EXECUTIVE SUMMARY
Recommendation to approve Year 1 of a 3-year Centers for Disease Control grant award to
support the Prevention Community Health Workers for COVID-19 Response for Resilient
Communities Program in the amount of $421,744 and authorize the necessary Budget Amendment.
OBJECTIVE: To provide accessible and equitable healthcare in the Immokalee farmworker community
to improve the response to the COVID-19 pandemic.
CONSIDERATIONS: On June 8, 2021, the Board of County Commissioners (Board) approved the
submittal of a Centers for Disease Control and Prevention (CDC) Community Health Workers for
COVID-19 Response for Resilient Communities Grant application in the amount of $1,446,255. The
grant supports the implementation of an outreach program to support those most impacted and at risk of
COVID-19. Funding will be used for hiring and partnering with individuals who represent the racial,
ethnic, and cultural diversity of Immokalee and similar communities in the areas such as Golden Gate
City. The staff will focus on the social and structural context of the community within a network of
clinical supports. The Healthcare Network of Southwest Florida along with Partners in Health will
coordinate this project and Collier County will be responsible for the administration of the grant.
Collier County received notification of award for year 1 of 3 in the amount of $421,744 on August 24,
2021. Proposed project partners from Collier County Community Health Coalition (CCCHC) comprised
of the Collier County Department of Health, the Coalition of Immokalee Workers, and Mission Peniel,
who will all support the efforts to a Community Health Program. The grant will include training and
mentoring activities for Community Healthcare Workers who will serve the most aff ected by COVID-19
and integrate Community Healthcare Workers to form care teams to support COVID -19 response efforts
in our community. Year 1 of the grant program is August 31, 2021 to August 30, 2022. The award
notification is attached to this item for the Board’s consideration.
FISCAL IMPACT: A Budget Amendment is required to recognize grant funding in the amount of
$421,744 in Housing Grant Fund (705) Project 33772. There is no match required for this grant program.
GROWTH MANAGEMENT IMPACT: There is no Growth Management impact associated with this
Item.
LEGAL CONSIDERATIONS: Chair signature on the grant documents is not required to accept the
award. This item is approved for form and legality and requires a majority vote for Board approval. -
