5. The Chairman's signature line date has been entered as the date of BCC approval of the JAK
document or the fmal negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's JAK
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 9/14/21 and all changes made during the
meeting have been incorporated in the attached document. The County Attorney's JAK
Office has reviewed the changes,if applicable.

RESOLUTION NO. 2021 - 1 82
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, APPOINTING ELIDA OLSEN TO THE
COUNTY GOVERNMENT PRODUCTIVITY COMMITTEE.
WHEREAS, on January 12, 2021, the Board of County Commissioners (Board) adopted
Ordinance No. 21-02 reestablishing the County Government Productivity Committee
(Committee),which was further amended by Ordinance No. 21-16; and
WHEREAS,the Committee shall consist of eleven members, with one member appointed
from each of the five commission districts and six members at-large, in accordance with the
qualifications described in Ordinance No. 21-02, as amended; and
WHEREAS, there is a vacant seat on the Committee representing Commission District 1;
and
WHEREAS, the Board previously provided public notice soliciting applications from
interested parties; and
WHEREAS, Commissioner Rick LoCastro, District 1, has made a nomination to fill the
vacancy, in accordance with the requirements stated in Ordinance No. 21-02, as amended.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that ELIDA OLSEN is hereby
appointed to the County Government Productivity Committee representing Commission District
1,to serve the remainder of a vacant term expiring on March 9, 2023.
THIS RESOLUTION ADOPTED upon majority vote on the 14th day of September, 2021.
