same.
Our standard "plan of attack' has served us well in the past and we believe is well -suited for these
projects. For the Hamilton Avenue and Danford Street Corridor Improvement projects, the project
management will be administered out of our Collier County office with our inspection staff reporting
to that location when not at the project site.The following narrative provides a detailed description of
our approach to the corridor improvements.
Pre -Construction Activities
The pre -construction phase is critical to a successful project. It is imperative that all entities share
the same expectations and goals. These should be clearly defined and communicated in the pre -
construction phase.The following is a summary of the pre -construction phase sub -tasks:
Constructability Review - If desired by the County, the roadway improvement plans will
be reviewed prior to bid both by our construction personnel and design staff. Ryan Bell,
John Glenn, and Josh Hildebrand will work collectively in this effort to provide additional
QA/QC to help illuminate any potential sticking points before the plans are bid.
Pre -Construction Site Inspection - Our inspection staff will take comprehensive pre -
construction videos and photographs to fully document the existing conditions. It is critical
to document the baseline condition in evaluating potential future claims by the contractor
and adjacent residents.
Utility Coordination - Often times when a project goes to construction, it has been several
months since the Utility Agency Owners (UAOs) have reviewed or even thought about
the project. We suggest and will facilitate pre -construction utility coordination meetings
with the affected UAOs even if the contractor is not selected yet. These projects are a
collective effort and it is helpful to have the UAOs thinking about any relocations they may
have as soon as possible, so they can get the ball rolling on their end.
Pre -Construction Meeting - Our team can facilitate the pre -construction meeting once
the contractor is on board to discuss the project requirements. Specific topics include
establishing lines of communication, authority and escalation, reviewing documentation
procedures, reviewing all permit and contract document requirements, and discussing
specific project issues.

General Management and Project Administration
The day-to-day administration of the project during construction is led by the Project Manager and
assisted by the Contract Support Specialist (CSS).We will coordinate with County staff upfront on the
level of project administration expected of our staff versus those services to be handled by County
staff. Below is a summary of the sub -tasks associated with CEI Project Administration:
Project Coordination - The Project Manager will serve as the primary coordinator
between the contractor, engineer of record (EOR), and County staff.The Project Manager
will take a proactive approach in making sure everyone is on the same page, working
together and moving forward.
Progress Meetings - Meetings will be held on a regular basis, either weekly or bi-weekly
at the County's discretion. We have a regular agenda to discuss the necessary topics
including project status, submittals, reporting status, RFIs, pending issues, upcoming issues,
etc. Meetings are recorded and minutes are kept, being distributed to all interested parties
via e-mail.
Tracking Logs - Several tracking logs are utilized to monitor and document on -going
project issues.The logs include dates and entities responsible for resolution.
Pay Application Review - Our Project Manager or assistant Project Manager,with assistance
from the field personnel confirming quantities, will review and recommend approval/denial
on the contractors pay request making sure the contractor is being compensated fairly
and the County is getting what they are paying for.
Problem Resolution - Perhaps the most important and yet intangible function of the CEI
team is assisting in problem resolution. When issues arise, our CEI takes a proactive and
aggressive approach in pursuing their resolution. Problems will be addressed quickly and
completely.
Construction Inspection
The regular construction observation and inspection falls on our inspectors. We have a team of
inspectors available that are CTQP-certified for this type of work and have considerable "FDOT style"
inspection experience, much of it in Collier County.We understand how these facilities are supposed
to be built and we know what to look for. Below are some of the sub -tasks associated with the
construction inspection phase:
General Observation -The inspector will monitor the construction activities to determine
whether the items constructed are in general accordance with the contract documents.
Noted field issues are documented and incorporated in a tracking log.
Daily Inspection Reports - The inspector shall utilize the standard FDOT daily report
format to document each day's construction activities including items of work, quantity
placed, location, crew and equipment used, and note any special conditions. A log of
"dailies" will be maintained throughout the entire project.
Tracking Quantities -The inspector shall note and track quantities of all installed work
items forthe duration of the project to substantiate and assist in reviewing the contractors
pay requests.
Photographic Documentation -The inspector will document the construction progress,
specifically noting any special/problem areas.
Permit Compliance Monitoring -The inspector shall monitor the contractors activities to
determine if they are in compliance with the permit requirements. Any apparent deviations
will be documented and immediately brought to the contractor's attention.
Field Testing - The inspector shall perform field testing such as concrete slump, air, and
W/C ratio testing for concrete construction or obtain densities for compaction of base
material.

Project Close -Out
As the construction completion nears, it is essential to properly document and confirm all work items
have been addressed to the County's satisfaction.There is a set process to make sure this is achieved
prior to approval of the contractors final pay request. Below is a summary of some of the project
close-out sub -tasks:
Survey As -built Review - CEI shall review the preliminary as -built survey to ensure all
necessary information has been collected and recorded to prepare official record drawings.
Any missing items or deficiencies will immediately be brought to the contractor's attention.
Final Completion Inspection - After the punch -list has been completed, the CEI will
facilitate the final walk-through with CEI, contractor, FOR and County staff in attendance
to determine whether the work is complete. If further deficiencies are found, they are
brought to the contractors attention.
Final Construction Certification - Upon issuance of notice of final acceptance and
recommendation for final payment and receipt of all required close-out documents, i.e.,
release of liens, warranties, O&M manuals, as -built survey, etc., the CEI will make an official
recommendation of completion.
Document Turnover - Upon completion and final acceptance of the project, the CEI shall
provide a digital copy, and hardcopy if requested, of all documentation collected on the
project.This information will be clearly organized and indexed for ease of future use.
Our Collier County office is in close proximity to the projects which allows us the flexibility and
responsiveness to manage our day-to-day onsite data collection, store testing equipment, attend
meetings, be onsite, or at the County offices on short notice, and provide project information by hand
if needed. Additionally, our office may be used as the location to store project information and host
construction progress meetings, if desired by Collier County staff.

For projects in excess of $200,000, bonds shall be submitted with the executed

Liability where required. This insurance shall be primary and non-contributory with respect to any other insurance maintained
by, or available for the benefit of, the Additional Insured and the Vendor's policy shall be endorsed accordingly.
