FOURTH AMENDMENT TO
AGREEMENT BETWEEN COLLIER COUNTY, FLORIDA
AND
PRESTIGE HOME CENTERS, INC.
This FOURTH AMENDMENT TO AGREEMENT is made and entered into as of this

of the State of Florida (tile "COUNTY" or "Grantee") and Prestige Home Centers, Inc., existing
under the laws of the State of Florida("SPONSOR").
WITNESSETH
WHEREAS, on May 8, 2018, Agenda Item 16.D.8,the County entered into an Agreement
with Prestige Home Centers, Inc. to administer the State Housing Initiatives Partnership (SHIP)
Demolition and Replacement Manufactured of Home Program;
WHEREAS, on August 14, 2018, Agenda Item 16.D.7, the County approved the First
Agreement Amendment;
WHEREAS, on October 23, 2018, Agenda Item 16.D.5, the County approved the Second
Agreement Amendment;
WHEREAS, on April 9, 2019, Agenda Item 16.D.8, the County approved the Third
Agreement Amendment;
WHEREAS, the Parties desire to further amend the Agreement to add funding for FY
2020/2021 in the amount of$44,170.00 for a total contract amount of$1,020,670.00, extend the
contract date, and include a price increase for a manufactured home (Exhibit I);

NOW, THEREFORE, in consideration of the mutual promises and covenants contained
herein, the parties hereto agree to amend the Agreement as set forth below.

II. SCOPE OF SERVICES
The SPONSOR shall, in a satisfactory and proper manner as determined by the COUNTY,
perform the necessary tasks to administer and implement the described services herein
incorporated by reference as Exhibits B-C (Program Narrative and Budget Narrative) and
Exhibit I (Cost Sheet) in accordance with the terms and conditions of Requests for
Applications "Demolition and/or Replacement of Manufactured Home Project, SHIP FY
Years 2016-2017,and 2017-2018, dated December 4,2017 for all funding provided by this
Agreement including FY2018-2019 and FY 2020/2021.

IV. PERIOD OF PERFORMANCE
This Agreement shall be in effect from May 8, 2018 through June 30, 2021 November 30,
31, 2021 for FYs 2016/2017, 20 1 7/20 1 83_and 2018/2019 and 2020/2021 funds and all
services required hereunder shall be completed in accordance with the expenditure deadline
set forth in Exhibit C (Budget Narrative). Funds may be expended throughout the entire
period and are not limited to the fiscal year appropriation.
V. AGREEMENT AMOUNT
It is expressly agreed and understood that the total amount to be disbursed by the COUNTY
for the use by the SPONSOR during the term of the Agreement shall not exceed Nine
Hundred Seventy Six Thousand Five Hundred dollars ($976,500.00)One Million Twenty
Thousand Six Hundred and Seventy dollars ($1,020,670.00) pursuant to the quoted price
offered by the Sponsor in response to the SHIP application dated December 04, 2017, and
the price methodology as defined below.
